Development and preclinical evaluation of a biodegradable ventricular septal defect occluder.

OBJECTIVES: This study evaluated the feasibility, effectiveness, and safety of a biodegradable (BD) occluder for closure of ventricular septal defect (VSD) in an acute canine model. BACKGROUND: All current available VSD occluders are permanent implants which consist of a metal framework and synthetic fabrics. However, the septal occluder in vivo plays the role of a temporary bridge that facilitates the ingrowth of fibrous connective tissue and endothelialization. The ideal occluder may be a temporary scaffold which can be gradually absorbed in vivo and replaced by "native" tissue. METHODS: T

Results of accelerated postoperative rehabilitation using novel "suture frame" repair of Achilles tendon rupture.

The management of Achilles tendon rupture is a much-debated subject. In recent years, there has been much interest in early postoperative mobilization. We present the results of our Achilles tendon repair technique and accelerated rehabilitation program. The technique we propose uses the strength of a 1-loop polydioxanone "suture frame" to enable restoration of the tendon length, immediate positioning of the foot in a near-plantigrade position, and an accelerated rehabilitation program. We followed up 15 cases of Achilles tendon rupture treated with this technique. The initial follow-up was a

Clinical experience with PDS II augmentation for operative treatment of acute proximal ACL ruptures--2-year follow-up.

The results of prospective anterior cruciate ligament (ACL) refixation in 33 patients with high proximal rupture is reported at 20-28 months' follow-up: mean age was 31.1 +/- 12.5 years. The surgical technique was a specially developed refixation of the ACL using a multiple suture loop (modified Marshall technique) augmented with intra-articular PDS II (polydioxanon, resorbable, Ethicon, Hamburg, Germany) to avoid derangement of blood circulation and to guarantee early functional rehabilitation. All patients were operated on within 7.3 +/- 4.5 days after injury. According to the IKDC evaluatio

[Results of treatment after different surgical procedures for management of acromioclavicular joint dislocation].

Sixty-four patients underwent surgery for acromioclavicular (AC) disruption, Tossy type III, at the Department of Surgery, University of Heidelberg, between January 1983 and May 1990. Surgery consisted of a suture of the AC and coracoclavicular ligaments. Fixation of the joint was achieved with three different techniques: tension band wire with two Kirschner wires, special hook-plate (Wolter), double tension band fixation using polydioxanon (PDS) cordula. The early postoperative complication rate was higher following tension band wires (42.9%) and hook plate (58.3%) than after tension band PDS

All-arthroscopic triangular fibrocartilage complex repair: safety and biomechanical comparison with a traditional outside-in technique in cadavers.

PURPOSE: To compare the biomechanical strength and safety of an all-arthroscopic triangular fibrocartilage (TFCC) repair technique with an outside-in technique in cadavers. METHODS: Ten matched pairs of fresh-frozen cadaveric wrists were used for this study. The control group TFCC tears were treated with an outside-in technique using 2-0 polydioxane (PDS) sutures. The experimental group was treated with two FasT-Fix suture devices. I measured the location of the implants in relation to the neurovascular structures using a digital caliper. The strength of the repairs was then determined using

Mechanical properties of collagen membranes: are they sufficient for orbital floor reconstructions?

INTRODUCTION: The most common reconstruction materials for orbital floor fractures are PDS (polydioxanone) foil and titanium meshes. These materials have advantages and disadvantages. Therefore, new materials are needed to improve surgical outcomes. MATERIALS AND METHODS: Three resorbable collagen membranes (Smartbrane(®), BioGide(®), Creos(®)) were tested for their mechanical properties (puncture strength) in mint and artificially aged (3, 6, 8 weeks) conditions and were compared to PDS foil, titanium meshes (0.25 mm, 0.5 mm) and human orbital floors (n = 7). RESULTS: The following puncture

Is barbed better? Evaluation of triclosan-coated barbed suture on wound complications following emergency laparotomy.

INTRODUCTION: Emergent laparotomy is associated with significant wound complications including surgical site infections (SSIs) and fascial dehiscence. Triclosan-coated barbed (TCB) suture for fascial closure has been shown to reduce local complications but primarily in elective settings. We sought to evaluate the effect of TCB emergency laparotomy fascial closure on major wound complications. METHODS: Adult patients undergoing emergency laparotomy were prospectively evaluated over 1 year. Patients were grouped into TCB versus polydioxanone (PDS) for fascial closure. Subanalysis was performed

Polymers for Pelvic Floor Repair: Current State and Future Prospects.

IMPORTANCE: Surgical repair of pelvic organ prolapse and stress urinary incontinence frequently relies on polymeric biomaterials in sutures, meshes, and membranes to provide structural support during tissue healing and beyond. Polypropylene has long served as the default material owing to its strength, durability, and processability, but the chronic foreign body response and mechanical mismatch between stiff implants and viscoelastic pelvic tissue have been linked to complications, including mesh contraction, exposure, and dyspareunia. These limitations, together with regulatory actions on tra

The Clip-Pulley Maneuver: Atraumatic and Adjustable Retraction in Robotic Liver Resection.

BACKGROUND: Precise liver retraction is essential in robotic hepatectomy. Conventional retraction methods using barbed sutures1,2 or hook-needle systems3 are penetrative and traumatic, risking diaphragmatic injury or tension pneumothorax. We developed a nonpenetrating Clip-Pulley Maneuver (CPM) to achieve safe and adjustable retraction. MATERIALS AND METHODS: From February to November 2025, 26 robotic hepatectomies were performed using CPM. A 3-0 polydioxanone monofilament (PDS; Ethicon, Inc., Bridgewater, NJ, USA) was placed on the liver surface at the resection margin. After suture placemen

Mid-facial soft tissue re-suspension following skeletal fixation after maxillofacial trauma-a prospective case series study.

PURPOSE: Open reduction and internal fixation (ORIF) is used for reduction and stabilization of facial fractures. Despite perfect anatomic skeletal reduction, subtle asymmetries are seen in facial appearance as mid-facial soft tissue often reattaches to the respective facial bones in an aberrant, mostly lower in position. Thus, the present study evaluates the effectiveness of mid-facial soft tissue re-suspension procedure to prevent changes in mid-facial soft tissue profile after ORIF of maxillo-facial fractures. METHODS: Fifteen patients with maxillo-facial fractures requiring ORIF were incl
