STATEMENT OF PROBLEM: Endodontically treated teeth often require post-and-core restorations for structural support because of extensive hard tissue loss. Assessment of the effect of the residual dentin thickness on the biomechanical performance of these restorations is lacking.

CASE DESCRIPTION: The authors describe a patient who experienced a spontaneous dissection of an internal carotid artery that manifested initially as dental pain in an endodontically treated molar.
CLINICAL IMPLICATIONS: It is important for dental practitioners to be familiar with unusual causes of dental pain. In this case in which the cause of dental pain was uncommon, the authors referred the patient appropriately, thus avoiding potentially serious complications.

A 3-and-a-half-year-old male child presented with fused cariously involved right maxillary primary central and lateral incisors as well as a previously traumatized non-vital left primary central incisor with a draining fistula.
