The changes in central and peripheral corneal thickness and topography during initial (1st-day) wear, adaptation to, and long-term wear of the DuraSoft lens were monitored in a group of 10 patients. Monitoring was by means of the electronic digital pachometer and the photoelectronic keratoscope (PEK). Ocular fundus photographs of 100 eyes were obtained from an institutionalized population of patients with Down's Syndrome (66 males, 34 females). The vascularity of the optic nerve head was assessed in each photograph by counting the number of arterioles, venules, and fine vessels crossing the disc margin. The results of this analysis were compared to similar data obtained from 100 ocular fundus photographs of normal patients and show that there are more large vessels crossing the disc margin in those with Down's Syndrome.

A 17-yr-old female, classified as retarded educable, was found to have uncorrected visual acuity of 20/800 in both eyes, unimproved by a pinhole disc. A low myopic correction determined objectively did not improve her visual acuity. Data on phorias, vergences, and fixation disparities were gathered for distant and near viewing conditions from a nonclinical, young adult population. Normative values were computed from the data for analysis and comparison. Our study revealed slightly less exophoria at far and near than Morgan's study, together with large base-out vergence readings. We found proportionately more esophores with exo fixation disparity who require base-in prism to neutralize the fixation disparity than Ogle's studies found.

Relatively mild ocular trauma may cause lesions within the anterior chamber which are easily overlooked. The frequent onset of secondary glaucoma after such injuries emphasizes the importance of carefully evaluating the anterior chamber for the presence of hyphema, iridodialysis, and anterior-chamber recession. A case is reported in which relatively mild trauma caused iridodialysis, anterior-chamber recession, and tearing of the iris stroma and pupillary margin.

Spectacles were prescribed for 5-year-old identical twins with similar high, uncorrected, isometropic myopia. One received the full correction, and the other was undercorrected by 4 D. The fully corrected subject was a constant esotrope at distance and near; the undercorrected subject exhibited exotropia at near only. Evaluations over a 5-month period indicated that the full correction was the treatment of choice.

This report describes several methods of degrading imagery through the central portion of a spectacle lens to provide a cosmetically acceptable means of obtaining single vision in the central field of patients with intractable diplopia. For 2 strabismic patients with annoying diplopia, we applied to 1 spectacle lens a centrally placed disc (about 1 inch diameter) consisting of (1) translucent tap,a (2) a +7 D Fresnel lens,b or (3) stippled, clear lacquer. Twenty patients with corneal disease were treated with a recently released soft lens specifically designed for use as a bandage. Patients wore their lenses continuously 24 hr a day. The results were similar to those of previous studies conducted with other lens designs. Improvement was frequent (10 of 12 cases) for bullous keratopathy and corneal graft, burn, or neuroparalytic lesion; limited (1 of 4 cases) for dry-eye syndrome; and absent (4 cases) for irregular cornea and vernal keratoconjunctivitis. No difficulties were found with the concurrent use of medication.

The axial chromatic aberration of the eye was determined as in previous studies by finding the lens required to eliminate or minimize blur of the fixation target when it is illuminated by a series of monochromatic sources. Objective measurement of the refraction of the eyes during this procedure showed that accommodation remained constant throughout. Cholinergic drugs and anticholinesterases have caused ocular complications including cataracts, iris cysts, retinal detachment, iritis, increased intraocular tension, allergic and toxic reactions, and hypermia. The visual complications include accommodative spasm; myopia and reduced visual acuity; changes in vision sensitivity, dark adaptation, and visual fields; and discomfort. The systemic complications are those common to cholinergic drugs and anticholinesterases. This review is intended as a ready reference to the drugs that cause such effects and source material.

The present paper reviews recent studies of retinal pathology from the American and British Journals of Ophthalmology during a one-year period (1973-1974).
