Outcome of implant therapy in patients with previous tooth loss due to periodontitis.

BACKGROUND: It is frequently debated whether implant treatment in individuals with previous tooth loss due to periodontitis is characterized by an increased incidence of implant loss and peri-implantitis. OBJECTIVE: The objective of the present systematic review was to assess whether individuals with previous tooth loss due to periodontitis have an increased risk of loss of suprastructures, loss of implants, peri-implantitis, and peri-implant marginal bone loss as compared with individuals with previous tooth loss due to reasons other than periodontitis. SEARCH STRATEGY: Studies considered f

The causes of early implant bone loss: myth or science?

The success of dental implants is highly dependent on integration between the implant and intraoral hard/soft tissue. Initial breakdown of the implant-tissue interface generally begins at the crestal region in successfully osseointegrated endosteal implants, regardless of surgical approaches (submerged or nonsubmerged). Early crestal bone loss is often observed after the first year of function, followed by minimal bone loss (< or =0.2 mm) annually thereafter. Six plausible etiologic factors are hypothesized, including surgical trauma, occlusal overload, peri-implantitis, microgap, biologic wid

Clinical and microbiologic effects of chemical versus mechanical cleansing in professional supportive implant therapy.

The aim of the present study was to compare the cleansing properties of mechanical supportive care for dental implants with the use of an etching gel. Sixteen patients underwent a 5-month clinical trial with monthly recalls. These patients, wearing maxillary complete dentures and mandibular overdentures supported by a bar device on 4 implants, were treated in a split-mouth study design. Test and control therapy were randomly assigned to left and right sides of the mandible. At the test side, 35% phosphoric etching gel (pH 1) was applied in the peri-implant sulcus. After 1 minute, the sulcus wa

Should prophylactic antibiotics be used for patients having removal of erupted teeth?

Generally, antibiotics should not be required before the removal of erupted carious or periodontally involved teeth unless a significant risk of postoperative infection is present. The decision to use prophylactic antibiotics in noninfected cases should also be based on whether patients have any significant medical risk factors that could adversely affect their humoral and cellular defense mechanisms, and whether any systemic risks are associated with the bacteremia that accompanies tooth extraction. This article discusses the various indications for using prophylactic antibiotics in patients

Antibiotic-eluting medical devices for various applications.

Infection is defined as a homeostatic imbalance between the host tissue and the presence of microorganisms. It is associated with a large variety of wound occurrences ranging from traumatic skin tears and burns to chronic ulcers and complications following surgery and device implantations. If the wound setting manages to overcome the microorganism invasion by a sufficient immune response then the wound should heal. If not, the formation of an infection can seriously limit the wound healing process. Evidence of increasing bacterial resistance is on the rise, and complications associated with in

Osseous regeneration in immediate postextraction implant placement: a literature review and clinical evaluation.

Immediate implant placement following tooth extraction in appropriately selected cases has been considered to be the optimal time for the procedure for the following reasons: The natural healing processes are mobilized to the maximum, no bone resorption has taken place as yet, drilling is avoided, the number of surgical stages are reduced, design and construction of the prosthesis is simplified, and the positive psychologic effect on the patient is immediate. The learning objective of this article is to discuss the results of immediate implant placement and guided osseous regeneration with mem

Maintaining Osseointegration: Understanding, preventing, and treating Risk Factors for Peri-implant Diseases.

The dental implant is a predictable way of restoring fully and partially edentulous patients and has shown high success rate for managing a broad range of clinical conditions. Improving implant surfaces has revolutionized the osseointegration of implants and changed the focus of implant research from osseointegration more toward the risk factors associated with the failure of implants in the long-term.

Influence of Connector Design on Displacement and Micromotion in Tooth-Implant Fixed Partial Dentures Using Different Lengths and Diameters: A Three-Dimensional Finite Element Study.

The literature presents insufficient data evaluating the displacement and micromotion effects resulting from the combined use of tooth-implant connections in fixed partial dentures. Analyzing the biomechanical behavior of tooth-implant fixed partial denture (FPD) prothesis is vital for achieving an optimum design and successful clinical implementation. The objective of this study was to determine the relative significance of connector design on the displacement and micromotion of tooth-implant-supported fixed dental prostheses under occlusal vertical loading. Periodontal infection in adult-onset Still's disease patient: clinical and haematological considerations.

In this case report, the authors described the first case of a patient with adult-onset Still's disease (AOSD) who presents advanced periodontal infection. AOSD is a rare systemic inflammatory disorder of unknown aetiology, characterised by spiking fever, usually exceeding 39°C, an evanescent salmon pink rash, arthritis and multiorgan involvement. Periodontal infection is a pathogen-induced oral inflammatory disease affecting the supporting tissues of teeth and is currently considered as a risk factor for cardiovascular disease. Several cytokines capable of inducing systemic effects are produc

Tissue integration of non-submerged implants. 1-year results of a prospective study with 100 ITI hollow-cylinder and hollow-screw implants.

It has been postulated that the wound healing in a closed submerged location is one of the prerequisites for osseointegration of dental implants. The purpose of the present study was to evaluate the tissue integration of intentionally non-submerged titanium implants inserted by a one-stage surgical procedure. 100 ITI implants were consecutively placed in 70 partially edentulous patients. After a healing period free of masticatory loading for at least 3 months, the implants were examined. The clinical status showed for all implants neither detectable mobility nor signs of a peri-implant infecti

Clinical flexibility of the metal free approach.

The present possibility of defining a plan for reconstructive treatment taking into consideration metal-free materials and related clinical procedures offers wider therapeutic options for the clinician and the dental technician, establishing function, esthetics and longevity in prosthetic rehabilitation. The metal-free approach may be regarded as a new philosophy of treatment that extends beyond the solution of esthetic and cosmetic shortcomings in the anterior regions and involves a greater variety of complex clinical cases such as periodontal breakdown conditions and edentulous sites requiri

Assessment of prosthetic restorations on bone-lock implants in patients after oral tumor resection.

The feasibility of implant treatment in patients after oral ablative tumor surgery has not yet been investigated with consideration of the requisite high periodontal standards. A report on this topic has to deal not only with implant survival but also with implant health, bone response, soft tissue health, failure pattern, time of failure, and ease of restoration. For the assessment of an implant system, an overview must be accomplished that takes into account the different restorations used and their interaction with the implant system that was used. This study presents the Bone-Lock implant
