Managing the personal side of health: how patient expertise differs from the expertise of clinicians.

BACKGROUND: When patients need health information to manage their personal health, they turn to both health professionals and other patients. Yet, we know little about how the information exchanged among patients (ie, patient expertise) contrasts with the information offered by health professionals (ie, clinician expertise). Understanding how patients' experiential expertise contrasts with the medical expertise of health professionals is necessary to inform the design of peer-support tools that meet patients' needs, particularly with the growing prevalence of largely unguided advice sharing th

Arthroscopic treatment of osteoarthritis of the knee: are there any evidence-based indications?

Despite the lack of consensus guidelines and randomized control trials, the use of arthroscopy for the treatment of osteoarthritis of the knee has increased over the last decade. Techniques used for the arthroscopic treatment of osteoarthritis of the knee include joint lavage, joint débridement, meniscectomy, abrasion arthroplasty, and microfracture. We performed a retrospective, evidence-based review of the current literature on the arthroscopic treatment of osteoarthritis of the knee and provide insight into the study design flaws and difficulties associated with the current research on this

Alzheimer disease research in the 21st century: past and current failures, new perspectives and funding priorities.

BACKGROUND: Extended-spectrum beta-lactamase (ESBL)-producing strains of Klebsiella pneumoniae have caused major therapeutic problems worldwide since the majority are resistant to various antibiotics. In this study, an investigation was conducted regarding antibiotic-resistant patterns of 128 isolates of K pneumonile cultured from Iranian patients at two teaching hospitals during 2004-2005. MATERIAL/METHODS: The susceptibility of isolates to 21 antimicrobial agents was determined using the disk diffusion method. Disks containing ceftazidime, cefotaxime, ceftazidime/clavulanic acid, and cefota

Application Prospect of Artificial Intelligence in Rehabilitation and Management of Myasthenia Gravis.

Myasthenia gravis (MG) is a chronic autoimmune disease of the nervous system, which is still incurable. In recent years, with the progress of immunosuppressive and supportive treatment, the therapeutic effect of MG in the acute stage is satisfactory, and the mortality rate has been greatly reduced. However, there is still no consensus on how to conduct long-term management of stable MG, such as guiding patients to identify relapses, practice exercise, return to work and school, etc. In the international consensus guidance for management of myasthenia gravis published by the Myasthenia Gravis F

[Supportive care for survivors].

Advances in early detection and treatment have pushed in a few decades the management of cancers from a management model of the end of life to the management of a chronic disease. This evolution has accelerated the development of supportive care in two directions, firstly towards the best possible support to the end of life in advanced cancer patients (palliative care) and secondly to the limitation of treatment toxicities, the prevention of relapse and the return to life as "normal" as possible (care for after cancer). If palliative care now has a legitimacy and a solid regulatory base, this

Developing a prioritized research agenda for necrotizing enterocolitis.

BACKGROUND: Necrotizing enterocolitis (NEC) research priorities informed by a broad group of stakeholders have not been previously determined. METHODS: We recruited 11 patient-families and 11 clinician-researchers to develop a prioritized research agenda from 11/19 to 8/22. We identified topics to prioritize through a systematic literature review, analysis of patient stories, and an open-ended survey, and prioritized these in a two-round Delphi process. RESULTS: We identified and consolidated 157 potential topics from 1754 research articles, 236 patient-family stories, and 10 survey themes,

Reframing dementia care in the era of disease-modifying therapies: informational, psychosocial, and systemic insights from Japan.

BACKGROUND: The introduction of disease-modifying therapies (DMTs) for Alzheimer’s disease has prompted major changes in diagnostic pathways, referral processes, and service coordination in dementia care. Japan, as an early adopter of DMTs within a nationally coordinated dementia-care framework, offers an opportunity to examine how health service structures respond to these changes. This study explored health service–related support needs emerging across the DMT pathway, focusing on patient and informal caregiver experiences, with complementary perspectives from service providers. METHODS: A q

Exploring enablers and barriers to the use of chest compression feedback devices in advanced life support: a qualitative study.

BACKGROUND: In sudden cardiac arrest, high-quality chest compressions are crucial for survival with favorable neurological outcomes. Chest compression feedback devices improve chest compression quality and the likelihood of achieving return of spontaneous circulation. However, implementation remains challenging. Understanding enablers and barriers is essential to inform implementation strategies and device design. METHODS: We conducted a qualitative exploratory study using semi-structured interviews with 15 healthcare professionals from pre- and in-hospital advanced life support settings in N

Mobile enhanced prevention support for people leaving jail: examining smartphone app integration with peer mentors and contingency management for a population at risk of HIV.

The period following community reentry from incarceration is associated with elevated risks of drug overdose, HIV transmission, and other harms. The importance of leveraging mobile technology to facilitate service access has grown with smartphone use. Peer navigation has shown efficacy for individuals at risk of HIV transmission. We conducted in-depth qualitative interviews with 19 individuals assigned male at birth (all who have sex with men) who were participants in a randomized controlled trial to test an intervention combining the three above components. Three peer mentors were also interv

Speaking in Metaphors: Exploring Black Women's Communicated Sense-Making of Postpartum Depression.

Guided by the communicated sense-making model, we describe the results of an analysis of interviews with 15 Black women about their experiences with postpartum depression (PPD). The findings indicate that the women made sense of their experience using three categories of metaphors: (1) mobility, (2) cultural expectations, and (3) social support. These categories of metaphors provide evidence that the discourse of motherhood as distressing is expressed in the narratives of Black women with PPD, even as they seek to fulfill the demands of the discourse of motherhood as intensive. The metaphors a

A universal home-visit programme to tailor support to first-time parents: a qualitative case study on parents' perspectives.

BACKGROUND: The first year of a child's life is essential for promoting a healthy life, and the transition to becoming a parent can be a challenge; parents need to develop confidence in their own capacity to care for their child. The national Child Health Services programme in Sweden offers parental support, both on a universal level and in accordance with the individual family's needs. This study explores parents' experiences of an extended home-visit programme offered through a Family Centre to all first-time parents in a municipality. METHODS: This case study is based on a qualitative refl

Building community through engagement in a Hub-and-Spoke, Peer-led Community Aphasia Group program: the perspectives of participants with aphasia and their close others.

PURPOSE: Long-term social support is a key protective factor for living well with chronic disabilities. Community Aphasia Groups (CAGs) provide opportunities for connection and recovery. To address service availability challenges, we developed the Hub-and-Spoke, Peer-led CAG program in which a "Hub" of clinical staff supports "Spokes"-community members with and without aphasia-to facilitate high-quality CAGs. A pilot, mixed-methods study confirmed the feasibility and acceptability of this model. MATERIALS AND METHODS: This nested qualitative study explored program acceptability from the persp

Wishes, conflicts, and support needs of informal caregivers of patients in the palliative phase: A qualitative study.

This study explored wishes, conflicts, beneficial, and wished support of informal caregivers (ICGs) providing care to a palliative ill close other. We interviewed five current and 15 bereaved ICGs (25-75 years), and used thematic analyses. ICGs wished to continue life as normal as possible, spend time together and with their family, comfort their close other, and continue own activities. Caregivers experienced conflicts in balancing caregiving and own activities, quality time with each other and social contacts, the level of professional or informal support preferred, and in their shifting rol
