Full arch rehabilitation in severe maxillary atrophy with palatal approach implant placement: a case report.

PURPOSE: The edentulous severely atrophic maxilla, as consequence of alveolar bone resorption and pneumatisation of the maxillary sinus, represents a serious limitation to the implant rehabilitation. Implants insertion via palatal approach (PA), in combination with relatively minimally invasive techniques aimed at increasing bone volume without the use of autologous bone harvesting is a valid alternative among the options for the rehabilitation of the upper jaw. CLINICAL CASE: In a 70-year-old female, with a severe maxillary atrophy, 6 spiral taper implants were placed with the PA, combined w

'Pillow technique' to improve lip support in the context of zygoma implant rehabilitation.

This study proposes a simple, off-the-shelf ancillary method for application in the dental rehabilitation of severe maxillary atrophy with zygoma implants, allowing simultaneous improvement of lip support in cases with a moderate lack of premaxillary projection. Three consecutive patients with an atrophic maxilla were evaluated retrospectively. All were treated with a fixed rehabilitation over four zygomatic implants and the pillow technique. The study variables included radiological assessment of the premaxilla volume, upper lip and perinasal soft tissue changes, clinical complications, and s

Effect of barrier membranes on the outcomes of maxillary sinus floor augmentation: a meta-analysis of histomorphometric outcomes.

PURPOSE: Sinus floor elevation via the lateral window approach represents a reliable technique for augmenting bone volume in the atrophic posterior maxilla. However, controversy remains regarding the effect of placement of a barrier membrane over the lateral window. This histomorphometric meta-analysis sought to clarify the effect of barrier membranes in lateral window sinus augmentation. MATERIALS AND METHODS: An electronic search of three databases and a hand search in implant-related journals for studies published through January 2013 in the English language was conducted. Randomized contr

Prevalence, location and morphology of maxillary sinus septa: systematic review and meta-analysis.

AIM: To gain further insights and resolve conflicting results in the literature regarding prevalence, predominant location and morphologic variability of maxillary sinus septa. MATERIAL AND METHODS: Electronic and hand searching of English literature identified 33 investigations published from 1995 to 2011. Septa were defined as at least 2-4 mm in height. RESULTS: Septa were present in 28.4% of 8923 sinuses investigated (95% confidence interval: 24.3-32.5%). Prevalence was significantly higher in atrophic sinuses compared with dentate maxillae (p < 0.001). Septa were located in premolar, mol

Clinical Outcome and 8-Year Follow-Up of Alveolar Bone Tissue Engineering for Severely Atrophic Alveolar Bone Using Autologous Bone Marrow Stromal Cells with Platelet-Rich Plasma and β-Tricalcium Phosphate Granules.

BACKGROUND: Although bone tissue engineering for dentistry has been studied for many years, the clinical outcome for severe cases has not been established. Furthermore, there are limited numbers of studies that include long-term follow-up. In this study, the safety and efficacy of bone tissue engineering for patients with a severely atrophic alveolar bone were examined using autogenous bone marrow stromal cells (BMSCs), and the long-term stability was also evaluated. METHODS: BMSCs from iliac bone marrow aspirate were cultured and expanded. Then, induced osteogenic cells were transplanted wit

Histomorphometric comparison of maxillary pristine bone and composite bone graft biopsies obtained after sinus augmentation.

INTRODUCTION: Sinus grafting is a technique oriented to facilitate implant placement in posterior atrophic maxillae. Several modifications of the original technique and a wide variety of materials have been proposed; most of them associated with implant survival rates. However, the quality of the bone obtained after the application of certain grafting materials has not been fully elucidated yet. The aims of this multicenter study were to analyse histomorphometrical samples obtained 6 months after sinus grafting using a composite graft consisting of anorganic bovine bone (ABB)+ autologous bone

Prosthodontic rehabilitation using a patient-specific implant in an anterior maxilla with severe alveolar bone deficiency: A clinical report.

Severe alveolar bone deficiency in the anterior maxilla challenges prosthodontic rehabilitation, particularly when conventional endosseous implants are unfeasible. This clinical report describes the prosthodontic rehabilitation of a patient with extensive anterior maxillary bone deficiency using a digitally planned patient-specific implant. Following clinical and radiographic evaluation, a custom implant was designed using 3-dimensional imaging and computer-aided design to adapt to the residual bone anatomy. The implant was surgically placed and restored as part of a complete mouth prosthodont

Delayed-onset maxillary sinus-cutaneous fistula of the lower eyelid following zygomatic implant removal: A case report.

Zygomatic implants are an effective option for rehabilitating severely atrophic maxillae, but can occasionally cause complications involving the maxillary sinus and surrounding soft tissues. We report a rare case of a delayed cutaneous fistula that developed 1 year after zygomatic implant removal. A 62-year-old man with progressive tooth loss due to familial aggressive periodontitis underwent zygomatic implant placement, which failed because of sinusitis and was subsequently removed. One year later, he presented with a mass at the lateral lower eyelid. Computed tomography demonstrated a linear

Treatment of Mucous Retention Cyst in Association with Sinus Lift and Implant Placement: A Case Report with 1-Year Follow-Up.

Sinus lift augmentation techniques, lateral or crestal approaches, have been well documented, with bone substitute graft, or without bone material, with immediate or delayed implant placement as a treatment option for the atrophic maxilla in the posterior area. However, the sinus lift procedures performed in the presence of cysts, mucoceles, mucous retention cysts (MRCs), and antral pseudo-cysts could mainly decrease the sinus cavity volume and could increase the possibility of ostium obstruction and might lead to infection followed by failure of the grafting procedure. A radiological assessme

Clinical Assessment of the Bone Width Following Lateral Ridge Expansion in Augmentation of Narrow Alveolar Ridges for Placing Immediate Dental Implants.

BACKGROUND: Less residual alveolar bone at extraction site at esthetic and functional tooth position is the main limitation in placing a dental implant, especially at long-span edentulous ridges. This needs ridge augmentation. AIMS: To assess bone width gain and implant stability following narrow ridge augmentation using the vertical ridge split technique. MATERIALS AND METHODS: In 22 subjects within the age group of 20-60 years and the mean age of 38.46 years. The vertical ridge split technique was done in all subjects for horizontal ridge augmentation. Paraesthesia, pain/discomfort, mobili

Implant survival rates after maxillary sinus augmentation.

Implant therapy in the atrophic posterior maxilla becomes challenging in the presence of reduced maxillary bone height. Sinus augmentation can be performed for resolving this condition prior to implant placement. The aim of this article was therefore to evaluate implant survival rates in the grafted sinus taking into account the influence of the implant surface, graft material, and implant placement timing. A systematic review of the literature was performed. Articles retrieved from electronic databases were screened using specific inclusion criteria, and data extracted were divided according
