Rheumatoid Factor Is Associated With the Distribution of Hand Joint Destruction in Rheumatoid Arthritis.

OBJECTIVE: Rheumatoid arthritis (RA) is a chronic disease leading to joint destruction. Although many studies have addressed factors potentially correlated with the speed of joint destruction, less attention has been paid to the distribution of joint destruction in patients with RA. In this study, destruction of the hand bones in patients with RA was classified into 2 anatomic subgroups, the fingers and the non-fingers, with the aim of analyzing which factors are associated with destruction of the finger joints. METHODS: A total of 1,215 Japanese patients with RA were recruited from 2 differe

Clinical outcomes of primary skin closure with Y-V and Z-plasties for Dupuytren's contracture: use of one-stage skin closure.

Y-V or Z-plasties are a useful one-stage technique for skin closure after aponeurotomy. However, we know no details about postoperative improvement, particularly at each joint. The purpose of this study was to evaluate the clinical outcomes of primary skin closure with Y-V and Z-plasties for Dupuytren's contracture. We retrospectively reviewed the postoperative results of 23 patients (25 hands, 29 fingers). The preoperative severity of the contracture evaluated by the Meyerding classification was grade I in 11 fingers, II in two fingers, and III in 16 fingers. In total, 26 metacarpophalangeal

Involuntary Neuromuscular Coupling between the Thumb and Finger of Stroke Survivors during Dynamic Movement.

Finger-thumb coordination is crucial to manual dexterity but remains incompletely understood, particularly following neurological injury such as stroke. While being controlled independently, the index finger and thumb especially must work in concert to perform a variety of tasks requiring lateral or palmar pinch. The impact of stroke on this functionally critical sensorimotor control during dynamic tasks has been largely unexplored. In this study, we explored finger-thumb coupling during close-open pinching motions in stroke survivors with chronic hemiparesis. Two types of perturbations were a

Swan Neck Deformity Mimicking Claw Hand Caused by Arthritis in Leprosy.

Swan neck deformity is a hyperextension of the proximal interphalangeal (PIP) joints and flexion of the distal interphalangeal (DIP) joints. Claw hand is a hyperextension of the metacarpal joints and flexion of the PIP joints, accompanied by reduced motor strength. A 23-year-old female, who was released from leprosy treatment, presented with a bend of the second to fifth fingers of both hands. There was hyperextension of the PIP joints and flexion of the DIP joints from the second to fifth fingers of both hands, thickening of the ulnar nerves, and hypoesthesia without motor impairment of the f

Cross-finger flaps from scarred skin in burned hands.

An innovation is described in the secondary repair of two types of longstanding post-burn flexion contracture of the little finger. The correction of the PIP joint flexion contractures caused by palmar burns can result in a defect on the palmar side of the finger after incision or excision of the scar. The reconstruction of the post-burn buttonhole flexion deformity of the PIP joint, can result in a defect on the extensor surface of the PIP joint after excision of the dorsal scarred skin. Both acquired defects were covered by two different cross-finger flap techniques, despite extensive scarri

Amputations in the treatment of Dupuytren's disease.

23 finger amputations in 19 patients operated on for Dupuytren's disease were reviewed 6 months to 8.5 years after operation (mean 4 years). The distribution of amputations were 17 little fingers and six ring fingers. We found a recurrent lack of extension in nine out of 16 finger amputations distal to the MP joint and painful neuroma or phantom limb pain in five out of seven little finger amputations through or proximal to the MP joint. When amputation in the little finger is necessary, disarticulation of the MP joint may be preferable to amputation at a more distal level. Alternatives to fin

Efficacy of Minimally Invasive Ulnar Superficialis Slip Resection for Unfavourable Results after Trigger Finger Release.

Background: This study aimed to evaluate the efficacy of ulnar superficial slip resection (USSR) for improving hand function after unfavourable results after triggering finger release using a minimally invasive approach. Methods: We evaluated 17 consecutive fingers of 16 patients who complained of unfavourable outcomes after primary trigger finger release. The chief complaints of the two index and 15 middle fingers were proximal interphalangeal (PIP) joint pain during movement, flexion contracture of the PIP joint and snapping at the A2 pulley in eight, seven and two fingers, respectively. Use of digital artery perforator flaps for repairing soft tissue defects after fasciectomy for Dupuytren's contractures in the ring and little fingers: A case report.

Fasciectomy is the standard treatment for Dupuytren's contracture, but, in many cases, skin defects may occur after fasciectomy. If the preoperative contracture is severe, the skin defect is large, which makes covering the defect difficult. We describe a case of severe skin defects after fasciectomy of Dupuytren's contractures in the ring and little fingers covered with multiple digital artery perforator (DAP) flaps . A 58-year-old man with extension restrictions of the ring and little fingers on his left hand was diagnosed with Dupuytren's contracture. The angles of insufficient extension wer

New concepts in arthroplasty of the hand and wrist.

Clinical trials with cemented polyethyiene and metal total joint arthroplasties were initiated in 1973. Replacements have been developed for the wrist, carpometacarpal joint of the thumb, metacarpophalangeal joints of the thumb and fingers, and the proximal interphalangeal joints of the fingers. Results, evaluated by pain relief and joint stability, were excellent at all sites. Motion, however, averaged only 50% of normal. Significant problems included abnormal posture in the wrist, roentgenographic evidence of loosening in the finger metacarpophalangeals, extensor lag in the metacarpophalange

Bioinspired Robotic Fingers Based on Pneumatic Actuator and 3D Printing of Smart Material.

In this article, we have proposed a novel robotic finger design principle aimed to address two challenges in soft pneumatic grippers-the controllability of the stiffness and the controllability of the bending position. The proposed finger design is composed of a 3D printed multimaterial substrate and a soft pneumatic actuator. The substrate has four polylactic acid (PLA) segments interlocked with three shape memory polymer (SMP) joints, inspired by bones and joints in human fingers. By controlling the thermal energy of an SMP joint, the stiffness of the joints is modulated due to the dramatic

Spiral oblique retinacular ligament reconstruction using lateral band technique to treat swan neck deformity due to chronic mallet finger: A case report.

INTRODUCTION: Swan neck deformity (SND) is characterized by hyperextension of proximal interphalangeal (PIP) joint and extension lag of distal interphalangeal (DIP) joint with functional loss of finger and impairs of tight grip of the finger. SND often results from chronic mallet injury and requires surgical treatment. One of the procedure is spiral oblique retinaculum ligament (SORL) reconstruction. We reported good outcome of swan neck deformity due to chronic mallet finger cases treated with SORL reconstruction using lateral band technique. CASE PRESENTATION: We presented 2 case of swan ne

Correction of long term joint contractures of the hand by distraction. A case report.

Joint contractures are a common complication of hand trauma. The conventional treatment consists of arthrolysis, tenolysis and occasionally arthrodesis. Frequently, this does not achieve a good result, particularly when there has been a long delay in presentation. Progressive lengthening of a joint by distraction (joint distraction) allows the release of joint contractures even in cases of failure of traditional methods. We present a case of a delayed (20 years) work related traumatic flexion deformity of the PIP joint of the left index and middle fingers. This was the result of a complete div

Nonsurgical treatment of closed mallet finger fractures.

PURPOSE: Surgical repair of closed mallet finger fractures has been favored for displaced injuries involving more than one third of the articular surface and for injuries with palmar subluxation of the distal phalanx. This study analyzed the results of nonsurgical treatment for closed and displaced mallet finger fractures with greater than one-third articular surface damage, comparing cases with and without concomitant terminal joint subluxation. METHODS: Twenty-two closed mallet finger fractures in 21 patients who were treated nonsurgically and involving more than one third of the articular

Gold finger: metal jewellery as a disease modifying antirheumatic therapy!

Polyarticular psoriatic arthritis is a chronic, progressive and disabling auto-immune disease often affecting the small joints of the hands in a symmetrical fashion. The disease can progress rapidly causing joint swelling and damaging cartilage and bone around the joints resulting in severe deformities. We report a very unusual case of a 49-year-old woman who presented with polyarticular psoriatic arthritis affecting all proximal interphalangeal (PIP) joints of both hands except the left ring finger PIP joint. On clinical examination there was no evidence of arthritis in the left ring finger P

Three-Dimensional Motion Analysis of Finger in Sport Climbing in Different Age Groups.

Climbing with extreme finger positions is a risk factor for osteoarthritic changes. Our initial study revealed a large variation in PIP joint flexion, especially in older climbers. This study compares finger joint motion between age groups and examines relations to degenerative changes. Nine young (~33.9 years) and 11 old (~55.8 years) participants climbed a defined boulder problem in four grip positions: crimp, half-crimp, open-hand and participant-selected grip. Finger and wrist kinematics were recorded by an optical motion capture system, contact forces were measured using four instrumented

Soft Wearable Body-Powered Hydraulic Actuation System for a Prosthetic Finger Design.

OBJECTIVE: Finger and fingertip loss is the most common form of upper-limb amputation. With a focus on amputations involving the loss of distal and/or partial middle finger segments, this paper outlines the design and development of a novel soft body-powered hydraulically-driven actuation system for a prosthetic finger, while offering an in-depth examination of its subsystems. METHOD: The proposed device utilises a soft wearable hydraulic mechanism to transfer pressure from the proximal interphalangeal (PIP) joint of the human finger to the distal interphalangeal (DIP) joint of the prosthetic

Free vascularized joint transfer from the nonreplantable digit as a free flap for primary reconstruction of complex hand injury.

We report our experience in treating a a patient with an electrical saw injury to the right hand that resulted in incomplete amputation of the ring and small fingers at the metacarpophalangeal (MCP) joint with segmental tissue loss. Ray amputation of the small finger was performed because of extensive tissue loss. The proximal interphalangeal joint of the nonreplantable small finger was transferred as a fillet flap for primary reconstruction of the severely damaged MCP joint of the ring finger after revascularization. Two years after surgery active range of motion of the reconstructed MCP join
