Comparative experimental study of myocardial protection with crystalloid solutions for heart transplantation.

BACKGROUND: There is a growing need to improve myocardial protection, which will lead to better performance of cardiac operations and reduce morbidity and mortality. Therefore, the objective of this study was to compare the efficacy of myocardial protection solution using both intracellular and extracellular crystalloid type regarding the performance of the electrical conduction system, left ventricular contractility and edema, after being subjected to ischemic arrest and reperfusion. METHODS: Hearts isolated from male Wistar (n=32) rats were prepared using Langendorff method and randomly div

Cardiopulmonary bypass and activation of antithrombotic plasma protein C.

OBJECTIVE: We hypothesized that antithrombotic plasma-activated protein C plays a defensive antithrombotic role during coronary ischemia and postischemic reperfusion. METHODS AND RESULTS: We evaluated protein C activation during cardiopulmonary bypass and coronary reperfusion in 20 patients undergoing coronary bypass surgery. During cardiopulmonary bypass and during the 10 minutes after aortic unclamping, the plasma levels of protein C (mean +/- standard error of the mean) decreased from 123% +/- 7% to 74% +/- 5% of normal mean. In contrast, the levels of activated protein C in plasma increas

[Influence of left ventricular wall thickening on myocardial injury in aortic valve replacement].

We retrospectively examined the influence of left ventricular wall thickening on myocardial injury in aortic valve replacement. Thirty-five patients who underwent aortic valve replacement in our hospital between January 1995 and July 1997 were studied. We divided these patients into 3 groups: group N (left ventricular thickness (LVT) < or = 10 mm), group H (10 < LVT < or = 15 mm), and group S (LVT > 15 mm). All patients of group S had aortic stenosis. CK and CK-MB were significantly higher and the cardiac index was significantly lower in group S than in the other groups. Intra-aortic balloon p

Adequate preservation of right ventricular function after coronary sinus cardioplegia. A clinical study.

Whether retrograde coronary sinus cardioplegia adequately preserves right ventricular (RV) function is still a point of concern. Using technetium Tc 99m-labeled red blood cells, we assessed global and segmental RV function by first-pass and gated blood-pool radionuclide angiocardiography before and within 24 hours after aortic valve replacement in 14 consecutive patients (age, 58 +/- 5 years; mean +/- SEM). Coronary sinus cardioplegia was given in a multidose fashion at a flow rate of 50-70 ml/min through a balloon-tipped catheter, with the inflated balloon kept seated around the intra-atrial

[Glucose-insulin-potassium (GIK) solution: cardioprotective effects of insulin].

In the Anesthesiology Department of the Instituto Nacional de Cardiología The medical staff uses the glucose insulin potassium i.v. for myocardial protection. The energy is dereived from lipidic beta oxidation, glucose breakdown and amino acid catabolism in mitochondria. In myocardial ischemia, from de aortic cross clamping, the metabolic myocardial substrates diminished as well as the energy produced by adenosine (ATP). During myocardial ischemia, an increase in long chain lipids emhances mitochondrial permeability, promotes depletion of citochrome C and lost of the capability of transmembran

High-dose glucose-insulin-potassium treatment reduces myocardial apoptosis in patients with acute myocardial infarction.

BACKGROUND: Several clinical trials have suggested that a metabolic cocktail of glucose-insulin-potassium (GIK) decreases mortality rates in patients with acute myocardial infarction (AMI). It has also been reported that Fas-mediated apoptosis plays an important role in ischaemic/reperfusion injury in the rat model. This study was designed to evaluate the interaction of ischaemic/reperfusion and reperfusion therapy coadministered with high-dose GIK treatment on soluble Fas/APO-1 (sFas) and Fas ligand (sFasL) plasma concentration in patients with AMI. del Nido versus St. Thomas' blood cardioplegia in the young (DESTINY) trial: protocol for a multicentre randomised controlled trial in children undergoing cardiac surgery.

INTRODUCTION: Myocardial protection against ischaemia-reperfusion injury is a key determinant of heart function and outcome following cardiac surgery in children. However, myocardial injury still occurs routinely following aortic cross-clamping, as demonstrated by the ubiquitous rise in circulating troponin. del Nido cardioplegia was designed to protect the immature myocardium and is widely used in the USA but has not previously been available in the UK, where St. Thomas' blood cardioplegia is most common. The del Nido versus St. Thomas' blood cardioplegia in the young (DESTINY) trial will eva

A cost-effective proof-of-concept ex vivo perfusion model for myocardial ischemia-reperfusion injury in wistar rats: enhancing accessibility in resource-constrained settings.

INTRODUCTION: Myocardial ischemia-reperfusion injury (IRI) continues to impede the advancement of effective cardioprotective therapies. Experimental limitations, particularly the lack of accessible and cost-effective ex vivo platforms, restrict mechanistic and translational research. This study aimed to develop a simplified, resource-conscious proof-of-concept ex vivo rat heart perfusion model capable of inducing key features of myocardial IRI, rather than establishing a fully validated replacement for conventional Langendorff systems. METHODS: Twelve adult male Wistar rats were randomly assi

Ex vivo endothelial denudation of porcine aortic valves.

BACKGROUND: Aortic valve stenosis (AS) is a progressive and life-threatening condition characterized by narrowing of the aortic valve, leading to impaired outflow from the left ventricle and increased cardiovascular risk. With rising disease prevalence in aging populations and the absence of effective pharmacological treatments, reliable animal models are crucial for studying disease mechanisms and evaluating new therapeutic approaches. The response-to-tissue-injury is considered a key driver of valvular degeneration, yet current models remain limited. This study aimed to assess whether cryoba

Comparison of gross pathology inspection and 9.4 T magnetic resonance imaging in the evaluation of radiofrequency ablation lesions in the left ventricle of the swine heart.

Aims: Gross pathology inspection (patho) is the gold standard for the morphological evaluation of focal myocardial pathology. Examination with 9.4 T magnetic resonance imaging (MRI) is a new method for very accurate display of myocardial pathology. The aim of this study was to demonstrate that lesions can be measured on high-resolution MRI images with the same accuracy as on pathological sections and compare these two methods for the evaluation of radiofrequency (RF) ablation lesion dimensions in swine heart tissue during animal experiment. Methods: Ten pigs underwent radiofrequency ablations

Preconditioning enhances myocardial protection in patients undergoing open heart surgery.

To date, ischemic preconditioning is regarded as the most powerful form of endogenous myocardial protection. For the purpose of surgical myocardial protection, a few clinical studies have investigated the effects of ischemic preconditioning in conjunction with hypothermia or blood cardioplegia during open heart surgery, but the results were controversial. We now tested the hypothesis that preconditioning improves myocardial protection in patients undergoing cold crystalloid cardioplegic arrest. 36 patients needing mitral prosthetic valve replacement for rheumatic heart disease were studied. Assessment of myocardial protection during global ischemia with myocardial gas tension monitoring.

Intramyocardial gas tension monitoring with mass spectrometry allows for the continuous assessment of myocardial metabolic activity during prolonged global ischemia. With aortic cross-clamping there is a rapid decrease in intramyocardial oxygen tension (PmO2) and a steady increase in carbon dioxide tension (PmCO2). In laboratory studies the extent to which myocardial metabolic activity is reduced has correlated with the degree of myocardial protection being afforded. In the present study the metabolic consequences of single-dose versus multiple-dose infusions of a hyperkalemic cardioplegic sol

Antegrade Versus Continuous Retrograde del Nido Cardioplegia in the David I Operation.

BACKGROUND: The efficacy of continuous retrograde del Nido cardioplegia for myocardial protection is still controversial. We hypothesised that antegrade and retrograde cardioplegia offer equivalent safety for myocardial protection in the David I procedure. METHODS: We retrospectively reviewed 33 patients undergoing the David I operation with antegrade or retrograde del Nido solution from June 2014 to January 2016. The outcomes were compared. The follow-up was 1 month to 15 months. RESULTS: There was no hospital mortality or reoperation in both groups. Cardiopulmonary bypass, and aortic clamp

Trimetazidine as cardioplegia addictive without pre-treatment does not improve myocardial protection: study in a swine working heart model.

OBJECTIVE: The aim of this study is to verify in an isolated working heart swine model if the acute administration of trimetazidine to cardioplegia, without pre-treatment improves heart performance. METHODS: Eighteen pairs of swine were used in this working heart model, divided into three groups (n = 6) that underwent regional and global ischemia. Each group was selected to a different treatment: St. Thomas cardioplegia (ST), St. Thomas enriched with trimetazidine (TMZ) and control group (Co). Data was collected during reperfusion period at 30, 60 and 90 minutes. Hemodynamic parameters such a

Improved myocardial protection using continuous coronary perfusion with normothermic blood and beta-blockade with esmolol.

Continuous coronary perfusion with warm beta-blocker-enriched blood has been suggested as an alternative to cardioplegic arrest for myocardial protection during coronary artery surgery. The purpose of the present work was 1.) to experimentally investigate this technique using an animal model, and 2.) to clinically apply this alternative myocardial protection technique and compare it to standard crystalloid cardioplegia in a controlled study. We placed 6 dogs on CPB and 6 dogs on a biventricular assist device and created "beta-blocker-induced cardiac surgical conditions" by suppressing myocardi

Brain death-related energetic failure of the donor heart becomes apparent only during storage and reperfusion: an ex vivo phosphorus-31 magnetic resonance spectroscopy study on the feline heart.

BACKGROUND AND OBJECTIVE: Recently, we have shown, by using localized in vivo phosphorus-31 magnetic resonance spectroscopy (31P MRS) of the anterior left ventricular wall, that brain death (BD) is not associated with reduced myocardial energy status. In this study, we applied ex vivo 31P MRS of the entire heart to study the effects of BD on the energy status of the feline donor heart following explantation. METHODS: We used cats (6 BD and 6 controls [C]) in a 26-hour protocol. After 2 hours of preparation, we induced BD by filling an intracranial balloon at t = 0 hour. At t = 6 hours, the he

The importance of acid-base management for cardiac and cerebral preservation during open heart operations.

The basic physiologic characteristics of acid-base equilibria during hypothermia were briefly reviewed. By graphic analysis, four possible clinical strategies for managing the acid-base status of the patient undergoing H-CPB were documented. The effect of hemodilution on buffer capacity was charted in a manner applicable to common current operative procedures. During hypothermia for cardiac operations as presently conducted, the perfusionist is in control of the temperature of the body and the perfusion preservation of the body and brain; the surgeon must assume responsibility for preservation

The protective effects of L-carnitine on myocardial ischaemia-reperfusion injury in patients with rheumatic valvular heart disease undergoing CPB surgery are associated with the suppression of NF-κB pathway and the activation of Nrf2 pathway.

Myocardial ischaemia-reperfusion injury (MIRI) is a main pathophysiologic change following CPB surgery. L-carnitine, a natural amino acid, is able to transport fatty acids for generating energy and has a protective effect on MIRI. We aim to investigate the protective effect of L-carnitine on MIRI in patients with rheumatic valvular heart disease (RVHD) performed CPB surgical operation and the underlying mechanism. In this study, patients were randomized to three groups. L-carnitine was added to the crystalloid cardioplegic solution for experimental group 1 (6 g/L) and experimental group 2 (12
