3D-reconstruction of craniofacial structures of a human anencephalic fetus. Case report.

It was the purpose of this study to investigate the craniofacial bones, cartilages and major blood vessels of a human anencephalic fetus and to describe the malformations of these structures accompanying this developmental disruption. The head of a 16 week old human fetus with anencephaly was serially sectioned and the craniofacial bones, cartilages and major blood vessels were traced and reconstructed by 3D-computer technology. The sphenoidal, temporal and occipital bones showed severe malformations, whereas the bones of the facial cranium were normally developed. Both internal carotid arteri

Poly(vinyl alcohol) induced chirality inversion and amplification of circularly polarized room-temperature phosphorescence in homopolypeptide aggregates.

Organic circularly polarized room-temperature phosphorescence (CPRTP) materials represent an emerging research frontier with broad application prospects. However, achieving efficient and controllable CPRTP remains challenging due to inefficient chirality transfer and the limited ability to regulate chiral environments. Here, we develop a strategy involving self-assembled chiral homopolypeptides to realize manipulable CPRTP. Chiral homopolypeptides functionalized with achiral phosphorescent terminals are first designed, which self-assemble into vesicles exhibiting weak CPRTP. Remarkably, disper

Cerebellar Bergmann glia integrate noxious information and modulate nocifensive behaviors.

The cerebellum is activated by noxious stimuli and pathological pain but its role in noxious information processing remains unknown. Here, we show that in mice, cutaneous noxious electrical stimuli induced noradrenaline (NA) release from locus coeruleus (LC) terminals in the cerebellar cortex. Bergmann glia (BG) accumulated these LC-NA signals by increasing intracellular calcium in an integrative manner ('flares'). BG flares were also elicited in response to an intraplantar capsaicin injection. Chemogenetic inactivation of LC terminals or BG in the cerebellar cortex or BG-specific knockdown of

Role of computed tomography of abdomen in difficult to diagnose typhoid fever: a case series.

Background and Aim Diagnosis of typhoid is challenging when blood cultures fail to isolate Salmonella species. We report our experience with interpreting computed tomography (CT) abdomen findings in a case series of typhoid fever. Methods The case series consisted of patients who had a CT abdomen done as part of their investigations and a final diagnosis of typhoid fever. The CT films were reviewed and findings evaluated for distinctive features. Results During 2011-2017, 11 patients met the inclusion criteria. Indication for CT was pyrexia of unknown origin in the majority of patients. Review

Intraoperative ear bleeding with bilateral otorrhagia during laparoscopic sacrocolpopexy.

BACKGROUND: Spontaneous perioperative otorrhagia is an extremely rare entity with only 4 cases reported in the literature to date, all of which were recognized after the termination of the procedure. CASE: We describe the first reported case of otorrhagia recognized intraoperatively causing abortion of the procedure in a 60-year-old woman undergoing laparoscopic sacrocolpopexy. Otoscopy by the otolaryngology service revealed multiple intracutaneous hematomas and bleeding in bilateral external auditory canals. Computed tomography scan of the head revealed no intracranial hemorrhage. She underw

Tests for treatment group differences in the hazards for survival, before and after the occurrence of an intermediate event.

In many settings, one would expect that the hazard for a terminal event would change with the occurrence of an intermediate event. For example, in an AIDS clinical trial, it is of interest to assess whether there is a difference between treatments in the hazards for death prior to drop in Karnofsky performance score and in the hazards subsequent to the drop in Karnofsky score. Tests for the effect of treatment on these hazard functions, separately or jointly, are useful in conjunction with tests of overall survival. We consider four Cox regression models for the hazard function, constructed by

[Intestinal invagination due to endometriosis of the terminal ileum].

HISTORY AND CLINICAL FINDINGS: A 39 year old female patient presented with nausea and crampy abdominal pain in the right lower quadrant. INVESTIGATIONS: Physical examination showed a markedly distended abdomen with a diffuse pain, punctum maximum in the right lower part of the abdomen and reduced peristalsis. Plain abdominal x-rays revealed slightly dilated loops of small bowel with air-fluid levels. Abdominal ultrasound revealed thickening of the bowel wall of the distal ileum and some free fluid. To exclude Crohn's disease, Sellink-computer tomography was performed, which also showed thicke

Intra-Atrial Right Coronary Artery: A Condition Revealed by Coronary CT Scan.

Coronary artery anomalies are rare and may involve the coronary origin, course, or termination. Most are benign; however, some can lead to myocardial damage, sudden death, or potential hazards during interventional or surgical procedures. The advancement of imaging studies has led to an increase in their diagnosis. Coronary computed tomography angiography (CCTA) especially has characterized new anomalies that were previously anecdotal. This is the case of the intra-atrial course of the right coronary artery. The present is a brief review about this rare congenital anomaly.

Myocardial infarction due to a retained epicardial pacing wire.

A 20-year-old man with a temporary epicardial pacing wire retained after infant repair of an atrioventricular septal defect presented with cardiopulmonary arrest. After resuscitation with extracorporeal membranous oxygenation, a coronary angiogram demonstrated a retained epicardial pacing wire compressing the left anterior descending artery and obtuse marginals. Owing to poor prognostic findings on a computed tomography scan of his head, he was terminally withdrawn from mechanical support. This case demonstrates a potential devastating complication of a retained temporary epicardial pacing wir

Intractable supraventricular tachycardia as first presentation of thoracic aortic dissection: case report.

A patient presented with palpitations at the emergency department 3 days after a percutaneous coronary intervention complicated by dissection of the left anterior descending and circumflex coronary arteries. Physical examination revealed a high pulse rate and low blood pressure and the electrocardiogram demonstrated atrioventricular nodal re-entry tachycardia. This arrhythmia was eventually terminated by electrical cardioversion. Echocardiography demonstrated moderate aortic regurgitation and subsequent computed tomography showed a large Stanford type A aortic dissection. The patient was succe

[Ileocoecal intussusception of the terminal ileum caused by a carcinoid tumor].

Intussusception of the intestine is a frequent reason for intestinal obstruction in infants; the course is acute but the causes are often not recognizable. In adults intussusception is the cause for bowel obstruction in only 1% of cases. The course is subacute and in over 90% a benign or malignant process in this part of the intestine is found. We report the case of a 45-year-old male with abdominal discomfort, increasing over weeks. The preoperative ultrasonography and computed tomography of the abdomen revealed the typical finding of ileocoecal intussusception. An ileocoecal resection was pe

Anaphylaxis due to Hymenoptera sting progressing to thoracic aortic dissection.

Here we present an 83-year-old woman with truncal pain, hypoxemia, and nausea after a Hymenoptera sting. Due to progressive truncal pain, emergent computed tomography angiography was ordered and confirmed an acute Stanford type A aortic dissection extending from the aortic root and terminating in the left common iliac artery. She was emergently transferred to a quaternary care center and managed surgically. This case highlights anaphylaxis as a unique potential trigger of aortic dissection and the need for a high index of suspicion for early diagnosis.

Intestinal tuberculosis in a patient with Cushing's syndrome.

A 39-year-old woman presented with cushingoid features was worked up and diagnosed to have ACTH-independent Cushing's syndrome. Computed tomography of the whole abdomen revealed a left adrenal mass. She was scheduled for elective laparoscopic left adrenalectomy, however, a few days prior to the surgery, the patient had hematochezia. Colonoscopy revealed multiple ulcers on the terminal ileum, to which biopsy revealed Mycobacterium tuberculosis infection. The patient underwent laparoscopic left adrenalectomy on the same admission, which revealed adrenal adenoma on histopathology.

Gastric Outlet Obstruction Due to Malposition of Replacement Gastrostomy Tube.

A 78-year old male presented to the emergency department after accidental dislodgement of his chronic gastrostomy tube. A replacement gastrostomy tube was passed easily through the existing stoma and flushed without difficulty. Confirmatory abdominal radiography demonstrated contrast in the proximal small bowel, but the patient subsequently developed epigastric pain and refractory vomiting. Computed tomography revealed the tip of the gastrostomy tube terminating in the pylorus or proximal duodenum. This case highlights gastric outlet obstruction complicating the replacement of a gastrostomy tu

Incidentally Discovered Middle Mesenteric Artery In A Renal Donor.

The middle mesenteric artery is a very rare anomalous artery originating from the ventral surface of the abdominal aorta in-between the superior mesenteric artery and inferior mesenteric artery. We identified a middle mesenteric artery during abdominal computed tomographic angiography in a renal donor patient as a part of his work up. The middle mesenteric artery branched out into ileal and ileocolic arteries, supplying the terminal ileal loops as well as the cecum. The anomalous artery had no effect on patient's eligibility as a renal donor candidate.

[Adult intussusception in small bowel due to tissue containing ectopic ventricular mucosa].

Adult intussusception is rare and most often not identified until surgery. We report a case involving a 59-year-old woman admitted to the surgical ward. Computed tomography identified intestinal obstruction caused by intussusception of the terminal ileum. Exploratory laparotomy was performed - an intussusception was identified 30 cm from the ileocoecal valve. The intussusception was caused by what looked like a 6 x 2 cm tumour. Histology revealed no tumour but ectopic ventricular mucosa and no indication of malignancy. The case and literature on the subject are discussed.

[Cystic adenomatoid malformation: late diagnosis of a congenital anomaly].

Cystic adenomatoid malformation (CAM) is a congenital lung malformation characterized by adenomatoid proliferation of alveoli and terminal bronchioli. It is usually diagnosed perinatally, but some silent cases are discovered during childhood or adulthood. We present the case of a 9-year-old girl who presented with chest pain. Chest X-ray showed a cystic image in the upper lobe of the left lung, which was confirmed by computed tomography and magnetic resonance angiography. Lobectomy of the affected segment was performed and histological examination confirmed the diagnosis of CAM type I.

Biomechanical effects of far-cortex over-penetration in locking plate fixation: A finite element study.

This study quantified the biomechanical consequences of incremental far-cortex over-penetration at terminal locking screw positions in a locking compression plate construct using a validated three-dimensional finite element model of a femoral diaphyseal fracture (AO/OTA 32-A3). Four terminal screw engagement configurations were evaluated: unicortical fixation, fully contained bicortical fixation, 1-thread over-penetration (0.6 mm), and 3-thread over-penetration (1.8 mm). Axial compression (1000 N), torsional loading (±10 Nm), and cyclic dynamic loading (100-1000 N, 1 × 106 cycles) simulating t

A DCT-UNet-based framework for pulmonary airway segmentation integrating label self-updating and terminal region growing.

Objective.Intrathoracic airway segmentation in computed tomography is important for quantitative and qualitative analysis of various chronic respiratory diseases and bronchial surgery navigation. However, the airway tree's morphological complexity, incomplete labels resulting from annotation difficulty, and intra-class imbalance between main and terminal airways limit the segmentation performance.Approach.Three methodological improvements are proposed to deal with the challenges. Firstly, we design a dilated contextual transformer-UNet to collect better information on neighboring voxels and on
