Outcome of free digital artery perforator flap transfer for reconstruction of fingertip defects.

BACKGROUND: Fingertip defect can be treated with many flaps such as random pattern abdominal flap, retrograde digital artery island flap, V-Y advancement flap, etc. However, swelling in the fingertip, dysfunction of sensation, flexion and extension contracture or injury in the hemi-artery of the finger usually occurs during the recovery phase. Recently, digital artery perforator flaps have been used for fingertip reconstructions. With the development of super microsurgery techniques, free flaps can be more effective for sensory recovery and durability of the fingertip. MATERIALS AND METHODS:

Thin superficial circumflex iliac artery perforator flap and supermicrosurgery technique for face reconstruction.

Distant free flaps have become a routine option for reconstruction of large, complicated facial soft tissue defects. The challenge is to find a flap that is pliable to provide good contour and function. The purpose of this paper was to evaluate the use of superficial circumflex iliac artery perforator (SCIP) flaps for facial defects. From November 2010 to June 2013, facial reconstruction was performed on 6 patients (age range, 15-79 years). The harvesting technique was modified to elevate above the deep fat, and the pedicles were taken above or just below the deep fascia. The mean size of the

Perforator-based radial forearm fascial flap for management of recurrent carpal tunnel syndrome.

PURPOSE: To study the benefit of using the perforator-based radial forearm fascial flap for patients with recurrent carpal tunnel syndrome. METHODS: We used a perforator-based radial forearm fascial flap in 8 patients to cover the median nerve. All of the patients had undergone an index carpal tunnel release, and 3 of them had undergone at least 1 revision surgery to further decompress the median nerve. RESULTS: At average of 20 months (range, 6-30 mo) after the forearm fascial flap, all patients reported symptomatic improvement with complete resolution of nighttime symptoms. No patient repo

[The clinic application of thoracodorsal artery perforator flap: a report of 16 cases].

OBJECTIVE: To investigate the effects of free and pedicled thoracodorsal artery perforator (TDAP) flaps for repairing skin and soft tissue defects in limbs, neck, axillary and shoulder. METHODS: From October 2009 to Auguest 2011, 16 TDAP flaps were used to repair skin and tissue defects. Among them, five ipsilateral pedicled flaps were used to repair wounds in neck, axillary and shoulder. 11 free TDAP flaps were used to repair the wounds with bone or tendon exposure. In 12 cases, the flaps were pedicled with thoracodorsal artery and vein-lateral branches-perforators, in 4 cases, pedicled with

The medial sural artery perforator flap: a versatile donor site for hand reconstruction.

BACKGROUND: The challenge of modern hand reconstruction goes beyond simple coverage. Thanks to the advances of microsurgery, there are ever-improving standards of functional and esthetic outcomes in hand reconstruction. The versatile donor site of the medial sural artery perforator flap can fulfill this purpose. MATERIALS: Between June 2006 and October 2008, we used free medial sural artery perforator flaps for hand reconstruction in 14 cases. The sites of reconstruction included digits (n=7), dorsal hand (n=3), palmar hand (n=2), and wrist (n=2). Associated tendon and nerve defects were foun

Gluteal artery perforator flap: a viable alternative for sacral radiation ulcer and osteoradionecrosis.

Radiotherapy is a crucial part in the treatment of cancer; however, it may cause adverse effects to normal tissue such as radiation-induced ulcer and osteoradionecrosis. The few cases of conservative management that were reported had a limited value and unsatisfactory results. The most reliable method to treat sacral radiation ulcer and osteoradionecrosis is a wide excision of the affected tissue, followed by coverage with well-vascularised tissue. Musculocutaneous free flaps and local gluteus maximus musculocutaneous flaps have been used; however, there were many drawbacks such as dissection

Versatility of the pedicled thoracodorsal artery perforator (TDAP) flap in soft tissue reconstruction.

For the last decades, the latissimus dorsi skin-muscle flap has contributed to the efficient reconstruction of the loss of skin cover (especially in breast surgery) and in long-distance tissue defects. Unfortunately, the nonuse of such an important muscle as the latissimus dorsi for the patient, as well as the resulting thickness of the flap after reconstruction, has turned it into a second choice flap. However, this flap is still indicated in the reconstruction of areas which need a great amount of cutaneous and muscular tissue. The appearance of the perforator flaps and, specifically, thorac

The internal mammary artery perforator flap: an anatomical study and a case report.

The anatomic basis for the internal artery mammary perforator (IMAP) flap is described in this cadaveric study, together with a clinical case report. The IMAP flap is based on a single or double perforator of the internal mammary artery that is included in the pedicle for added length. It provides a very useful source of local tissue with skin of good texture and color for head and neck reconstruction and, being muscle free, is thin. With preservation of the anterior cutaneous branch of the intercostal nerve, the flap has the potential to be sensate. A large area can be covered, particularly i

Perforator types and anatomical characteristics of the deep circumflex iliac artery perforator flap with iliac crest: findings based on 40 clinical cases.

The aim of this study was to report the clinical experience of repairing mandibular defects with a deep circumflex iliac artery perforator flap with iliac crest (DCIAPF) and to analyse the relevant anatomical data to guide clinical application. Forty patients with mandibular defects, who underwent reconstruction with a DCIAPF after oncological resection were included in the study. During the operation, anatomical features relevant to the structure of the DCIAPF were measured, including the position of the perforator, mobility of the skin paddle, length of the vascular pedicle, and adipose tiss

Traumatic Forefoot Reconstructions With Free Perforator Flaps.

The forefoot is critical to normal walking; thus, any reconstruction of forefoot defects, including the soft tissues, must be carefully done. The free perforator flap, with its physiologic circulation, lower donor site morbidity, and minimal thickness is the most popular technique in plastic and microsurgery, and is theoretically the most suitable for such forefoot reconstruction. However, these flaps are generally recognized as more difficult and time-consuming to create than other flaps. In 41 patients with traumatic forefoot defects, we reconstructed the forefoot integument using 5 types of

Inadvertent free intercostal artery perforator flaps.

Two cases of free lateral intercostal artery perforator flaps are presented in this article. Both flaps were raised instead of thoracodorsal artery perforator flaps, which had been the initial operative plan. The free intercostal artery perforator flap can be technically difficult, the pedicle is relatively short and the vessel diameters can be small. The pedicle may need to be dissected along the intercostal groove to obtain sufficient vessel length and diameter. Despite all these issues, we describe a relatively straightforward salvage operation if a perforator flap raised on a presumed thor

Analysis of the distribution of cutaneous perforators in cutaneous flaps.

An anatomic and statistical analysis was performed on the distribution of cutaneous perforators that perfuse the scapular, radial forearm, and lateral arm cutaneous flaps. Perforators were categorized as direct, terminal, and intransitive, depending upon perforator origin and termination site relative to the source artery. Statistical cluster analysis of perforator distributions was performed to determine the regions in which cutaneous perforators are consistently found. The scapular and radial forearm flaps could be divided into up to three well-perfused segments. The analysis predicted the p

Myringoplasty for the anterior perforation: experience with the Kerr flap.

Surgical closure of the anteriorly located tympanic membrane perforation can present a problem. The lack of anterior support for the graft frequently leads to graft failure if an underlay method is used whereas anterior blunting is a complication of onlay techniques in this situation. The experience with the Kerr flap, an underlay graft fashioned to include a tab of fascia which is placed laterally under the annulus and the anterior meatal skin, is presented. This method gave a 97.5 percent closure rate with no cases of anterior marginal blunting and a mean auditory threshold gain of 8.5 dB (9

The inferior turbinate flap for closure of septal perforations.

In 31 patients a symptomatic septal perforation was repaired using an inferior-turbinate flap originally described by Masing. In approximately 50% of the cases the perforation was closed for two-thirds or more. Postoperative scarring causing nasal obstruction or alar asymmetry occurred in 21% of the cases. Some specific symptoms such as epistaxis, whistling and frontal headache appeared to improve even after partial closure. However, the overall degree of satisfaction of the patients and the degree of closure of the perforation were related. Based on the results of this series the inferior-tur

Free-Flap Lower Extremity Reconstruction: A Cohort Study and Meta-Analysis of Flap Anastomotic Outcomes between Perforator and Nonperforator Flaps.

INTRODUCTION: Free-flap outcomes in lower extremity reconstruction carry the lowest anastomotic success rates compared with other anatomical sites. Despite their advantages over traditional nonperforator flaps, free perforator flaps have only recently become established in this area due to the additional challenges faced. It is therefore crucial to assess the anastomotic outcomes of perforator and nonperforator free flaps. METHODS: We performed a single-center retrospective cohort study and combined this with a meta-analysis of the relevant literature. We evaluated three flap anastomotic outc

A novel reconstruction of 2 distinct defects: concomitant use of a thoracodorsal artery perforator flap and its corresponding muscle flap.

A perforator flap completely spares the underlying muscle, so the muscle tissue can now be used as a second flap for the reconstruction of another defect after the harvest of the overlying perforator flap as long as sufficient nutritive branches to the muscle remain. The authors describe here the creation of 2 independent free flaps from a single donor site, the latissimus dorsi musculocutaneous unit. The 2 flaps, thoracodorsal artery perforator and its corresponding muscle flap, were created and transferred to 2 distinct defects. Using this technique, we could avoid creation of double donor s

Combined method of free lateral leg perforator flap with cervicoplasty for reconstruction of anterior cervical scar contractures: a new flap.

Anterior cervical scar contracture remains a challenging problem for the plastic surgeon. Recent advances in flap techniques have made it possible to transfer various kinds of autogenous tissue. A free lateral leg perforator flap (LLPF) was first introduced and developed to reconstruct this burn deformity, according to the special nature of perforator anatomy. The authors used this flap to resurface post-burn anterior cervical contracture in a 31-year-old female patient with second-to-third-degree flame burns. The size of the excised scar was 25 x 5.5 cm; the size of the flap was 22 x 8 cm, wi

Role of perforator flaps in leg and foot reconstruction.

INTRODUCTION: Lower extremity wounds have always been a challenge for the reconstructive surgeons. Free perforator flaps are considered to be the best option for this problem but require the complexity of microsurgery. So, pedicled perforator flaps have emerged as an alternative option. PATIENTS AND METHODS: Prospective study was conducted in 40 patients with traumatic soft tissue defects in the leg and foot. The free flaps included the anterolateral thigh flap (ALT) and medial sural artery perforator flap (MSAP). In pedicled perforator flaps group, 10 cases were designed as propeller flaps w

Propeller perforator flaps in forearm and hand reconstruction.

After a long history in flaps' surgery, the perforator flaps became the most used flaps nowadays. From the beginning, their use as free flaps diminished substantially the donor site morbidity. In the attempt to not only diminish the donor site morbidity, but also to achieve more similar reconstructions, a new concept appeared 20 years ago: local perforator flaps. The local perforator flaps offer as main advantages the absence of microsurgical sutures ("microsurgical non-microvascular flaps"), same surgical field, the sparing of muscles and main vascular pedicles, and shorter hospitalization ti
