Surgical risk factors and maxillary nerve function after Le Fort I osteotomy.

PURPOSE: Data on intraoperative risk factors for long-term postoperative complications after Le Fort I osteotomy (LFO) are limited. The aim of this study was to describe prospectively the overall postoperative changes in maxillary nerve function after LFO, and to correlate these changes with a number of possible intraoperative risk factors. PATIENTS AND METHODS: Twelve men and 13 women (mean +/- standard deviation: aged 25+/-10 years) participated in 4 sessions: 1 before LFO (baseline), and the rest at 3, 6, and 12 months after LFO. At each session, somatosensory sensitivity was assessed for

[Hemodynamic and clinical studies following injury of the arteries of the forearm].

Forearm arterial injury usually does not lead to acute ischemia, but a functional deficit may develop. We tried to evaluate the need for two patent forearm arteries using rheological, Doppler sonographical and clinical parameters. Twenty-seven patients were examined after arterial and/or nerve injury in the forearm as well as six patients in whom a forearm flap was harvested. In seventeen patients both arteries were patent after primary reconstruction. Nine patients showed only one patent artery, while in the six patients with a forearm flap the radial artery was reconstructed in only one case

Intraoral Sensory Alterations in Myofascial Orofacial Pain Patients: Implications for Clinical Management.

INTRODUCTION: This study evaluated differences in intraoral sensitivity to thermal and mechanical stimuli, and pulp sensibility between patients with myofascial orofacial pain (MOP) and healthy controls. We hypothesized that patients would exhibit increased sensitivity to these stimuli. METHODS: A controlled study was performed, including 50 MOP patients and 104 controls aged 18-65. Intraoral and extraoral sensitivities were assessed at various sites using simple quantitative sensory testing (numerical ratings of thermal/mechanical stimuli) and qualitative somatosensory testing (side-to-side

Cold hyposensitivity after topical application of capsaicin in humans.

Capsaicin activates the transient receptor potential vanilloid 1 receptor (TRPV1) on small sensory afferents, and capsaicin is commonly used to elucidate mechanisms of neuropathic pain. This study was performed to describe changes in cold and cold pain perception after topically applied capsaicin. Fourteen healthy subjects were included. Cold detection and cold pain thresholds and the response to suprathreshold cold stimuli were evaluated before and after topical application of capsaicin (200 microl, 50 mg/ml in 70% ethanol solution, 10.2 cm2) for 30 min. The skin temperature was kept between

Centrophobism/thigmotaxis, a new role for the mushroom bodies in Drosophila.

Insects, like vertebrates, exhibit spatially complex locomotor activity patterns when foraging or navigating. Open field studies recently showed that Drosophila avoids central zones and stays at the periphery, an effect that can be interpreted as centrophobism and/or thigmotaxis. In this study, we further characterized this phenomenon and studied the responsible underlying neural mechanisms. The implication of the Drosophila mushroom bodies (MBs) in olfactory learning and memory processes is well documented. In an open field situation in which fly locomotor activity is recorded by video tracki

Sensory effects of microvascular decompression in trigeminal neuralgia.

Nineteen patients with "idiopathic" trigeminal neuralgia, who had not undergone any previous interventional procedures, possessed a vessel or vessels compressing the preganglionic nerve root that was demonstrated by magnetic resonance tomographic angiography. Pain was relieved immediately in all of these patients after they underwent microvascular decompression without observed nerve damage. Although preoperative measurement of sensory perception thresholds showed elevations in the thresholds for touch (von Frey filaments) and warmth and coolness sensations, these thresholds normalized during

Temperature thresholds in assessment of the clinical course of acquired cold contact urticaria: a prospective observational one-year study.

Cold contact urticaria is the second most common subtype of physical urticaria. Cold stimulation standardized tests are mandatory to confirm the diagnosis. The aim of this study is to define the utility of determining thresholds (critical time and temperature) in assessment of the clinical course of typical acquired cold contact urticaria. Nineteen adult patients (10 women and 9 men; mean age 45 years) were included in the study and the diagnosis was confirmed with the ice-cube test and TempTest 3.0. Patients were treated continuously for 1 year with 20 mg/day rupatadine (anti-H1). Thresholds

Responses of intact and injured sural nerve fibers to cooling and menthol.

Intact and injured cutaneous C-fibers in the rat sural nerve are cold sensitive, heat sensitive, and/or mechanosensitive. Cold-sensitive fibers are either low-threshold type 1 cold sensitive or high-threshold type 2 cold sensitive. The hypothesis was tested, in intact and injured afferent nerve fibers, that low-threshold cold-sensitive afferent nerve fibers are activated by the transient receptor potential melastatin 8 (TRPM8) agonist menthol, whereas high-threshold cold-sensitive C-fibers and cold-insensitive afferent nerve fibers are menthol insensitive. In anesthetized rats, activity was re

Dopamine and pain sensitivity: neither sulpiride nor acute phenylalanine and tyrosine depletion have effects on thermal pain sensations in healthy volunteers.

Based on animal studies and some indirect clinical evidence, dopamine has been suggested to have anti-nociceptive effects. Here, we investigated directly the effects of increased and decreased availability of extracellular dopamine on pain perception in healthy volunteers. In Study 1, participants ingested, in separate sessions, a placebo and a low dose of the centrally acting D2-receptor antagonist sulpiride, intended to increase synaptic dopamine via predominant pre-synaptic blockade. No effects were seen on thermal pain thresholds, tolerance, or temporal summation. Study 2 used the acute ph

Impaired somatosensation in tongue mucosa of smokers.

Smoking has been indicated as a risk factor for oral diseases and can lead to altered sense of taste. So far, the effects of sensory changes on the tongue are not investigated. In this study, quantitative sensory testing was used to evaluate somatosensory function in the lingual region. Eighty healthy volunteers were investigated (20 smokers, 20 non-smokers). Subjects were bilaterally tested in innervation areas of lingual nerves. Thresholds of cold and warm detection, cold and heat pain, and mechanical detection were determined. As control for systemic, extraoral effects of smoking, tests wer

Topical application of L-menthol induces heat analgesia, mechanical allodynia, and a biphasic effect on cold sensitivity in rats.

Menthol is used in analgesic balms and also in foods and oral hygiene products for its fresh cooling sensation. Menthol enhances cooling by interacting with the cold-sensitive thermoTRP channel TRPM8, but its effect on pain is less well understood. We presently used behavioral methods to investigate effects of topical menthol on thermal (hot and cold) pain and innocuous cold and mechanical sensitivity in rats. Menthol dose-dependently increased the latency for noxious heat-evoked withdrawal of the treated hindpaw with a weak mirror-image effect, indicating antinociception. Menthol at the highe
