RATIONALE: Ankle syndesmotic injuries occasionally require long-term therapy for recovery and can result in tendon injury. Posterior tibial tendon dysfunction (PTTD) is an acquired deformity that can cause flatfoot deformity. The current nonoperative management of PTTD includes nonsteroidal antiinflammatory drugs (NSAIDs), orthopedic devices. Although various treatment options have been attempted, optimal treatments for each stage of the condition are debated. Polydeoxyribonucleotide (PDRN) is effective in healing of chronic wounds associated with tissue damage by attracting tissue growth fact

Anterior cruciate ligament reconstruction aims to improve knee stability and range of motion. The AperFix system consists of polymer components, and fixed-loop fixation is an established endoscopic technique. Our aim in this study was to compare the long-term clinical and radiological results of AperFix and fixed-loop fixation and to prove that the long-term results of the AperFix fixation method are at least as good as those of the fixed loop device. This retrospective study included 109 patients who underwent primary anterior cruciate ligament reconstruction using single bundled hamstring te

STUDY DESIGN: Retrospective review of all patients with intact spinal cords, who had posterior spinal fusion (PSF) to the pelvis with Dunn-McCarthy instrumentation. Inclusion criteria were communicative patients with Dunn-McCarthy instrumentation, functioning spinal cords, and sensate lower extremities. We excluded patients with myelomeningocele or spinal cord injury because they are physiologically unable to develop neuropathic lower extremity pain. We distinguished neuropathic lower extremity pain from positional discomfort by eliminating patients whose symptoms resolved in the first postope

Sandwich structures are known for their excellent strength-to-weight ratio and are being increasingly used in the automotive, aerospace, marine, and construction sectors. These structures may also offer promising designs for other fields such as biomedical fixation devices, in which the combination of lightness and stiffness is of paramount importance. This study investigated the potential of fabricating orthopedic external ring fixators from sandwich panels to develop innovative designs and broaden the scope of sandwich-structure applications. Semicircular and circular rings were fabricated f

Maintaining an appropriate balance between mobilization and immobilization is a central challenge in hand rehabilitation and has a decisive impact on tissue healing, functional recovery and patient outcomes. This narrative review examines the concept of balance in the hand by outlining risks that threaten this balance, reviewing the available evidence comparing mobilization and immobilization strategies, and highlighting the strength of the evidence and existing research gaps. The practical achievement of balance is explored with particular emphasis on the timing of mobilization, effective com

Surgical site infections are a major complication of instrumented orthopaedic surgery, affecting 0.5 to 2% of patients following arthroplasty, and up to 30% of patients after fixation of open fractures. Acute infections may result from exogenous inoculation during or in the weeks following surgery (early infections), or from hematogenous seeding from a remote origin at any time after implantation (late infections). These infections are generally due to virulent organisms, such as Staphylococcus aureus, and must be treated rapidly. Delayed infections, arising between 3 and 24 months after surge

The experience of 14 cases with surgical stabilization of multiple rib fracture and flail chest was reported. They were 11 men and 3 women of 31 to 87 years of age. Paradoxical chest movement was noted in 10 patients. Thirteen of 14 patients successfully weaned from the ventilator less than 7 days after surgery. Of 14, 4 cases were treated with internal fixation and the others were with acetabular reconstruction plates with or without rib stapler. No case of death was experienced. Ten patients who were performed fixation with acetabular reconstruction plate weaned from the ventilator earlier t

Totally absorbable internal fracture fixation devices were introduced clinically in the treatment of fractures and osteotomies of the extremities at our department in 1984. A total of 2,500 patients were managed using bone or ligament fixation devices made of self-reinforced (matrix and fibers of same polymer) absorbable alpha-hydroxy polyesters between November 5, 1984, and January 12, 1994. The devices used included cylindrical rods or pins, screws, tacks, plugs, and wires. The most common indication for the use of absorbable implants was displaced malleolar fracture of the ankle. Transphyse

The analysis of 35 cases of delayed healing of fractures of the upper arm treated from 1981 to 1990 shows that fractures resulting from road accidents (16/35) and comminuted fractures (16/35) are particularly frequent in this group. Delayed fracture healing after conservative treatment (10 patients) was caused mainly by inadequate reduction (4/10) or by the fact that the conservative treatment performed was in fact inappropriate (4/10). Primary osteosynthesis was performed in 25 patients. Inadequate stabilization (9/25) was the most frequent reason for delayed healing, followed by the omission

Posteriorly displaced Type II odontoid fractures (Type II-P) are difficult to stabilize in an anatomic position with accepted methods of posterior atlantoaxial arthrodesis. Nine patients with Type II-P odontoid fractures with 4 to 15 mm displacement were treated with anterior odontoid screw stabilization. Seven of these patients had associated fractures or defects of the posterior arch of the first cervical vertebra (C1). Atlantoaxial posterior arthrodesis in these patients would not have been possible initially because of the lack of structural integrity of the posterior arch of C1. Two patie

For stabilization of the lower thoracic and lumbar spine, an external spinal skeletal fixation (ESSF) system has been developed and clinically used since 1977. The system consists of two pairs of Schanz screws and an adjustable external fixation device. The Schanz screws are firmly anchored through the pedicles into the vertebral bodies. Application of ESSF offers some unique characteristics beneficial in the treatment of unstable spinal injuries and spinal osteomyelitis. In contrast to other fixation systems, the number of vertebrae immobilized can be reduced with ESSF. Versatility of the sys

BACKGROUND: A small number of patients treated with minimally invasive correction of pectus excavatum recur after bar removal. This risk appears to be greater in younger children who continue to grow following bar removal.
METHODS: We propose the use of wrist films to determine skeletal maturity and delay bar removal until it is completed. This is not possible in very young patients (less than 14 years of age) or necessary in patients older than 19.
RESULTS: In the 14-year to 18-year age group, we have used wrist films to determine skeletal maturity in 25 patients. Ten patients (age 14-18) d

Trapeziectomy with suture-button suspensionplasty is a surgical treatment option for thumb carpometacarpal osteoarthritis refractory to nonsurgical management. We describe the cases of 3 patients who presented with index metacarpal fracture, in the absence of traumatic injury, over 4 months after trapeziectomy with suture-button suspensionplasty. All 3 fractures demonstrated the same pattern: short oblique/spiral, oriented proximal radial to distal ulnar with the distal end in the vicinity of the index metacarpal button, presumably after the orientation of the metacarpal drill hole. Two of the

Finger fractures are common and potentially disabling. Certain complex injuries, particularly with small fragments and/or open wounds, are not manageable with conventional orthopedic hardware. We created 3D-printed finger fracture fixators using polylactic acid (PLA) and polyethylene terephthalate glycol (PETG). We tested flexion stiffness and lateral stiffness, before and after sterilization for surgical use, and compared them against standard 316L surgical stainless-steel (SSS) wires. Because PLA and PETG are compromised at high temperatures, these were sterilized using hydrogen peroxide gas

When there is a considerable loss of first metatarsal phalangeal joint (MTPJ) motion and/or ankylosis is apparent then a joint preservation procedure may not be feasible. For end-stage degenerative change within the MTPJ, nonreducible joint incongruity, or instability of the first MTPJ, an arthrodesis can provide the most predictable and, arguably, the most definitive correction of the deformity, especially in patients with higher functional demands. This article discusses principles of techniques with an emphasis on the procedure to prepare a successful arthrodesis, and expounds on technical
