Testicular sperm extraction in men with sertoli cell-only testicular histology - 1680 cases.

OBJECTIVE: To study the outcomes of testicular sperm extraction (TESE) among men with pure Sertoli cell-only histology identified during diagnostic testicular biopsy. METHODS: This retrospective cohort study involved 1680 cases of patients with nonobstructive azoospermia (NOA) diagnosed with pure Sertoli cell-only histology who underwent testicular biopsy with TESE in a reference center in Brazil by a single surgeon. Sperm retrieval rates (SSR) were the main outcome measure. RESULTS: Overall, 14.83% of patients with Sertoli cell-only had sperm retrieved with TESE in quantity that allowed the

AI-Based Augmented Reality Microscope for Real-Time Sperm Detection and Tracking in Micro-TESE.

Non-obstructive azoospermia (NOA) is a severe male infertility condition characterized by extremely low or absent sperm production. In microdissection testicular sperm extraction (Micro-TESE) procedures for NOA, embryologists must manually search through testicular tissue under a microscope for rare sperm, a process that can take 1.8-7.5 h and impose significant fatigue and burden. This paper presents an augmented reality (AR) microscope system with AI-based image analysis to accelerate sperm retrieval in Micro-TESE. The proposed system integrates a deep learning model (YOLOv5) for real-time s

Varicocelectomy in men with nonobstructive azoospermia.

The management of nonobstructive azoospermia (NOA) is challenging. Even the most successful intervention for this condition, microsurgical testicular sperm extraction (microTESE) with in vitro fertilization (IVF)-intracytoplasmic sperm injection (ICSI), is not successful at least 40% of the time. Varicocele is present in 15% of young men, and varicocelectomy improves fertility outcomes in men with palpable varicoceles and abnormal semen analyses. Unfortunately, there are limited investigations into the effect of varicocelectomy in men with NOA. Uncontrolled cohort studies report the return of

Seven pregnancies and deliveries from non-mosaic Klinefelter syndrome patients using fresh and frozen testicular sperm.

PURPOSE: The aim of this study was to investigate the feasibility of using frozen-thawed testicular sperm as well as the timing of testicular sperm extraction (TESE) in patients with non-mosaic Klinefelter syndrome. METHODS: Intracytoplasmic sperm injection (ICSI) was performed in six of 17 (35%) patients whose sperm was recovered by TESE. Multiple biopsies of both testes were performed on the day of oocyte retrieval in all but one of the six patients. RESULTS: Seven pregnancies and deliveries were achieved in five couples, and one couple was unsuccessful. Five pregnancies were achieved usin

A step closer to parenthood with non-obstructive azoospermia: Unveiling the impact of microdissection testicular sperm extraction in Australia's largest single-centre study.

BACKGROUND: Non-obstructive azoospermia (NOA) diagnosis poses challenges for couples seeking parenthood. Microdissection testicular sperm extraction (MD-TESE) excels in retrieving testicular sperm cells for NOA cases. However, limited live birth data in Australian NOA patients hinders accurate counselling. AIMS: This study aimed to determine the likelihood of infertile couples with a male partner diagnosed with NOA conceiving biological children using MD-TESE / intracytoplasmic sperm injection (ICSI). MATERIALS AND METHODS: A retrospective cohort study included 108 NOA men treated at a publi

Medical treatment prior to micro-TESE.

Except in cases of hypogonadotropic hypogonadism, the use of medical therapy before microsurgical testicular sperm extraction (micro-TESE) is controversial. In some studies, hormone therapy has been shown to improve the possibility of sperm retrieval during micro-TESE and even lead to the presence of sperm in the ejaculate in some cases, thereby obviating the need for micro-TESE. However, their routine use before micro-TESE in cases of nonobstructive azoospermia (NOA) being associated with hypergonadotropic hypogonadism and eugonadism (normogonadotropic condition) has not been supported with r

The use of immotile testicular sperm for in vitro fertilization/intracytoplasmic sperm injection: what are the outcomes?

Sperm extraction for in vitro fertilization (IVF)/intracytoplasmic sperm injection (ICSI) remains the only option for men with azoospermia to father biologic offspring. Even in cases of successful sperm retrieval, the challenge of selecting optimal sperm for use in IVF/ICSI is an area of research interest. Testicular sperm, unlike epididymal or ejaculated sperm, are often immotile, and there remains debate on the use of immotile testicular sperm in IVF/ICSI and how to best assess sperm viability. Therefore, we aimed to summarize the available literature regarding IVF/ICSI outcomes using immoti

Microdissection testicular sperm extraction outcomes in azoospermic patients with bilateral orchidopexy.

BACKGROUND: Cryptorchidism is considered to be one of the most common causes of non-obstructive azoospermia. There are several surgical techniques to retrieve sperm in these patients. Microdissection testicular sperm extraction (m-TESE) is a recent sperm retrieval technique which is considered to be a safe, non-blind, and feasible method. OBJECTIVES: This study aimed to investigate sperm retrieval rate (SRR) by the mTESE method in patients who have undergone orchidopexy due to bilateral cryptorchidism. MATERIALS AND METHODS: In this retrospective study, 56 ex-cryptorchid patients, who underw

Clinical Outcomes of Pathologist-guided Ex-vivo Onco-TESE: A Single Institution Experience and Contemporary Review.

OBJECTIVE: To assess the fertility outcomes of pathologist-guided ex vivo TESE of radical orchiectomy specimens, without involvement of reproductive urologist or microsurgical skills. Ex-vivo testicular sperm extraction (TESE) is a viable alternative for fertility preservation in patients with testicular cancer and concomitant azoospermia. While existing literature describes urologist-driven sperm retrieval, pathologist-driven extraction provides the opportunity to prioritize fertility outcomes in individuals while optimizing oncologic staging. METHODS: We performed a single-institution retro

Testicular sperm extraction: impact of testicular histology on outcome, number of biopsies to be performed and optimal time for repetition.

Testicular sperm extraction (TESE) may not always be successful in patients with non-obstructive azoospermia, as they only have minute foci of active spermatogenesis from which a tiny number of spermatozoa can be extracted. The aim of this study was to find the percentile incidence of successful TESE in non-obstructive azoospermia patients in relation to various histopathological patterns and the number of performed biopsies, and to determine the optimal time needed for repetition. A total of 216 patients underwent bilateral testicular biopsy taking a single piece from each testis for sperm re

Aging may adversely affect testicular sperm recovery in patients with Klinefelter syndrome.

OBJECTIVES: To investigate the clinical parameters or laboratory analyses that may have a predictive value for successful sperm retrieval in Klinefelter syndrome (KS). METHODS: A total of 74 patients with nonmosaic KS were included in this study. All patients were azoospermic and underwent microdissection testicular sperm extraction for sperm recovery. The predictive values of patient age, testicular volume, serum follicle-stimulating hormone, luteinizing hormone, and testosterone levels were assessed for successful sperm recovery. RESULTS: Testicular sperm recovery was successful in 42 (56.

Infertility treatment for patients having a microdeletion of azoospermic factor (AZF).

In genetic causes of male infertility, Y chromosome microdeletions are the second most common after Klinefelter's syndrome. Although sperm recovery rate is relatively high for subjects with azoospermic factor (AZF) c chromosome microdeletion, intracytoplasmic sperm injection (ICSI) results using retrieved sperm has been reported to be poor. We retrospectively examined the infertility treatment for subjects with AZF microdeletion. From October 2017 to September 2020, chromosomal examination of 67 azoospermic subjects and 12 cryptozoospermia were performed. Of these, twenty-three subjects (29.1%

A Case Highlighting the Importance of Timely Intervention for a Male With AZFb + c Microdeletion.

Male infertility can be caused by various microdeletions of the azoospermia factor (AZF) gene, some of which require sperm retrieval techniques for treatment. Yet, the likelihood of success with such techniques depends partly on the affected AZF gene segment. For patients with select AZF microdeletions, other options such as in vitro fertilization, sperm donation, and adoption may be more appropriate. We present a case about a 32-year-old male diagnosed with infertility due to azoospermia from a complete AZFb + c deletion, which is unlikely to be successfully treated with sperm retrieval techn

Abdominal electrical stimulation rescues failures to penile vibratory stimulation in men with spinal cord injury: a report of two cases.

Most men with spinal cord injury are anejaculatory. To retrieve their semen for insemination, the procedure of penile vibratory stimulation is recommended over electroejaculation. Some men with spinal cord injury, however, cannot ejaculate with penile vibratory stimulation. We present 2 cases in which a simple, over-the-counter abdominal muscle stimulator rescued failures to penile vibratory stimulation. Use of this safe, easy, and inexpensive method may prevent some patients from undergoing more expensive, more invasive sperm retrieval methods such as electroejaculation or surgical sperm retr

Post-harvest motility and morphological changes of spermatozoa following caudal epididymal recovery in farmed common eland (Tragelaphus oryx) bulls.

The caudal epididymal recovery of spermatozoa has been utilised in game animal species to preserve genetically superior material after trophy hunting or slaughter, due to the difficulties of handling wild animals. Furthermore, the potential application of assisted reproductive techniques using harvested spermatozoa may contribute towards maintaining genetic diversity in isolated captive populations internationally. However, this technique requires clear and ideally simple protocols for the collection and handling of gametes after the death of the male animals, as the point of harvest is usuall

[Testicular and epididymal sperm extraction surgery].

BACKGROUND: Testicular and epididymal sperm extraction surgery is a frequent procedure. However, to date, there has been no French consensus on the technique to be preferred in the various situations of male infertility and on the best way to perform them. We therefore decided to proceed with a formalized expert consensus, using the method recommended by the French National Authority for Health. The aim was to provide the French urology community with practical advice on how best to perform these procedures. METHODS: Twenty-six international experts met online for the consensus.
