Incidence, preventability, and causality of adverse drug reactions at a university hospital emergency department.

PURPOSE: To investigate the characteristics of ADRs in patients admitting at the emergency room of a tertiary hospital. METHODS: We collected the patient records of 1600 emergency room visits of a university hospital in 2018. The patient files were studied retrospectively and all possible ADRs were identified and registered. Patient characteristics, drugs associated with ADRs, causality, severity, preventability, and the role of pharmacogenetics were assessed. RESULTS: There were 125 cases with ADRs, resulting in a 7.8% overall incidence among emergency visits. The incidence was greatest in

Reassessment of causality of ABCC6 missense variants associated with pseudoxanthoma elasticum based on Sherloc.

PURPOSE: Pseudoxanthoma elasticum (PXE) is a heritable disorder affecting elastic fibers in the skin, eyes, and cardiovascular system. It is caused by biallelic pathogenic variants in the ABCC6 gene. To date, over 300 ABCC6 variants are associated with PXE, more than half being missense variants. Correct variant interpretation is essential for establishing a direct link between the variant and the patient's phenotype and has important implications for diagnosis and treatment. METHODS: We used a systematic approach for interpretation of 271 previously reported and 15 novel ABCC6 missense varia

[Benzodiazepines, cognitive decline and dementia: A review of causality criteria from published observational studies].

OBJECTIVE: Benzodiazepines are a source of well-known adverse reactions related to their pharmacological properties: memory disorders, falls in the elderly, confusion and sedation, drug addiction. The impact of their chronic use on cognitive decline and dementia has been the subject of many studies for two decades. METHODS: From 25 studies identified in the international literature from 1997 to 2017, the Bradford-Hill causality criteria were systematically applied. RESULTS: Fifteen studies found a statistically significant association with a deleterious effect, two a protective effect and ei

Evaluating the causality of novel sequence variants in the prion protein gene by example.

The estimation of pathogenicity and penetrance of novel prion protein gene (PRNP) variants presents significant challenges, particularly in the absence of family history, which precludes the application of Mendelian segregation. Moreover, the ambiguities of prion disease pathophysiology renders conventional in silico predictions inconclusive. Here, we describe 2 patients with rapid cognitive decline progressing to akinetic mutism and death within 10 weeks of symptom onset, both of whom possessed the novel T201S variant in PRNP. Clinically, both satisfied diagnostic criteria for probable sporad

Spontaneous coronary artery dissection and takotsubo syndrome: The chicken or the egg causality dilemma.

Acute coronary syndrome (ACS) including spontaneous coronary artery dissection (SCAD) has been reported to trigger its own clinical twin takotsubo syndrome (TS). The pathogenetic association between SCAD and TS remains to be elucidated. The two diseases afflict predominantly women and both conditions may be triggered by an emotional stress factor and an unusual extreme physical exercise. Herein, we describe a case of 54-year-old woman presenting with ACS caused by an obtuso-marginal SCAD. Concurrently, the patient had typical clinical features and course of mid-apical pattern of TS. The causal

Pancreatitis and portal vein thrombosis in children: the chicken or the egg causality dilemma.

BACKGROUND: The association of pancreatitis and portal vein thrombosis (PVT) is extremely rare in children. We report on 3 cases which suggest that there may be a causal relation between the two. METHODS: Clinical characteristics and evolution of 3 children with this particular condition were analyzed retrospectively. In this group of patients, the strategy consisted in opting for early surgical decompression of the portal hypertension, which was followed by a favorable outcome, not only in terms of complications related to the portal hypertension but also of a contemporaneous spontaneous reg

In vitro drug causality assessment in Stevens-Johnson syndrome - alternatives for lymphocyte transformation test.

BACKGROUND: Patients with Stevens-Johnson syndrome (SJS) or toxic epidermal necrolysis (TEN) are often exposed simultaneously to a few potentially culprit drugs. However, both the standard lymphocyte transformation tests (LTT) with proliferation as the assay end-point as well as skin tests, if done, are often negative. OBJECTIVE: As provocation tests are considered too dangerous, there is an urgent need to identify the relevant drug in SJS/TEN and to improve sensitivity of tests able to identify the causative drug. METHODS: Fifteen patients with SJS/TEN with the ALDEN score ≥ 6 and 18 drug-e

Fetal growth restriction and maternal smoking in the Macedonian Roma population: a causality dilemma.

PURPOSE: Macedonia is one of the top five countries globally in reported smoking rates. Over 10 % of the population consists of the underprivileged Roma minority. We aimed to determine whether Roma ethnicity is an independent risk factor for adverse pregnancy outcome or merely mediating maternal smoking. METHODS: Maternal data were retrieved from the perinatal computerized database for all deliveries during 2007-2011 at the only Clinical Hospital in Bitola, Macedonia. Multivariable regression models were constructed to control for confounders. RESULTS: Of nearly 7,000 deliveries, 8.65 % were

Time versus causal inferences: Action-outcome learning and prediction differentially shape the experience of causality and simultaneity.

The human brain is capable of recalibrating the perceived timing of events to achieve multisensory temporal grouping. For example, systematic delays separating an action and its subsequent outcome are reduced in perception, promoting the perception of simultaneity and recalibrating people's experience of causality. In this study we investigated whether sensorimotor recalibration depends on the ability to predict the specific outcome an action produces and whether this process differs for simultaneity and causal judgments. Across two experiments, we replicated the classic recalibration effect:

Intolerance of uncertainty, causal uncertainty, causal importance, self-concept clarity and their relations to generalized anxiety disorder.

Although numerous studies have provided support for the notion that intolerance of uncertainty plays a key role in pathological worry (the hallmark feature of generalized anxiety disorder (GAD)), other uncertainty-related constructs may also have relevance for the understanding of individuals who engage in pathological worry. Three constructs from the social cognition literature, causal uncertainty, causal importance, and self-concept clarity, were examined in the present study to assess the degree to which these explain unique variance in GAD, over and above intolerance of uncertainty. Exploring the Causal Relationship Between Blood Metabolites and Chronic Periodontitis: Insights From Genetic Causal Analysis.

To investigate the potential bidirectional causal relationship between specific blood metabolites and chronic periodontitis using Mendelian randomisation (MR). A two-sample bidirectional MR analysis was conducted. Data on 1400 blood metabolites were obtained from a genome-wide association study involving 8299 participants. Chronic periodontitis data were sourced from the FinnGen consortium, comprising 4784 cases and 272,252 controls. The primary analysis involved the inverse-variance weighted (IVW) method. Furthermore, a metabolic pathway enrichment analysis was conducted on the metabolites id

Causal mediation analysis with multiple causally non-ordered and ordered mediators based on summarized genetic data.

Causal mediation analysis investigates the mechanism linking exposure and outcome. Dealing with the impact of unobserved confounders among exposure, mediator and outcome is an issue of great concern. Moreover, when multiple mediators exist, this causal pathway intertwines with other causal pathways, rendering it difficult to estimate the path-specific effects. In this study, we propose a method (PSE-MR) to identify and estimate path-specific effects of an exposure (e.g. education) on an outcome (e.g. osteoarthritis risk) through multiple causally ordered and non-ordered mediators (e.g. body ma

Protocol for mapping frequency-specific information transfer between cortical areas using ECoG and Granger causality.

This protocol details the application of Granger causality (GC) analysis to primate electrocorticography (ECoG) data to map frequency-specific information transfer. We describe steps for computing pairwise GC and generating spatial GC maps, including differential maps to isolate task-specific activity. We then explain how to perform hierarchical clustering on GC patterns to identify and visualize the spatial distribution of functional electrode clusters, revealing the organization of information flow. For complete details on the use and execution of this protocol, please refer to Zhou et al.1.

Gut microbiota-host co-metabolism in hypertriglyceridemia-associated acute pancreatitis: from causality to precision intervention.

Clinical outcomes in hypertriglyceridemia-associated acute pancreatitis remain highly heterogeneous. Despite comparable lipid burdens, patients may experience anything from mild interstitial edema to fulminant necrotizing disease, a divergence that current models struggle to explain. Hypertriglyceridemia-associated acute pancreatitis (HTG-AP) now accounts for roughly one in nine acute pancreatitis cases worldwide, but serum triglyceride levels alone do not explain the significant East-West mortality gap (4.1% versus 1.0%). Emerging evidence shifts the focus away from the pancreas itself and to

The OmniAge compendium of aging omic biomarkers links mitotic clocks to clonal hematopoiesis and causality.

Interest in aging 'omic' biomarkers has grown due to their ability to quantify biological age. Most of these biomarkers have been derived in blood and fall into many diverse categories, yet relatively little is known about their correlative patterns, especially between biomarkers from different categories. Here we present the OmniAge R and Python package, a collection of 413 aging omic biomarkers representing 12 different categories, including traditional epigenetic clocks, epigenetic mitotic clocks, DNA methylation-based proxies for clonal hematopoiesis and inflammaging, causal clocks, cell-t

Using Google Trends to Predict Dengue Outbreaks in Pakistan: A Time-Series and Causality Analysis.

BACKGROUND: Dengue fever is a significant public health issue in Pakistan. Seasonal outbreaks lead to considerable illness and economic strain. Traditional surveillance systems, like the Integrated Disease Surveillance and Response (IDSR) system, depend mainly on passive reporting. They often identify outbreaks only after transmission has already worsened. Digital data sources, such as Google Trends, might provide early signs of increasing disease activity by reflecting changes in public information-seeking behavior. OBJECTIVE: To determine if Google search activity for dengue-related terms c

Economic Evaluation of Multi-Objective Schistosomiasis Control Through Systemic Causality: Theoretical Advances and Governance Implications.

Schistosomiasis elimination is increasingly constrained less by the technical efficacy of single interventions than by systemic dynamics in coupled human-animal-environment settings, including nonlinear feedback, spatial heterogeneity, and cross-sectoral govern frictions. We conducted a systematic methodological review (search date: 1 January 2026) across PubMed, Web of Science, Scopus, EconLit, and CNKI to identify studies that (i) addressed schistosomiasis control, (ii) used explicit system-based, causal, or network-oriented analytical structures, and (iii) incorporated economic evaluation w

Causality between periodontitis and schizophrenia, bipolar disorder, and major depressive disorder: A mendelian randomization study.

BACKGROUND: Increasing evidence suggests that periodontitis (PD) is epidemiologically associated with mental health. To minimize the influence of confounding factors, we employed Mendelian randomization (MR) as the analytical approach to investigate the potential causal relationships between PD and schizophrenia (SZ), bipolar disorder (BD), and major depressive disorder (MDD). METHODS: 8 single nucleotide polymorphisms (SNPs), sourced from a genome-wide association study (GWAS) of 45,563 individuals were used as instrumental variables for PD. 131, 52 and 0 SNPs were employed from 3 GWASs of S
