The MOnitoring Resynchronization dEvices and CARdiac patiEnts (MORE-CARE) randomized controlled trial: phase 1 results on dynamics of early intervention with remote monitoring.

BACKGROUND: Remote monitoring (RM) in patients with advanced heart failure and cardiac resynchronization therapy defibrillators (CRT-D) may reduce delays in clinical decisions by transmitting automatic alerts. However, this strategy has never been tested specifically in this patient population, with alerts for lung fluid overload, and in a European setting. OBJECTIVE: The main objective of Phase 1 (presented here) is to evaluate if RM strategy is able to reduce time from device-detected events to clinical decisions. METHODS: In this multicenter randomized controlled trial, patients with mode

Incidence and clinical predictors of low defibrillation safety margin at time of implantable defibrillator implantation.

BACKGROUND: Determination of the defibrillation safety margin (DSM) is the most common method of testing device effectiveness at the time of implantation of implantable cardioverter defibrillator (ICD) or cardiac resynchronization therapy defibrillator (CRTD). Low DSM remains a problem in clinical practice. OBJECTIVE: The purpose of this study is to ascertain the incidence and clinical predictors of low DSM and the treatment strategies for low DSM in ICD or CRTD recipients. METHODS: Selected ICD or CRTD recipients from January 2006 to May 2008 who underwent DSM test at the time of implantati

The number of implanted the cardiovascular implantable electronic device(s) (CIED(s))--pacemakers (PM) and implantable cardioverters defibrillators (ICD)--increases each year. The number of CIED(s) exchange procedures as well as changes in models of stimulation (upgrade to dual chamber pacemakers or three chamber cardiac resynchronization therapy devices) also grows. Also increases the inactive electrode left in the cardiovascular system. The risk of infection is higher during the exchange of devices than with their implantation. Treatments for patients with multiple electrode systems are beco

Real-time three-dimensional echocardiographic assessment of left ventricular systolic dyssynchrony in healthy children.

BACKGROUND: The use of resynchronization therapy for the treatment of left ventricular (LV) systolic dysfunction in children has been expanding. Because QRS duration is not a reliable indicator of the presence or severity of dyssynchrony in every case, additional methods of quantitation of dyssynchrony are needed. The purpose of this study was threefold: (1) to define normal values for LV real-time quantitative three-dimensional echocardiographic (3DE) dyssynchrony indices (DIs), (2) to analyze the feasibility and observer variability of 3DE DIs in a wide range of children, and (3) to determin

Real-time 3-dimensional echocardiographic assessment of left ventricular dyssynchrony: pitfalls in patients with dilated cardiomyopathy.

OBJECTIVES: This study sought to establish normal values for real-time 3-dimensional echocardiography (RT3DE)-derived left ventricular (LV) dyssynchrony index (LVDI) and determine its age dependency, and to compare dyssynchrony in patients with normal LV function and patients with dilated cardiomyopathy (DCM), with and without left bundle branch block (LBBB). BACKGROUND: Cardiac resynchronization therapy is known to be ineffective in one-third of patients with heart failure, highlighting the need for alternative techniques to assess LV dyssynchrony. METHODS: Datasets from RT3DE were analyzed

Usefulness of baseline electrocardiographic QRS complex pattern to predict response to cardiac resynchronization.

Cardiac resynchronization therapy (CRT) improves morbidity and mortality in patients with heart failure with QRS>120 ms, yet most patients studied in clinical trials manifested baseline left branch bundle block (LBBB). It is unclear whether benefits of CRT extend to patients with right branch bundle block (RBBB) or a paced QRS at baseline despite QRS>120 ms. Orthotopic heart transplantation- and ventricular assist device-free survival, symptomatic response, and echocardiographic response were evaluated in the 636 patients who underwent CRT at our institution from 2000 to 2007 in whom the basel

Persistent atrial fibrillation worsens heart rate variability, activity and heart rate, as shown by a continuous monitoring by implantable biventricular pacemakers in heart failure patients.

BACKGROUND: Atrial fibrillation (AF) induces loss of atrial contribution, heart rate irregularity, and fast ventricular rate. OBJECTIVES: The objectives of the study were to accurately measure AF incidence and to investigate the mutual temporal patterns of AF and heart failure (HF) in patients indicated to cardiac resynchronization therapy. METHODS: Four hundred ten consecutive patients (70% male, age 69 +/- 11) with advanced HF (NYHA = 3.0 +/- 0.6), low ejection fraction (EF = 27 +/- 9%), and ventricular conduction delay (QRS = 165 +/- 29 ms) received a biventricular pacemaker. Two-year performance of a preshaped lead for left ventricular stimulation.

Important goals, when implanting a cardiac resynchronization therapy (CRT) system, are to reach a stable LV lead position in a suitable coronary sinus (CS) tributary associated with a low capture threshold and no extracardiac stimulation. The aim of this study was to assess the 2-year performance of a new preshaped lead designed for LV stimulation. The study enrolled 102 patients with heart failure and bundle branch block who underwent implantation of a CRT system, including a sheath-guided, unipolar, S-shaped LV lead placed in a CS tributary. At each follow-up the electrical parameters and CR

Clinical Implications of Functional Mitral Regurgitation Severity in Patients with Heart Failure with Reduced Ejection Fraction (HFrEF).

BACKGROUND: An estimated 64.3 million people are living with heart failure worldwide. Functional MR in chronic HFrEF reflects primarily the severity of LV dysfunction and is not related to structural alterations of the mitral valvular apparatus. FMR in patients with HFrEF independently of the etiology of HFrEF and its underlying mechanisms, contributes to progression of the symptoms of HF and is independent predictor of worse clinical outcomes. OBJECTIVE: The purpose of this study was to assess the severity of functional mitral regurgitation (FMR) and its clinical implications in patients wit

Hemodynamic changes in response to GLP-1 treatment in ICD and CRT patients: insights from HeartLogic sensor data.

BACKGROUND: Glucagon-like peptide-1 receptor agonist (GLP-1RA) therapy is increasingly used, but the physiological effects in patients with heart failure and reduced ejection fraction (HFrEF) remain uncertain. Continuously collected data from implantable cardiac devices may enable evaluation of drug effects in a real-world setting. METHODS: In a nationwide Danish retrospective matched cohort study, GLP-1RA initiators with implantable cardiac devices were matched 1:5 to unexposed device recipients by sex and age. Sensor data were assessed from 30 days before to 30 days after initiation. Primar

Radiotherapy for patient with cardiac implantable electronic device, consensus from French radiation oncology society.

Radiotherapy in patients with cardiac implantable electronic device such as pacemakers or defibrillators, is a clinical situation that is becoming increasingly common. There is a risk of interaction between the magnetic field induced by accelerators and the cardiac implantable electronic device, but also a risk of device dysfunction due to direct and/or indirect irradiation if the cardiac implantable electronic device is in the field of treatment. The risk can be dose-dependent, but it is most often independent of the total dose and occurs randomly in case of neutron production (stochastic eff

Electrical latency predicts the optimal left ventricular endocardial pacing site: results from a multicentre international registry.

AIMS: The optimal site for biventricular endocardial (BIVENDO) pacing remains undefined. Acute haemodynamic response (AHR) is reproducible marker of left ventricular (LV) contractility, best expressed as the change in the maximum rate of LV pressure (LV-dp/dtmax), from a baseline state. We examined the relationship between factors known to impact LV contractility, whilst delivering BIVENDO pacing at a variety of LV endocardial (LVENDO) locations. METHODS AND RESULTS: We compiled a registry of acute LVENDO pacing studies from five international centres: Johns Hopkins-USA, Bordeaux-France, Eind

Feedback to providers improves evidence-based implantable cardioverter-defibrillator programming and reduces shocks.

BACKGROUND: Implantable cardioverter-defibrillator (ICD) shocks are associated with increased anxiety, health care utilization, and potentially mortality. OBJECTIVE: The purpose of the Shock-Less Study was to determine if providing feedback reports to physicians on their adherence to evidence-based shock reduction programming could improve their programming behavior and reduce shocks. METHODS: Shock-Less enrolled primary prevention (PP) and secondary prevention (SP) ICD patients between 2009 and 2012 at 118 study centers worldwide and followed patients longitudinally after their ICD implant.

Current evidence on treatment of patients with chronic systolic heart failure and renal insufficiency: practical considerations from published data.

Chronic kidney disease (CKD) is increasingly prevalent in patients with chronic systolic heart failure. Therefore, evidence-based therapies are more and more being used in patients with some degree of renal dysfunction. However, most pivotal randomized clinical trials specifically excluded patients with (severe) renal dysfunction. The benefit of these evidence-based therapies in this high-risk patient group is largely unknown. This paper reviews data from randomized clinical trials in systolic heart failure and the interactions between baseline renal dysfunction and the effect of randomized tr

Insights into mildly dilated cardiomyopathy: temporal evolution and long-term prognosis.

AIMS: Mildly dilated cardiomyopathy (MDCM) has been proposed as a subtype of dilated cardiomyopathy (DCM) characterized by systolic dysfunction in the absence of significant LV dilatation. Few data on the characteristics and outcomes of MDCM patients are available. We sought to assess the main features and the long-term natural history of MDCM. METHODS AND RESULTS: From 1988 to 2010 we analysed all DCM patients consecutively evaluated at our Institution. MDCM was defined as LVEF <45% and LV end-diastolic volume index (LVEDVI) ≤70 mL/m2 in women and ≤89 mL/m2 in men. Among a total population o

True Left Bundle Branch Block by Strauss Criteria: Impact on CRT Response and Conduction System Pacing Trial Design-A Systematic Evidence Synthesis Toward Personalized Patient Selection.

Background/Objectives: About one-third of patients who receive cardiac resynchronization therapy (CRT) do not show meaningful clinical improvement. One possible reason is that current criteria for diagnosing left bundle branch block (LBBB) do not clearly separate true conduction block from other conditions that can produce a similar QRS pattern, since they rely mainly on surface ECG features. The Strauss criteria have been proposed as a stricter alternative. They include sex-specific QRS duration thresholds and require a mid-QRS notch or slur, with the goal of better identifying patients whose
