BACKGROUND: Global rollout of dolutegravir-based antiretroviral therapy (ART) has been hampered in part by insufficient safety data in pregnancy. We compared birth outcomes among women initiating dolutegravir-based ART with those among women initiating efavirenz-based ART in pregnancy in Botswana.
METHODS: In this observational study, we captured birth outcome data at eight government hospitals throughout Botswana (~45% of all deliveries in the country) in an ongoing study that started on Aug 15, 2014. In 2016, Botswana changed first-line ART from efavirenz-tenofovir-emtricitabine to dolutegr

Huntington's disease-like 2 (HDL2) is a rare autosomal dominant progressive neurodegenerative disorder commonly seen in adults. It was first described in a large African-American family in the United States. HDL2 clinically resembles Huntington's disease (HD) and causes adult-onset relentlessly progressive movement, emotional and cognitive dysfunction. Onset is usually in the fourth decade with slow progression to death. We present a 47-year-old male Botswana native, with a four-year-history of chorea, slurred speech, mood instability, cognitive impairment and weight loss. Genetic testing reve

BACKGROUND: Gender power imbalance, which translates into a power imbalance in sexual interactions, is increasingly being recognized as a factor in fueling the spread of HIV/AIDS by increasing the number of unsafe sexual encounters.
OBJECTIVES: To examine the influence of gender power imbalance and other factors on women's capacity to negotiate self protection against HIV infection; as well as men's response to the suggested condom use.
METHODS: Drawing on data gathered from 2658 women aged 18-49 years in a cross-sectional survey in Kwa Zulu Natal Province of South Africa and Botswana, the s

BACKGROUND: Primary health care (PHC) systems across many low- and middle-income countries increasingly rely on mixed provider environments involving public, private, and mission-based actors. Public-private partnerships (PPPs) are often promoted as a way of improving coordination and expanding access to care. However, many PHC partnerships continue to be governed through highly centralized and compliance-oriented arrangements that may limit flexibility, trust, and long-term collaboration. Botswana provides a useful case for examining how PPPs are governed within a health system experiencing f

OBJECTIVE: Globally, growing attention has been directed toward the sexual health and developmental experiences of gay, bisexual, transgender, queer, and intersex (GBTQI+) youth. Existing literature links childhood sexual victimization, familial rejection, bullying, and gender nonconformity to adverse sexual health outcomes. Evidence further suggests that sexual and gender minority youth are often targeted for abuse before they self-identify based on perceived gender nonconformity; yet, despite these insights, little is known about these dynamics in Botswana. This study applies an intersection

INTRODUCTION: Prostate cancer (CaP) is the most common malignancy and the second leading cause of cancer-related deaths among men in Botswana. Currently, diagnosing CaP relies on examining prostate biopsy samples, which can be challenging due to benign mimics. This study aims to evaluate the potential of Alpha-methyl acyl-CoA racemase (AMACR/p504s) and p63, as diagnostic markers for CaP. This may potentially validate the use of immunohistochemistry for detecting CaP in Botswana, where it is not routinely utilized.
METHODS: The study included 69 samples, comprising 5 prostatic chip specimens,

Botswana implemented its universal "Treat All" antiretroviral therapy (ART) policy in 2016, expanding treatment eligibility to all people living with HIV (PLHIV). HIV has been known to be a leading risk factor for tuberculosis (TB) and poor TB treatment outcomes. The primary goal of this study is to assess whether HIV infection and HIV-associated immunosuppression remain risk factors for unfavorable TB treatment outcomes in the Post-Treat All era. We analyzed 636 TB patients treated in Gaborone (2017-2023), of whom 54.4% were HIV-positive. Unfavorable outcomes (death, failure, or loss to follo

BACKGROUND: The prevalence of substance use disorders (SUDs) is rising globally, significantly affecting public health. These disorders are associated with reduced quality of life, comorbid mental health conditions, and increased risk of infectious diseases such as HIV. This paper describes the development and implementation of group therapy as an intervention for SUDs in a resource-constrained setting.
PROGRAM DESCRIPTION: An 8-week inpatient group therapy program was established at Sbrana Psychiatric Hospital in Botswana. The interdisciplinary team includes a social worker, psychologist, oc

Diarrheal disease is a leading cause of death among young children globally. Current guidelines recommend supportive treatment of acute diarrhea and using antimicrobials only with presence of blood in the stool. Select enteric pathogens, including Shigella, commonly cause disease in high-burden settings; targeted treatment of these pathogens could decrease morbidity and mortality. In settings with limited access to microbiological testing, practical diagnostics are needed to differentiate treatable causes of pediatric diarrhea. Evolving evidence suggests fecal calprotectin (fCal) could help di

Promotive social protection programs aim to increase income and capabilities and could help address structural drivers of HIV-vulnerability like poverty, lack of education and gender inequality. Unemployed and out-of-school young women bear the brunt of HIV infection in Botswana, but rarely benefit from such economic empowerment programs. Using a qualitative exploratory study design and a participatory research approach, we explored factors affecting perceived program benefit and potential solutions to barriers. Direct stakeholders (n = 146) included 87 unemployed and out-of-school young women

Accurate HIV and CD4 testing are critical in program implementation, with HIV misdiagnosis having serious consequences at both the client and/or community level. We implemented a comprehensive training and Quality Assurance (QA) program to ensure accuracy of point-of-care HIV and CD4 count testing by lay counsellors during the Botswana Combination Prevention Project (BCPP). We compared the performance of field testing by lay counsellors to results from an accredited laboratory to ascertain accuracy of testing. All trained lay counsellors passed competency assessments and performed satisfactori

INTRODUCTION: Antiretrovirals such as dolutegravir (DTG) and tenofovir alafenamide (TAF) have been associated with excessive weight gain. The objective of this study was to understand the potential impact of ART-associated weight gain on pregnancy outcomes among women living with HIV.
METHODS: Using data from the Tsepamo birth outcomes surveillance study in Botswana, we evaluated the relationship between maternal weight (and weight gain) and severe birth outcomes (very preterm delivery <32 weeks, very small for gestational age (SGA) <3rd percentile, perinatal death), macrosomia (birthweight >

BACKGROUND: Evidence to inform cryptococcal antigen (CrAg)-screening guidelines among ART-experienced populations is lacking. We performed a study evaluating the utility of reflex CrAg screening in Gaborone, Botswana.
METHODS: CD4 count data were collected from the HIV reference laboratory from 2014-2016. CrAg screening was performed on samples with CD4 ≤100 cells/µL beginning January 2015. The proportion of CD4 counts ≤100 cells/µL was determined and the frequency of repeat CrAg testing described. Analyses ascertained the impact of ART status on CrAg prevalence and outcomes, and whether CrAg

Vitamin B12 deficiency is a rare diagnosis in young children. We present the case of a 1-year-old Zimbabwean child with profound anemia. Further testing revealed undetectable levels of vitamin B12 and positive intrinsic factor antibodies that were drawn after the initiation of empiric treatment with parenteral vitamin B12. We report the evaluation and management of vitamin B12 deficiency in a resource-limited setting. Vitamin B12 deficiency should be considered in children presenting with unexplained cytopenias with consideration of empiric treatment with parenteral vitamin B12, as development
