Voice Therapy Outcome Post Transoral Thyroarytenoid Myoneurectomy in Adductor Spasmodic Dysphonia: A Case Series.

OBJECTIVES: Postoperative evaluations of patients, who undergo of transoral thyroarytenoid myoneurectomy using CO2 laser for the treatment of Adductor Spasmodic Dysphonia (ASD), reveal some residual laryngeal symptoms such as intermittent spasms, vocal effort, and stiffness in laryngeal muscles which can be identified on videolaryngo-stroboscopy (VLS) by patterns of Muscle Tension Dysphonia (MTD) and mucosal wave, and as deviations in acoustic perceptual measures. This study aims to document these vocal symptoms observed postoperatively, and most importantly highlight the need for voice therap

Is Thyrohyoid Space Palpation a Reliable and Useful Clinical Tool Within Diagnostic Protocol for Voice Disorders?

OBJECTIVES: Palpation of the extrinsic laryngeal muscles is a common part of examination performed by otolaryngologists and speech-language pathologists on individuals presenting with voice complaints, thought to aid in diagnosis and treatment planning. While research has identified a significant relationship between thyrohyoid tension and hyperfunctional voice disorders, we are not aware of any studies exploring correlations between thyrohyoid posture during palpation and the full spectrum of voice disorders. This study aims to identify whether patterns in thyrohyoid posture at rest and durin

Voice Handicap Index Changes After Microflap Surgery for Benign Vocal Fold Lesions Are Not Associated With Recommended Absolute Voice Rest Duration.

PURPOSE: Voice rest is frequently prescribed after phonosurgery, but optimal type and duration for voice outcomes have not been demonstrated. Studies to date have been characterized by heterogeneity in surgical procedures and laryngeal diagnoses. We sought to analyze the effect of recommended absolute voice rest duration on outcomes of microflap surgery for benign vocal fold lesions. A secondary purpose was to identify patient factors associated with postoperative voice outcomes. METHOD: Forty-three patients were included in this retrospective review of patients aged 18 years and above who un

Hoarseness after thyroidectomy: blame the endocrine surgeon alone?

OBJECTIVE: Hoarseness is a postoperative complication of thyroidectomy, mostly due to damage to the recurrent laryngeal nerve (RLN). Hoarseness may also be brought about via vocal cord dysfunction (VCD) due to injury of the vocal cords from manipulations during anesthesia, as well as from psychogenic disorders and respiratory and upper-GI related infections. We reviewed the literature aiming to explore these potential surgical and non-surgical causes of hoarseness beyond thyroidectomy and the role of the endocrine surgeon. Is he/she alone to blame? METHODS/MATERIAL: The MEDLINE/PubMed databas

Predicting glottal closure insufficiency using fundamental frequency contour analysis.

BACKGROUND: Voice analysis has a limited role in a day-to-day voice clinic. We developed objective measurements of vocal folds (VF) glottal closure insufficiency (GCI) during phonation. METHODS: We examined 18 subjects with no history of voice impairment and 20 patients with unilateral VF paralysis before and after injection medialization laryngoplasty. Voice analysis was extracted. We measured settling time, slope, and area under the fundamental frequency curve from the phonation onset to its settling-time. RESULTS: The measured parameters, settling time, slope, and area under the curve wer

Automatic and quantitative measurement of laryngeal video stroboscopic images.

The laryngeal video stroboscope is an important instrument for physicians to analyze abnormalities and diseases in the glottal area. Stroboscope has been widely used around the world. However, without quantized indices, physicians can only make subjective judgment on glottal images. We designed a new laser projection marking module and applied it onto the laryngeal video stroboscope to provide scale conversion reference parameters for glottal imaging and to convert the physiological parameters of glottis. Image processing technology was used to segment the important image regions of interest.

Sulcus vocalis: A new clinical paradigm based on a re-evaluation of histology.

OBJECTIVES/HYPOTHESIS: Sulcus vocalis is an epithelial invagination adherent to deep tissues of the vocal fold. Traditionally, dysphonia is believed to result from attenuation or absence of lamina propria and consequent alteration of mucosal dynamics. This conception does not account for several clinical features of the lesion, most notably inflammation. The goal of this study is to elucidate the clinical nature of sulcus by re-examination of its histopathology. STUDY DESIGN: Retrospective review. METHODS: Clinical features, including stroboscopic examination, and hematoxylin and eosin secti
