Pancreatolithiasis in a 55-year-old woman treated surgically with a modified Puestow procedure: a case report.

INTRODUCTION: Pancreatolithiasis is the formation of stones in the pancreatic parenchyma, side branches, or the pancreatic ducts. The prevalence of the disease is 0-4 per 1000 people worldwide. It is found in 50-90% of people with chronic pancreatitis and is strongly linked to the condition. CASE PRESENTATION: A 55-year-old female patient presented to our hospital with the complaint of worsening epigastric pain of 2 weeks' duration. The patient had been experiencing pain for the past 7 years. An abdominal computed tomography scan showed whole-length pancreatic duct beaded-type dilatation and

Exploring CAR T-Cell Dynamics: Balancing Potent Cytotoxicity and Controlled Inflammation in CAR T-Cells Derived from Systemic Sclerosis and Myositis Patients.

Systemic lupus erythematosus (SLE), systemic sclerosis (SSc), and idiopathic inflammatory myositis (IIM) are autoimmune diseases managed with long-term immunosuppressive therapies. Hu19-CD828Z, a fully human anti-CD19 chimeric antigen receptor (CAR) with a CD28 costimulatory domain, is engineered to potently deplete B-cells. In this study, we manufactured Hu19-CD828Z CAR T-cells from peripheral blood of SLE, IIM, and SSc patients and healthy donors (HDs). CAR-mediated, CD19-specific activity of these cells was evaluated in vitro by assessing cytotoxicity, cytokine release, and proliferation as

Dysphagia and Muscle Weakness Secondary to Botulinum Toxin Type A Treatment of Cervical Dystonia: A Drug Class Analysis of Prescribing Information.

The first-line management of cervical dystonia (CD) symptoms is intramuscular injection of botulinum toxin type A (BoNTA). However, a comparison of safety among BoNTAs is difficult because, per regulatory authorities, units of BoNTA activity are not interchangeable. Dysphagia and muscle weakness are widely considered two key adverse events to monitor closely in the treatment of CD. This integrated analysis compared the safety of BoNTAs approved for CD in the US by evaluating relationships between the incidence of dysphagia and muscle weakness in prescribing information and the core neurotoxin

Current recommendations for treatment of severe toxic alcohol poisonings.

BACKGROUND: Ethylene glycol (EG) and methanol are responsible for accidental, suicidal, and epidemic poisonings, resulting in death or permanent sequelae. Toxicity is due to the metabolic products of alcohol dehydrogenase (ADH) and aldehyde dehydrogenase. Conventional management of these intoxications consists of ethanol and hemodialysis. Fomepizole, a potent ADH inhibitor, has largely replaced antidotal ethanol use in France and two recent prospective U.S. trials definitively established its efficacy. Fomepizole appears safer than ethanol and while no comparative study of efficacy exists, fom

Assessment of Abdominal Constrictor's Forces for Informing Computational Models of Orthostatic Hypotension.

Orthostatic hypotension is defined as a sudden drop in blood pressure upon standing from a sitting or supine position. The prevalence of this condition increases exponentially with age. Nonpharmacological treatments are always the first step in the management of this condition, such as the use of an abdominal constriction belt to optimize the blood volume in the abdomen. A multitude of clinical trials have shown the efficacy of elastic abdominal compression as well as compression using an inflatable bladder; however, there are currently few accessible consumer products that can provide abdomin

Diagnosis and management of paediatric magnet ingestion: a systematic review of clinical practice guidelines.

BACKGROUND: The widespread use of small and powerful neodymium magnets in consumer products has led to a significant rise in paediatric magnet ingestion. The ingestion of multiple magnets poses serious risks, including bowel obstruction and perforation. Evidence-based Clinical Practice Guidelines (CPGs) are necessary to inform clinicians of the safest and most effective management strategies across a range of healthcare settings. AIM: This systematic review aims to summarise existing CPGs for diagnosing and managing paediatric magnet ingestion, evaluate the supporting evidence, highlight vari

Genome-wide association and functional genomic analyses for various hoof health traits in North American Holstein cattle.

Hoof diseases are a major welfare and economic issue in the global dairy cattle production industry, which can be minimized through improved management and breeding practices. Optimal genetic improvement of hoof health could benefit from a deep understanding of the genetic background and biological underpinning of indicators of hoof health. Therefore, the primary objectives of this study were to perform genome-wide association studies, using imputed high-density genetic markers data from North American Holstein cattle, for 8 hoof-related traits: digital dermatitis, sole ulcer, sole hemorrhage,

Tracing the Path from Obesity to Diabetes: How S-Allyl Cysteine Shapes Metabolic Health.

Background: Metabolic Syndrome (MetS) is a multifactorial condition characterized by insulin resistance, dyslipidemia, hypertension, and abdominal obesity, which collectively increase the risk of type 2 diabetes mellitus and cardiovascular diseases. Lifestyle modification represents the first-line strategy in its management, whereas pharmacological interventions are complex and typically require long-term polypharmacotherapy. In this context, natural bioactive compounds with pleiotropic effects are gaining increasing attention. Among these, S-allyl cysteine (SAC), the major sulfur-containing c

Compassionate use of contezolid in a toddler with severe community-acquired pneumonia induced by staphylococcus aureus: a case report and follow-up.

BACKGROUND: Initial choices of antimicrobial therapy for most cases of community-acquired pneumonia (CAP) in children under 5 years of age are typically based on local epidemiology, risk factors assessment, and subsequent clinical parameters and positive cultures, which can lead to the underdiagnosis and underestimation of lung infections caused by uncommon pathogens. Contezolid, an orally administered oxazolidinone antibiotic, gained approval from the National Medical Products Administration (NMPA) of China in June 2021 for managing complicated skin and soft tissue infections (cSSTI) caused b

Exploring novel pharmacological trends: Natural compounds in dry eye disease management.

Dry eye disease (DED) is an ocular condition characterized by altered tear film homeostasis, resulting in symptoms like tear film instability, hyperosmolarity, inflammation, and neurosensory abnormalities. It affects visual acuity and quality of life and is influenced by age, gender, and environmental factors. The first line of treatment consists of dynamically developing artificial tears, gels, and eyelid sprays, which can be supplemented with natural ingredients for enhanced efficacy. Other therapeutic steps include auto-logous serum tears, anti-inflammatory and immunosuppressive eyedrops, o

[Topical Treatment of Pruritic Skin Disease and the Role of Community Pharmacists].

Itching, or pruritus, can be defined as an unpleasant sensation that evokes the desire to scratch. Pruritus is most commonly associated with a primary skin disorder such as atopic dermatitis (AD), psoriasis, etc., and can have a major impact on the quality of life of those patients. Itch-induced scratching can further damage the skin barrier, leading to a worsening of symptoms. For that reason, it is important to manage pruritus. Topical glucocorticoids are commonly the first-line therapy in the management of AD and psoriasis patients. We found that topical glucocorticoids induce pruritus in m

Changing the topography of acne with topical medications.

PURPOSE OF REVIEW: Acne vulgaris is a common skin condition that affects approximately 40-50 million Americans annually. The peak incidence of acne occurs during adolescence and young adulthood, with approximately 80% of persons aged 11-30 years having at least minor acne. Because the onset of acne is typically in early adolescence, pediatricians are often at the front-lines in terms of caring for patients with this chief complaint. RECENT FINDINGS: Over the last 2 years, new acne therapies have become available. These include new combination fixed-dose products, which may allow for greater f

Antibiotic therapy for common infections.

Several important points regarding the treatment of urinary tract infections should be made. Single-dose and short-course antibiotic therapy is appropriate only for women with acute bacterial cystitis due to E. coli. Studies comparing single-dose to full-course therapy have not been sufficiently designed to draw valid statistical conclusions, and only TMP/SMX is recommended at this time. Recurrent UTI in women is almost always due to reinfection, which is best managed by prophylactic antibiotics. Acute bronchitis and acute exacerbations of chronic bronchitis are often due to viral infections,

Potentials, challenges and future of chimeric antigen receptor T-cell therapy in non-Hodgkin lymphomas.

Background: Diffuse large B-cell lymphoma (DLBCL) is the most common lymphoma subtype. Disease progression or relapse following frontline chemoimmunotherapy, largely in the form of standard R-CHOP, occurs in 30-40% patients. Relapsed/refractory (R/R) DLBCL represents a major unmet medical need. In particular, patients with primary refractory disease or those whose lymphoma relapses after autologous stem cell transplantation have historically had poor outcomes.Material and methods: Chimeric antigen receptor T-cell (CART) therapy is a promising novel treatment with curative potential in this set

Antimicrobial resistance mechanisms, treatment challenges, and emerging strategies in Stenotrophomonas maltophilia infections.

Stenotrophomonas maltophilia is a Gram-negative opportunistic pathogen widely distributed in natural and clinical environments, and has increasingly been recognized as a significant cause of nosocomial infections. Owing to its complex intrinsic resistance mechanisms and the continuous evolution of acquired resistance, the clinical management of S. maltophilia infections remains highly challenging. Resistance in S. maltophilia is mediated by multiple mechanisms, including chromosomally encoded L1/L2 β-lactamases, multidrug efflux pump systems, reduced outer membrane permeability, and biofilm fo

A scoping review on the availability and utilization of essential opioid analgesics in Sub-Saharan Africa.

BACKGROUND: Management of acute, post-operative, and chronic pain requires access to and availability of opioids. While often oversupplied in high-income countries, significant shortages exist in low- and middle-income countries. We conducted a scoping review on availability and usage of opioids in Sub-Saharan Africa (SSA). METHODS: The five-stage approach of Arksey and O'Malley (2005) was used. MEDLINE via PubMed, EMBASE, and SCOPUS were search and results categorized into themes: 1) Local/regional availability and supply, 2) Consumption patterns, 3) Legislation and policy, 4) Costs and fina
