Long-term functional speech and swallowing outcomes following pharyngolaryngectomy with free jejunal flap reconstruction.

Surgery for advanced cancer of the hypopharynx is a complex issue. Surgical intervention needs to take into consideration the resultant quality of life, in particular fundamental functional outcomes such as speech and swallowing. The aim of this study is to look at these long-term functional outcomes, following pharyngolaryngectomy and free jejunal reconstruction. A total of 19 patients, each undergoing a pharyngolaryngectomy with free jejunal graft was included. Each had a primary tracheoesophageal puncture for insertion of an indwelling voice prosthesis for speech. Functional outcomes of spe

[Analysis of complications after surgical voice and speech rehabilitation in laryngectomized patients. Problems related to implantation and change of voice prosthesis].

INTRODUCTION: Surgical rehabilitation of voice and speech, with implantation of Provox 2 voice prosthesis is becoming a standard procedure in Poland in the last few years. Still some opinions are raised, considering the potential risk of complications, particularly, when tracheo-esophageal puncture is finally located in the irradiated tissue. AIM OF THE STUDY: The assessment of safety, and analysis of complication encountered with tracheo-esophageal puncture, and implantation of voice prosthesis. MATERIAL AND METHOD: The 106 cases of patients treated with mentioned above method. The primary

Supraglottic hemipharyngolaryngectomy for the treatment of T1 and T2 carcinomas of laryngeal margin and piriform sinus.

BACKGROUND: We evaluated the functional and oncologic results of supraglottic hemipharyngolaryngectomy as treatment for T1 and T2 lateral laryngeal margin and piriform sinus carcinomas. METHODS: Eighty-seven patients underwent this surgical treatment. The disease was classified T1 in 14 of these cases (16.1%) and T2 in 73 cases (83.9%). The nodal status indicated 39 cases of N0 (44.8%), 18 cases of N1 (20.7%), 28 cases of N2 (32.2%), and two cases of N3 (2.2%). With regard to the N0 cases, 15 (38.4%) were positive at the histologic examination. Within the N+ group, 52.1% involved capsular rup

Vascularized hemitracheal autograft for laryngotracheal reconstruction: a new surgical technique based on the thyroid gland as a vascular carrier.

BACKGROUND: The management of extensive laryngotracheal stenosis has been a challenge confronting head and neck surgeons for over a century. The key to the successful restoration of a stable airway is providing a cartilaginous infrastructure to provide support to withstand both the negative and positive lumenal pressures produced during normal respiration and deglutition. We introduce a novel technique for restoration of such defects. METHODS: The blood supply to the thyroid gland by way of the inferior thyroid artery and the superior thyroid artery and vein are mobilized for transfer. One ha

Comparison of preoperative computed tomographic findings with postoperative histopathological findings in laryngeal cancers.

In this study of 22 patients with laryngeal cancer, computed tomographic (CT) scans in the axial plane were compared with histopathological sections prepared in the same plane. The value of the preoperative CT for evaluating tumor invasion, location and size was then investigated. Findings demonstrated that CT was most sensitive in determining tumor invasion to the paralaryngeal and preepiglottic spaces, anterior and posterior commissures and subglottis. In contrast, CT was less sensitive in determining actual tumor invasion to the laryngeal cartilages, extralaryngeal tissues and metastases to

Wound amylase levels as an early indicator of orocutaneous fistulae.

The formation of an orocutaneous fistula following ablative head and neck surgery is a common complication. Early fistula detection could permit prompt management, hopefully preventing further wound breakdowns. Amylase is quantitatively the most prominent enzyme in saliva. This study prospectively evaluated whether the amylase content of wound drainage could predict the formation of an orocutaneous fistula earlier than clinically evident. Patients undergoing procedures that required postoperative wound drainage represented the study population. Baseline serum and salivary amylase determination

lncRNA RASSF8‑AS1 suppresses the progression of laryngeal squamous cell carcinoma via targeting the miR‑664b‑3p/TLE1 axis.

Long non‑coding (lnc)RNAs have been found to play a crucial role in tumor progression. The present study aimed to investigate the association between lncRNA RASSF8‑AS1 and laryngeal squamous cell carcinoma (LSCC) and the underlying mechanisms. Reverse transcription‑quantitative PCR was used to measure the mRNA expression level of RASSF8‑AS1, microRNA(miR)‑664b‑3p and transducin‑like enhancer of split 1 (TLE1) in LSCC. The associations between RASSF8‑AS1 and miR‑664b‑3p, and between miR‑664b‑3p and TLE1 were investigated using a dual luciferase reporter assay, while the former was further verif

Leiomyosarcoma of Larynx as a Metachronous Tumor of Squamous Cell Carcinoma: An Unusual Case.

Leiomyosarcomas (LMSs) are malignant tumors of smooth muscles accounting for 5-6% of all soft tissue sarcomas. They are mostly seen in the gastrointestinal tractus, the uterus, and the retroperitoneum. Soft tissue sarcomas comprise <1% of all laryngeal malignancies, most of which are chondrosarcomas. Laryngeal LMSs are rarely seen malignancies, which are published as case reports. We presented a case of laryngeal LMS, which was seen 4 years later in a laryngeal squamous cell carcinoma patient who underwent laryngeal laser microsurgery. We discussed the diagnosis and treatment algorithm in the

An individualised treatment algorithm for tumour stage 1 glottic squamous cell carcinoma.

OBJECTIVE: To report and discuss the outcome of a treatment algorithm for patients with tumour stage 1 glottic squamous cell carcinoma. METHOD: A retrospective outcome analysis study was performed using data from a tertiary referral centre. RESULTS: Sixty-nine patients were treated with radiotherapy and 26 with surgery, in accordance with the treatment algorithm. Five-year overall survival rates were the same for both treatment groups (92 per cent). Five-year disease-specific survival rates were 100 per cent for surgery, 98 per cent for radiotherapy and 99 per cent overall. The overall 5-yea

Planned simultaneous cervical skin reconstruction for salvage total pharyngolaryngectomy.

BACKGROUND: Salvage surgery after definitive radiotherapy with or without chemotherapy is still controversial, especially in cases of hypopharyngeal cancer because of the poor prognosis and surgical complications. Irradiation of the skin results in loss of flexibility of the skin and impairment of the normal healing processes, thereby increasing the risk of wound infections, which could be potentially life-threatening. In an attempt to diminish the risk of major complications, we performed planned cervical skin replacement with salvage total pharyngolaryngectomy (TPL). METHODS: From 2005 to 2

Preservation of form and function during management of cancer of the larynx and hypopharynx.

The treatment of laryngeal and hypopharyngeal cancer has seen notable changes during the twentieth century. Intensive surgical research has allowed laryngologists to deal with almost any local extension and to propose an appropriate surgical procedure for each case. This surgery is able to control the disease in most patients locally but is limited in its indications (resectable diseases and operable patients). On the other hand, radiotherapy has been impressively improved (dosimetry, definition of target volume) and has become an indisputable alternative to surgery. The two approaches are com
