A Contemporary Approach to Diagnosis and Treatment of Right Heart Thrombus Associated With Pulmonary Embolism.

Right heart thrombus (RHT) in the setting of pulmonary embolism (PE) is a uniquely high-risk event. RHT management has not been rigorously studied in clinical trials and is not part of risk stratification schemas for PE. However, it is a major risk factor for clinical deterioration and mortality. There are no guidelines for RHT management, and most practice is based on case series and anecdote. Historically, RHT has been managed with surgical embolectomy or systemic thrombolysis; however, there is now increasing experience with catheter-based mechanical thrombectomy. Furthermore, a subset of p

Double-staged thromboembolectomy in acute visceral ischemia with atypical origin of right hepatic artery, complicated by secondary intestinal infiltrate.

INTRODUCTION: Acute mesenteric ischemia is a rare but serious vascular complication often accompanied by intestinal damage. Treatment typically involves surgical and endovascular procedures. Proper diagnostic and interventional techniques are critical for preventing complications, resulting in short bowel syndrome, sepsis or need for repeated surgeries. Based on the case report, our publication provides an overview of managing acute mesenteric ischemia with intestinal involvement, highlighting both conventional and modern treatment approaches. CASE REPORT: A 57-year-old woman with thyrotoxico

A Look Back on the History of Cerebral Revascularization for Acute Ischemic Stroke: A Neurosurgeon's Perspective.

This review looks back on our experience with acute middle cerebral artery embolectomies in the 1990s, frowned upon by stroke experts at the time, and no match for the newly introduced and proven treatment of acute ischemic stroke with intravenous recombinant tissue plasminogen activator (alteplase). The past several decades have seen dramatic developments in acute cerebral revascularization, the major paradigm shift being in the form of endovascular thrombectomy. Mechanical thrombectomy has moved from the operating room, where we performed it, to the interventional angiography suite armed wit

Popliteal artery embolism of bullet after abdominal gunshot wound.

Bullet embolism to the peripheral arterial system is a rare phenomenon which frequently results in misdiagnosis due to lack of early symptoms. Embolisms can go to either arterial or venous systems with common sites of injury including the left ventricle, pulmonary vein, thoracic and abdominal aorta and peripheral arteries. Herein we present a case of a 19 year old patient with multiple gunshot wounds to the torso with a bullet embolism to the left popliteal artery necessitating embolectomy. This subsequently led to diagnosis and repair of an abdominal aortic psuedoaneurysm not clearly evident

[Local lysis therapy in acute arterial thrombosis].

Acute thrombotic arterial occlusion is a complication of chronic vascular arterial disease. The arteriogram usually reveals a sudden occlusion and the collateral vessels. Treatment with low-dose fibrinolytic therapy is widely accepted. A combination of low-dose fibrinolysis, aspiration embolectomy and angioplasty yields the best primary success rate. In cases of sudden popliteal occlusion and dilating arteriopathy ultrasound sonography should be used to exclude the presence aneurysm which is a contraindication for fibrinolysis. There is a high risk of peripheral embolisation, in the presence o

May-Thurner syndrome disguised as pulmonary embolism: A case report.

An 80-year-old female patient presented with intermediate-risk pulmonary embolism with underlying etiology called May-Thurner syndrome-a contralateral right common iliac artery compressing the left common iliac vein against the lumbar vertebra and causing a cascade of endothelial changes, stenosis, and thrombi formation. It is common among young women with deep vein thrombosis or chronic venous insufficiency symptoms over their left leg. Diagnosis is confirmed by intravascular ultrasound examination. This patient was successfully treated through a multidisciplinary team approach, which include

Anticoagulant Therapy in Patients Undergoing Acute Pulmonary Embolism Interventions.

Catheter-based interventions and surgical embolectomy represent alternatives to systemic fibrinolysis for patients with high-risk pulmonary embolism (PE) or those with intermediate-high-risk PE who deteriorate hemodynamically. They are indicated when systemic fibrinolysis is contraindicated or ineffective, or if obstructive shock is imminent. Extracorporeal membrane oxygenation can be added to reperfusion therapies or used alone for severe right ventricular dysfunction and cardiogenic shock. These advanced therapies complement but do not replace anticoagulation, which remains the cornerstone i

Acute limb ischemia and aortic mural thrombosis as primary manifestations of severe acute respiratory syndrome coronavirus 2.

A variable clinical presentation is emerging as a hallmark of the novel SARS-CoV-2 (severe acute respiratory syndrome coronavirus 2). In addition to hypoxic respiratory failure, multiorgan dysfunction, and septic shock, significant thromboembolic complications posited to result from diffuse coagulopathy have been associated with this viral infection. We report on a unique primary manifestation of SARS-CoV-2 infection presenting as acute limb ischemia and aortic mural thrombosis without clinical evidence of pulmonary disease. Despite our best attempts at limb salvage with therapeutic anticoagul

Incidental finding of a left atrial thrombus during surgical management of a massive pulmonary embolism.

A 58-year-old male presented with a hemodynamically significant pulmonary embolism. Computed tomography angiogram revealed a saddle embolus in the main pulmonary artery with extensive clot burden affecting all lobes and right heart strain. Transthoracic echocardiogram displayed a dilated right ventricle with reduced systolic function. The patient was scheduled for pulmonary embolectomy. The intraoperative transesophageal echocardiogram (TEE) demonstrated a mobile left atrial thrombus that was missed on previous imaging. After removal of the thrombi, TEE showed a patent foramen ovale (PFO). The

Perioperative pulmonary embolism.

Pulmonary thromboembolism (PTE) is a perioperative complication that requires prompt diagnosis and treatment to minimize mortality. Detection of deep vein thrombosis (DVT) suggests the presence of PTE. The clinical presentation of PTE is mainly hemodynamic and gas exchange abnormalities. Diagnostic tools include ventilation/perfusion scan, pulmonary angiography, spiral CT, and echocardiography. Therapeutic options include hemodynamic support with inotropics, anticoagulation, systemic thrombolysis, surgical embolectomy and an inferior vena cava filter. DVT prophylaxis should be considered in al

[Pulmonary embolism associated with right auricular thrombus. Apropos of 2 cases].

The authors report two cases of serious pulmonary embolism, derived from venous thrombus (inferior vena cava in one case, superficial femoral vein in the other), associated with mobile thrombus of the right auricle, with a very rapidly favourable course after intravenous fibrinolysis. This treatment was extremely effective, allowing rapid haemodynamic improvement and resolution of the thrombus in less than 24 hours. No major haemorrhagic complications or recurrent emboli were observed. Fibrinolysis, in serious pulmonary embolism associated with thrombus of the right auricle, therefore appears

[Blunt trauma with combined femoral arterial and venous occlusion].

Blunt trauma to common femoral artery is rare, and it is frequently unrecognized. A 61-year-old man who fell while carrying a log and struck on his right groin, presented right leg edema and intermittent claudication one month later. Both right femoral arterial and venous occlusion was suspected by Doppler flowmeter, confirmed by venography and arteriography which showed concomitant external iliac arterial occlusion. He underwent prosthetic bypass grafting from common iliac artery to common femoral artery and embolectomy of femoral vein. Circulation to the right leg was restored, but postphleb

Hybrid procedures for acute limb ischemia.

The most efficient treatment for acute arterial embolism is operative embolectomy using Fogarty's balloon catheter, especially if a single large artery is involved. Unfortunately, although the early surgical success of arterial thromboembolectomy often seems acceptable, the early clinical outcome still remains unsatisfactory. This may be related to the incomplete restoration of perfusion (i.e. residual thrombus in distal vessels not reached by the balloon catheter thromboembolectomy), propagation of residual thrombi or presence of underlying steno-occlusive lesions. In such a situation a metic

Acute Upper Extremity Arterial Occlusion Diagnosed on POCUS in the Emergency Department.

Upper extremity acute limb ischemia (ALI) is a limb-threatening and potentially lethal pathology that is most commonly caused by vascular embolization. Outcomes of limb ischemia are time-sensitive due to the correlation between a longer time from symptom onset to intervention with a vastly higher risk of amputation. In this report, point of care ultrasound (POCUS) was utilized to rapidly diagnose a patient with a proximal right brachial artery embolic occlusion, prompting expedited surgical consultation and successful embolectomy. POCUS can provide a focused vascular examination of the limbs t

[Thromboembolic complications 6 months after plate osteosynthesis of a mid-clavicular fracture].

A 32-year-old patient suffered from progredient intermittent claudication of the left arm 6 months after plate fixation of a midshaft clavicular fracture (OTA classification type A). The radiological analysis revealed a subacute stenosis of the left subclavian artery, caused by a granuloma at the tip of a protruding screw, which triggered recurrent emboli and symptoms. Reperfusion was achieved by catheter embolectomy and complete removal of the plate. The case shows that, when performing plate osteosynthesis of clavicular fractures, special consideration has to be given to the length and protr

Pulmonary hypertension due to tumor emboli: a report of three autopsy cases with morphological correlations to radiological findings.

Three cases of pulmonary hypertension caused by tumor emboli to the lungs are described. Two of the three cases had a clinical diagnosis of pulmonary thromboembolism until surgical embolectomy, and the other had a diagnosis of primary pulmonary hypertension. Autopsy disclosed chondrosarcoma, choriocarcinoma and gastric cancer as the primary tumors, respectively. Pulmonary vascular obstruction due to tumor embolism leading to pulmonary hypertension is a previously rare clinical entity, and obstructed pulmonary vessels are believed to tend to be small vessels. We compared the autopsy and radiolo

Pulmonary tumor embolism secondary to soft tissue and bone sarcomas: a case report and literature review.

BACKGROUND: Tumor embolisms (TE) are an underappreciated source of pulmonary embolisms in sarcoma. Most evidence in the literature is limited to case reports and none have described the presence of TE secondary to myxofibrosarcoma. We report the first case of myxofibrosarcoma TE and perform a review of the literature for TE secondary to bone and soft tissue sarcomas (STS). CASE PRESENTATION: A 36-year-old female presented with debilitating pain of the right upper extremity secondary to a recurrent soft tissue sarcoma. She had distant metastasis to the lung.
