Multimodal framework for swallow detection in video-fluoroscopic swallow studies using manometric pressure distributions from dysphagic patients.

PURPOSE: Oropharyngeal dysphagia affects up to half of head and neck cancer (HNC) patients. Multi-swallow video-fluoroscopic swallow studies (VFSS) combined with high-resolution impedance manometry (HRIM) offer a comprehensive assessment of swallowing function. However, their use in HNC populations is limited by high clinical workload and complexity of data collection and analysis with existing software. METHODS: To address the data collection challenge, we propose a framework for automatic swallow detection in simultaneous VFSS-HRIM examinations. The framework identifies candidate swallow in

Prevalence of Self-Reported Swallowing Difficulties and Swallowing-Related Quality of Life Among Community-Dwelling Older Adults in India.

Self-reported swallowing difficulties are highly prevalent but underreported among older adults. The aging population in India is increasing, yet there is a lack of empirical data on self-reported swallowing difficulties in older adults. In the present study, we aimed to estimate the prevalence of self-reported swallowing difficulties and assess the swallowing-related quality of life (QOL) among community-dwelling older adults in India. We recruited 361 older adults (60-91 years) from the community. Participants completed the Eating Assessment Tool-10 (EAT-10) to assess self-reported swallowin

Effect of Rest Interval Length on Oral Swallowing Pressure During Effortful Swallowing Exercises in Healthy Adults: A Crossover Design.

Evidence supporting the prescription of effortful swallowing (ES) as a rehabilitation exercise remains lacking. This study aimed to evaluate the effect of rest interval length between sets on oral swallowing pressure during ES exercises in healthy adults. This study was a randomized trial of participants using a crossover design. Forty-three healthy adults (25.0 ± 3.7 years; 32 women and 11 men) without swallowing difficulties were recruited and participated in all four conditions (rest intervals between sets: 0-, 1-, 3-, and 5-min intervals) in a random order. The exercise constituted five se

Refining measurement of swallowing safety in the Dynamic Imaging Grade of Swallowing Toxicity (DIGEST) criteria: Validation of DIGEST version 2.

BACKGROUND: Dynamic Imaging Grade of Swallowing Toxicity (DIGEST) is a validated method to grade the severity of pharyngeal swallowing impairment as a toxicity of cancer based on the degree and patterns of penetration/aspiration and pharyngeal residue over a standardly acquired radiographic modified barium swallow (MBS) study. Since its implementation in 2016, areas for the refinement of grading mild safety impairments have been identified by clinical and research users. The objective of this study was to assess the performance and validity of refined DIGESTsafety grading criteria (per DIGEST

Alterations of posterior pharyngeal wall movement during swallowing in postoperative tongue cancer patients: assessment with a videofluoroscopic swallowing study.

This study aimed to determine the association between the progressive contraction of the posterior pharyngeal wall and dysphagia in postoperative patients with tongue cancer. A videofluoroscopic swallowing study (VFSS) was performed in 34 patients after tongue cancer surgery. Images were analyzed using a two-dimensional video measurement software. Cases in which the processes on the posterior pharyngeal wall moved downward from the 2nd to 4th vertebral regions were defined as "normal type", other cases were defined as "abnormal type". Twenty-four patients showed normal movement of the posterio

MRI Assessment of Swallow Muscle Activation with the Swallow Exercise Aid and with Conventional Exercises in Healthy Volunteers: An Explorative Biomechanical Study.

Swallowing muscle strength exercises are effective in restoring swallowing function. In order to perform the exercises with progressive load, the swallow exercise aid (SEA) was developed. Precise knowledge on which muscles are activated with swallowing exercises, especially with the SEA, is lacking. This knowledge would aid in optimizing the training program to target the relevant swallowing muscles, if necessary. Three healthy volunteers performed the three SEA exercises (chin tuck against resistance, jaw opening against resistance and effortful swallow) and three conventional exercises [conv

Understanding speech and swallowing difficulties in individuals with Huntington disease: Validation of the HDQLIFE Speech Difficulties and Swallowing Difficulties Item Banks.

PURPOSE: As Huntington disease (HD) progresses, speech and swallowing difficulties become more profound. These difficulties have an adverse effect on health-related quality of life (HRQOL), thus psychometrically robust measures of speech and swallowing are needed to better understand the impact of these domains across the course of the disease. Therefore, the purpose of this study is to establish the clinical utility of two new patient-reported outcome measures (PROs), HDQLIFE Speech Difficulties and HDQLIFE Swallowing Difficulties. METHODS: Thirty-one participants with premanifest or manifes

Multimodal Swallowing Evaluation with High-Resolution Manometry Reveals Subtle Swallowing Changes in Early and Mid-Stage Parkinson Disease.

BACKGROUND: Parkinson disease (PD) has detrimental effects on swallowing function. Treatment options are largely behavioral; thus, patients would benefit from an earlier start to therapy. Early swallowing changes in PD are not well-known, so patients do not typically receive swallowing treatment until later in the progression of PD. OBJECTIVE: We used predictive modeling to determine what quantitative swallowing variables best differentiate individuals with early to mid-stage PD from healthy controls. METHODS: Participants included twenty-six individuals with early to mid-stage PD and 26 hea

Cardiac Dysphagia: When the Heart Disrupts Swallowing.

Cardiac dysphagia is a rare cause of dysphagia due to an enlarged left atrium compressing the esophagus. We describe three patients (ages 70, 71, and 31) with past medical histories of advanced heart disease who presented with diverse clinical manifestations of dysphagia. Recognition of cardiac dysphagia is important to prevent unnecessary esophageal interventions. Multimodal evaluation with barium swallow, esophageal manometry, endoscopy, and cardiac imaging facilitates diagnosis and management. These cases highlight the importance of considering cardiac causes of dysphagia in patients with s

Swallow screening of older adults at hospital admission.

OBJECTIVES: The risk of swallowing difficulties increases in older adults due to a combination of factors such as multiple comorbidities and polypharmacy. Swallowing may be further destabilised by the onset of acute illness. At hospital admission, these factors can be identified and combined with a simple screening tool to select patients for additional assessment and intervention. This prospective observational cross-sectional cohort study aimed to screen hospitalised adults age 75 years or older (older than 65 years for Māori and Pasifika) for swallowing risks, regardless of the reason for a
