Simultaneous cerebrospinal fluid and hematologic metastases in a high-grade ependymoma.

BACKGROUND: Ependymomas are relatively uncommon tumors that constitute about 7% of all primary intracranial neoplasms. Among these, high-grade ependymomas are locally aggressive and recur most commonly at the primary site following resection. Ependymomas are also known to be the one glial neoplasm that tends to frequently metastasize inside and outside the central nervous system (CNS) that complicates workup and management. Metastasis due to surgical manipulation is common and neurosurgeons should be well-versed in the most effective methods to remove these tumors in order to avoid such metast

[Retroperitoneal tumors: a case of liposarcoma].

The Authors report a case of retroperitoneal sarcoma, in the histological subtype "liposarcoma", occasionally diagnosed in a 57 year old woman, who presented aspecific symptoms, characterized by dry cough, vomiting and chest pain. Chest TC showed a neoplasm of the retroperitoneum, close to liver, right colon, duodenum and right kidney. The arteriography of the right renal artery showed a neovascolarization inside the mass. A surgical resection was performed. The histological examination confirmed the diagnosis of liposarcoma. Liposarcomas account for 30% of the retroperitoneal soft tissue sarc

Subgaleal recurrence of craniopharyngioma of rapid growing pattern.

The purpose of the present clinical case is to remind clinicians that craniopharyngiomas, which are benign neoplasms with a high incidence of local recurrences, may also present ectopic recurrences which may at first go unsuspected. These tumors most commonly arise in the suprasellar region and despite their benign histology, they may infiltrate the surrounding neurovascular structures making total removal challenging. Ectopic recurrences of craniopharyngiomas are very rare. We describe an adult patient with ectopic recurrence of craniopharyngioma, emphasizing unique features of the case prese

Inflammatory myofibroblastic tumor of omentum mimicking an ovarian lesion: A case report.

Inflammatory myofibroblastic tumor (IMFT) is a neoplastic entity with intermediate malignant potential, characterized by its tendency for local recurrence and, rarely, distant metastasis. It most often occurs in the lung, while extra-pulmonary sites, like the omentum, are rare and present with non-specific clinical and radiologic features, making diagnosis challenging. A 6-year-old girl presented with a 2-month history of intermittent fever, a painless abdominal lump, elevated inflammatory markers, and an ultrasound impression suggestive of an ovarian neoplasm, while magnetic resonance imaging

Rare Lower Extremity Fibrosarcomatous Variant of Dermatofibrosarcoma With Myxoid Features Treated With Transtibial Amputation.

Dermatofibrosarcoma protuberans (DFSP) is an uncommon soft tissue tumor originating from the dermis, with high rates of local recurrence and invasive growth but low likelihood of distant metastasis. Fibrosarcomatous transformation (FS-DFSP) of DFSP accounts for approximately 5-15% of DFSP tumors, is a higher-grade tumor, with higher chances of metastasis and poorer prognosis. We present a case of a 66-year-old female presented with a large fungating mass on the left dorsal foot. Ultrasound-guided core needle biopsy with immunohistochemistry suggested a spindle cell neoplasm, favoring myxofibro

Bowel cancer - guide for the GP.

BACKGROUND: Many patients will remain asymptomatic until the advanced stages of colorectal cancer and hence, will only be identified by means of a coordinated screening program. OBJECTIVE: This article outlines the risk assessment, early detection primary prevention and management of colorectal cancer. DISCUSSION: It is vital that general practitioners recognise the enormous variation in the patterns of clinical presentation of colorectal neoplasia such as rectal bleeding, iron deficiency anaemia, change in bowel habit, and unexplained weight loss. Any patient over 40 years of age presenting

[Handling fear of cancer recurrence].

Some fear of cancer recurrence is normal and may even be adaptive. However, high levels of anxiety may become problematic as it often results in for example sleeping problems, rumination, avoiding (incl. medical appointments) and/or physical complaints. For general practitioners and specialists it is important to recognizing this fear, discuss and ask questions about it and know how to intervene and refer. Validation and psychoeducation may be sufficient when fear is relatively low and manageable. When fear starts to interfere with normal life referral to a psychologist in the hospital or spec

The value of radiotherapy in patients with recurrent atypical meningioma: an update.

INTRODUCTION: Atypical meningiomas (WHO Grade 2) comprise approximately 18% of all meningiomas and may recur, despite surgical resection. Current evidence, derived from prospective cohort studies and retrospective series, supports the use of radiotherapy in achieving local control in atypical meningiomas. Given the lack of level 1 data, the role of RT in the management of recurrent disease in radiotherapy-naïve or previously irradiated patients remains a subject of ongoing debate, and optimal strategies are yet to be established. AREAS COVERED: This narrative review examines recent developmen

National Cancer Grid Virtual Tumor Boards of Head and Neck Cancers: An Innovative Approach to Multidisciplinary Care.

PURPOSE: Virtual tumor board (VTB) via videoconference facility involving multiple specialists in the decision making for various tumors is well accepted, especially in high-income countries. Information on virtual tumor boards for head and neck cancers especially from low- and middle-income countries is sparse. In this study, we have audited the findings of the National Cancer Grid VTBs performed for head and neck cancers. METHODS: All patients discussed in the head and neck VTBs at our center between December 2016 and February 2022 were included in the study. Details such as the type of ins

Molecular Markers that Matter in Salivary Malignancy.

Despite aggressive initial interventions, recurrent/metastatic salivary gland cancer is not uncommon. Standard chemotherapy has not been shown to have durable clinical benefits. Several potential molecular markers have been identified in different histologic subtypes of salivary cancers. The objective of this review is to highlight the molecular markers that have been targeted in clinical trials for salivary gland cancers.

Recurrence of Intraventricular Meningioma along the Surgical Track.

This is a report of three cases of intraventricular meningiomas treated in our department wherein the tumors recurred along the surgical tract. The meningioma in each case was histologically "benign" at the time of first surgery and even at the time of recurrence. Although reported earlier, such cases of recurrence of meningiomas along the surgical tract are relatively rare.

Surgical intervention in relapsed ovarian cancer is beneficial: contra.

Secondary cytoreduction in the management of patients with recurrent ovarian cancer is a commonly employed strategy. Unfortunately, there remain 'no evidence-based phase III trial data' to demonstrate the survival benefits of this approach compared with the reintroduction of chemotherapy without surgery at the time of documented progression of the cancer. In the absence of such evidence, it is critical that gynecologic oncologists employ their best clinical judgment when deciding if an individual patient is an appropriate candidate for this as yet unproven strategy.

Surgical treatment of parathyroid carcinoma (Review).

Cancer of the parathyroid gland is rare and most patients suffer from hyperparathyroidism (HPT). Therefore it is often difficult to distinguish between benign HPT and malignant disorders. Prognosis as well as therapeutic approaches are still limited. Surgical treatment should be considered the therapy of choice for primary and recurrent manifestation of parathyroid carcinoma. The initial operation includes en-bloc resection of the tumor and the ipsilateral thyroid lobe. Surgery may cure some patients, but is often only palliative and aims to control hypercalcemia. In some patients hypercalcemi

Early-stage glottic carcinomas: effect of tumor location and full-length involvement on local tumor recurrence after radiation therapy.

PURPOSE: To correlate tumor location along the vocal cord with local control in stage I and II glottic carcinoma. MATERIALS AND METHODS: Sixty-five patients with stage I and 18 patients with stage II glottic cancer were irradiated with 4-MV x rays. Most patients received a total of 64-66 Gy, at a dose of 1.8-2.0 Gy/d. RESULTS: In stage I tumors, local-control probability was 96% +/- 2 (standard deviation); in stage II tumors, 67% +/- 15. When stages I and II were considered together, tumors that involved the full length of the vocal cord were more likely to recur (P = .069). In stage I tumor

Recurrent pancreatic acinar cell carcinoma following a 7-year disease-free interval: A case report and literature review.

INTRODUCTION AND IMPORTANCE: Pancreatic acinar cell carcinoma (PACC) is a rare malignant pancreatic tumor, accounting for only 1% of all pancreatic neoplasms. It has distinctive clinical, radiological, and histological features. CASE PRESENTATION: Here, we report a case of PACC diagnosed 7 years after surgical treatment for pancreatic ductal adenocarcinoma (PDAC). The recurrence was suspected during routine follow-up via abdominal computed tomography scan, which showed a local mass without biliary ducts dilation, vascular involvement, or any specific radiological signs. However, clinical exam

Pediatric Undifferentiated Pleomorphic Sarcoma of the Cecum.

Background: Undifferentiated pleomorphic sarcoma (UPS) is a high-grade neoplasm typically diagnosed in older adults and localized to the extremities or retroperitoneum. Because of poor response to therapy and high rates of recurrence, this neoplasm is associated with a poor prognosis. Case Report: A 12-year-old female presented with weight loss, abdominal pain, fatigue, and diarrhea. She was profoundly anemic with occult blood-positive stools. On endoscopy, a fungating cecal mass was biopsied and diagnosed as malignant sarcomatoid neoplasm. The neoplasm was resected with clear margins during s
