Nalbuphine as analgesic in preschool children undergoing ophthalmic surgery and the occurrence of emergence delirium.

BACKGROUND/AIMS: Perioperative pain in children is often inadequately treated, and emergence agitation is common. The purpose of this analysis was to determine whether nalbuphine is suitable for perioperative eye pain and to analyse if it influences the occurrence of emergence delirium/agitation (EDA) in children undergoing ophthalmic surgery in general anaesthesia. METHODS: Retrospective cohort analysis of 50 children in preschool age undergoing general anaesthesia for ophthalmic surgery receiving nalbuphine as a postoperative analgesic in a German university hospital from June 2020 to Febru

Prompt Recognition and Management of Postoperative Intracranial Hypotension-Associated Venous Congestion: A Case Report.

BACKGROUND: Postoperative intracranial hypotension-associated venous congestion (PIHV) is an uncommon cause of clinical deterioration after a neurosurgical procedure that is often unrecognized until late in its course. Functional outcomes range from remarkable neurological recovery to death. Little is understood about the reason for deterioration in certain patients compared with improvement in others. A 68-year-old man with a progressive cervical myelopathy underwent an uncomplicated cervical decompression and alignment restoration at our hospital and suffered violent generalized tonic-clonic

Opioid consumption after levobupivacaine scalp nerve block for craniosynostosis surgery.

BACKGROUND AND OBJECTIVES: Craniosynostosis surgery is considered a very painful procedure due to extended scalp and periosteal detachment, and is associated with prolonged postoperative consumption of opioids and their side effects. In this observational descriptive case series study, we investigated perioperative opioid consumption in children undergoing craniosynostosis repair under general anesthesia when scalp nerve block with levobupivacaine was involved. METHODS: After standard anesthesia induction, scalp nerve block with levobupivacaine 2 mg/kg plus epinephrine 1:800,000 was performed

Caudal anaesthesia with 0.375% ropivacaine or 0.375% bupivacaine in paediatric patients.

We have determined the effectiveness and degree of motor block produced by the new local anaesthetic, ropivacaine, when used for caudal anaesthesia in children. We studied 60 children, aged 3-6 yr, ASA I, allocated randomly in a double-blind manner, to receive one of two local anaesthetics: 0.375% ropivacaine 1.0 ml kg-1 or 0.375% bupivacaine 1.0 ml kg-1. Patients were anaesthetized with continuous infusion of propofol 200 micrograms kg-1 min-1. The lungs were ventilated with 50% nitrous oxide in oxygen. Heart rate and arterial pressure were measured every 5 min after administration of local a

Effect of prophylactic epidural morphine or bupivacaine on postoperative pain after upper abdominal surgery.

Morphine 2 mg and 4 mg or bupivacaine 50 mg (another 50 mg 4 h later) was administered into the epidural space prior to general anaesthesia of 40 patients undergoing upper abdominal surgery. During anaesthesia, additional analgesics were not given. In the recovery room (4 h) the pain score (0-10) was lowest in the bupivacaine group (mean 2.4) followed by the 4 mg-morphine group (4.1), 2 mg-morphine group (5.3) and control group (5.7). Half of the patients of the bupivacaine group and those of the 4 mg-morphine group needed no analgesics in the recovery room. Later, in the ward (until next morn

A comparison of patients' and health care professionals' preferences for symptoms during immediate postoperative recovery and the management of postoperative nausea and vomiting.

In this study we sought to examine the differences in patients' and health care professionals' preferences for symptoms during immediate postoperative recovery and the management of postoperative nausea and vomiting (PONV). The key differences between symptoms during immediate postoperative recovery (PONV, sedation, and pain) and management of PONV (prophylaxis, efficacy of antiemetic, and extra cost) were used to develop 14 scenarios in a questionnaire. Fifty-two health care professionals (anesthesiologists and recovery room nurses) and 200 women undergoing elective gynecological surgery were

Hyperoxia during recovery from consecutive anaerobic exercises in the sickle cell trait.

The sickle cell trait (HbAS) does not affect anaerobic exercise performance. However, lower blood lactate concentrations ([La-]) are consistently found during repeated anaerobic exercise in HbAS, and could be related to type of recovery. To study this, on three different occasions 17 HbAS and 17 matched control athletes (HbAA) underwent a series of three maximal cycle exercise tests of approximately 2-min duration, separated by 10-min recovery periods of rest, breathing either room air (P) or 100% oxygen (H), or of light pedaling (A). In all tests, work performed, heart rate, blood hematocrit,

Finger pulse volume in patients with panic attacks.

To study the peripheral blood flow in patients with panic attacks during "Stress" conditions, nine patients with history of panic attacks were subjected to mental stress of arithmetic calculation and room-air voluntary hyperventilation. Finger Pulse Volume (FPV) was significantly altered during the period following mental activity in untreated patients when compared to controls and patients under treatment. There was also an increase in FPV in untreated patients at rest and during recovery after mental stress when compared to the treated patients.

Enhanced recovery program in ambulatory pediatric urology: A quality improvement initiative.

BACKGROUND: Enhanced Recovery After Surgery (ERAS) was established in 2001 for adult patients undergoing complex procedures. ERAS in adult ambulatory surgery later followed with similar positive outcomes. For the pediatric population, ERAS implementation has shown promising results in complex surgeries such as bladder reconstruction. Its application in pediatric ambulatory surgery has only recently been reported. We hereby report a Quality Improvement initiative in implementing an Enhanced Recovery Protocol (ERP) for pediatric urology in an ambulatory surgery center. METHODS: A project was la

Delayed-onset hypovolemic shock after the Nuss procedure for pectus excavatum.

The Nuss procedure, which is a minimally invasive approach for treating pectus excavatum, has better functional and cosmetic outcomes than other invasive procedures. Cardiac perforation is the most serious complication and several methods for the prevention of intraoperative events has been developed. Although most cardiac injuries are detected in the operating room, in the case described herein the patient experienced sudden hypovolemic shock during the postoperative recovery period. This indicates that special caution is mandatory even after successful execution of the Nuss procedure.

[Effects of naloxone on postoperative analgesia].

Two comparable groups of ten patients were studied. After nitrous oxide-oxygen fentanyl-pancuronium anesthesia, half the patients were reversed with a titrated dose of naloxone. Even in titrated doses naloxone rapidly abolished residual post-operative fentanyl analgesia in 80 p. 100 of the patients. In the control group none of the patients complained of pain for an average of six to eight hours. Blood gases in the recovery room were practically the same in reversed and non-reversed patients and were satisfactory.

Photosynthesis of Au/TiO2 nanoparticles for photocatalytic gold recovery from industrial gold-cyanide plating wastewater.

A series of Aux/TiO2 nanoparticles (NPs) with different gold loadings (x = 0.1-1.0 wt%) was synthesized by the photodeposition and then employed as photocatalysts to recover precious component from the industrial gold-cyanide plating wastewater. Effects of Au loading, catalyst dosage and types of hole scavenger on the photocatalytic gold recovery were investigated under ultraviolet-visible (UV-Vis) light irradiation at room temperature. It was found that different Au loadings tuned the light absorption capacity of the synthesized photocatalysts and enhanced the photocatalytic activity in compa

An efficient method for the application of PHA-poor solvents to extract polyhydroxybutyrate from Cupriavidus necator.

There are many published studies presenting ethanol and acetone as PHAs-poor solvents, where these two solvents are shown to dissolve <2% (w/v) of PHAs at low temperatures. In this study, the suitability of ethanol and acetone for the recovery of PHB at different temperatures (from room temperature to near boiling point) in Cupriavidus necator was investigated. Experiments were performed using response surface methodology to examine the effects of different temperatures and heating incubation times on recovery percentage using the two solvents. The highest recovery percentage (92.3%) and produ

Penile Strangulation by a Plastic Band With an Unknown Time of Onset: A Report of a Rare Case.

We report a case of penile strangulation by a plastic band with an unknown time of onset. A 61-year-old man was brought to the emergency department with complaints of painful edematous swelling of the penis caused by a plastic band. The patient had an intellectual disability and, thus, difficulties were associated with obtaining a detailed medical history, including the time of onset. Penile strangulation was released by surgical scissors in the emergency room. The patient made a satisfactory recovery seven days after admission, and no major complications were observed.

Spontaneous epiglottic abscess: pathophysiology and airway management options.

Laryngeal abscesses are rare in the modern antibiotic era. This report presents a novel case of an epiglottic abscess in a patient with diabetes who developed respiratory distress and was managed by awake intubation in the emergency room followed by transoral incision and drainage of the abscess and tracheostomy. Full recovery after 1 week of intravenous antibiotic treatment was observed. Surgical intervention is necessary for treatment and culture-directed antimicrobial therapy. Poorly controlled diabetes is a newly described risk factor for spontaneous epiglottic abscess development.

Unusual complication of bifascicular block during surgery under general anesthesia.

This report describes the occurrence of Mobitz type II AV block during surgery under general anesthesia in a patient with apparently uncomplicated right bundle branch and left anterior fascicular block (RBBB and LAH). Although prophylactic pacing is not usually recommended in uncomplicated RBBB and LAH, the events in this case suggest that this abnormality may not always be benign during surgery. Continuous monitoring is essential and emergency equipment for temporary pacing should be readily available near the operating and recovery rooms.

"Taking a Turn for the Better": Does Self-reported Recovery From Walking Limitation Improve Disability and Mortality Outcomes?

OBJECTIVES: Recovery from self-reported walking limitation may be a turning point in an individual's health trajectory and may lead to better physical and mental health in the future. This research examines whether recovery from walking limitation is associated with onset of mobility disability, activities of daily living (ADLs) disability, or mortality among a national sample of older Americans. DATA AND METHODS: Using Waves 4 through 11 (1998-2012) of the Health and Retirement Study (HRS), discrete-time event history models (N = 12,579 person-intervals) with multiple competing events were e

Evaluating Remimazolam for Procedural Sedation.

Remimazolam, a novel short-acting benzodiazepine, has garnered interest in the realm of procedural sedation. Targeting a desired level of sedation requires a medication with both anesthetic and analgesic properties, particularly in non-operating room anesthetizing locations. Via gammaaminobutyric acid type A receptor agonism, Remimazolam has demonstrated organ-independent metabolism and rapid time to both onset and recovery and has efficacy for procedural sedation in clinical trials. This article provides a review of the current literature on the utility of remimazolam for procedural sedation.
