Surveillance of adverse events following immunization: 10 years' experience in Oman.

A descriptive record-based review of adverse events following immunization (AEFI) was carried out in Oman using the national database for the period 1996-2005. A total of 790 adverse event reports were received with an annual rate during the review period of 33.7 per 100 000 population or 10.8 per 100 000 doses administered. There were no reported deaths. The most frequently reported AEFI were BCG adenitis (69.7 per 100 000 doses) and local reactions (3.6 per 100 000 doses respectively). The statistically significant higher rates among males, in children aged > 2 years and in some sparsely pop

Establishing a transplant coordinator-led living kidney donor follow-up clinic.

BACKGROUND: The long-term risks of renal failure and hypertension are statistically low for living kidney donors as a group, but can have serious consequences for the individual. OBJECTIVES: To describe the experience with a transplant coordinator-led living donor follow-up clinic. METHOD: Living kidney donors are reviewed on an annual basis by a designated coordinator (registered nurse). A 24-hour urine collection estimates renal function. Blood pressure and blood chemistry are measured and urinalysis performed. Current health status and wound discomfort are assessed. Any medical problems i

HIV-related brain impairment from palliative care to rehabilitation.

Brain impairment is a distressing manifestation of human immunodeficiency virus (HIV) disease characterized by progressive cognitive impairment leading eventually to dementia and death. Patients with advanced brain impairment are clinically difficult to manage and usually require residential care. In 1997, a brain impairment unit opened at the Mildmay Hospital UK in London to meet the needs of this patient group. It began as a nurse-led unit, has adopted an interdisciplinary approach to care and aims to maximize the quality of life until death. In a study of patients admitted during its first

BACKGROUND: Few researchers have examined the organizational features of high-performing and low-performing hospitals for COVID-19 mortality during the pandemic, and how differences in hospital performance contributed to mortality disparities among socially vulnerable patients hospitalized with COVID-19. OBJECTIVES: Our objectives were (a) to identify high- and low-performing hospitals on COVID-19 inpatient mortality and describe their distinguishing organizational characteristics, including nursing resources, and (b) to assess whether patients admitted to high-performing hospitals differed b

Urinary incontinence in men: current status and future directions.

BACKGROUND: Despite similar rates of voiding dysfunction in older men and women, most funded research has focused on women. Strategic treatment plans for managing urinary incontinence and other lower urinary tract symptoms in men are limited by sparse or absent direct clinical evidence with most interventions supported by data extrapolated from studies in women. OBJECTIVES: To explore what is known about the epidemiology and etiology of incontinence in men, highlight some of the gaps in the current knowledge, address limitations in existing research, and consider future directions in men's co

Caring for the community: the role of partnerships.

While many members of the public are deeply interested in and supportive of the three traditional missions of academic medicine--education, research, and clinical care, they also want to know what academic health centers (AHCs) are doing to improve the overall health of their communities. Much is already being done toward this goal, but improving communities' health in a measurable way requires a far broader agenda. AHCs must bring together the approaches of medicine and public health, and need to partner with many other players. This agenda must proceed despite all the other challenges that A

The Multicriteria Decision Analysis for Extended Reality (MCDA-XR) Governance Framework for Health Care Adoption: Mixed Methods Development Study.

BACKGROUND: Health services increasingly face decisions about how to integrate immersive technologies into routine practice. International guidance highlights the need for structured governance in digital health, yet extended reality (XR) initiatives are often launched through isolated pilots without a clear assessment of organizational readiness or implementation risk. Although factors influencing XR adoption are well documented, health care organizations and system-level decision-makers still lack practical, governance-oriented tools to translate these determinants into structured strategic
