Cutting torque measurements in conjunction with implant placement in grafted and nongrafted maxillas as an objective evaluation of bone density: a possible method for identifying early implant failures?

BACKGROUND: Bone grafts are frequently used to enable the placement of dental implants in atrophied jaws. The biomechanical properties of bone grafts used in one- or two-stage implant procedures (in comparison with the use of nongrafted bone) are not well known. PURPOSE: The purpose of this study was (1) to measure cutting torques during the placement of self-tapping dental implants in nongrafted bone and in bone grafts, either as blocks or in a milled particulate form, in patients undergoing implant treatment in an edentulous maxilla and (2) to identify implants with reduced initial stabilit

Five-year evaluation of implants placed at extraction and with dehiscences and fenestration defects augmented with ePTFE membranes: results from a prospective multicenter study.

BACKGROUND: Barrier membranes have been used to promote bone ingrowth on implants with dehiscences and fenestrations. Membranes also have been used to protect defects adjacent to implants placed at the time of extraction. The concept of guided bone regeneration relates to preferentially allowing cells from bone to migrate into various defects while excluding fibrous tissue and epithelium. The purpose of these procedures is to enhance bone-to-implant contact at the treated sites and to prevent mucosal complications. PURPOSE: The purpose of this article is to report clinical outcomes for implan

Fixed full-arch implant-supported prostheses in a patient with epidermolysis bullosa: a clinical case history report.

Epidermolysis bullosa (EB) is a rare skin disorder characterized by blister formation in response to minor trauma and accompanied by extracutaneous manifestations. The use of endosseous implants to support fixed prostheses for the rehabilitation of patients with recessive dystrophic EB might provide a considerably better clinical treatment outcome than traditional prosthodontic interventions. This case history report describes the clinical management of such an afflicted patient. Implants were placed immediately following teeth extractions and subsequently loaded with fixed full-arch prosthese

Prosthodontic rehabilitation of a patient diagnosed with multiple idiopathic root resorption.

A female college student was referred by her general dentist for evaluation of the interproximal caries-like lesions discovered during her biannual visit. The lesions were determined to result from external resorption of unknown origin. After the most affected teeth had been extracted and the other teeth were restored after periodontal surgery, an interim partial removable dental prosthesis was placed. Two months later, the resorption pattern had proliferated to the rest of her dentition, thus revising the treatment plan to involve full mouth extractions and to place immediate maxillary and ma

A Three-Dimensional Finite Element Analysis of Stress Distribution in Cement-Retained and Screw-Retained Implant-Supported Restorations in Mandible Using Porcelain-Fused-to-Metal and Zirconia Crowns.

Introduction Dental implants are widely used to replace missing teeth; however, the biomechanical performance of implant-supported restorations may be influenced by the prosthesis retention type, restorative material, and direction of occlusal loading. Unfavorable stress distribution may lead to biological and mechanical complications, affecting long-term implant success. The present study aimed to evaluate and compare the stress distribution patterns in cement-retained and screw-retained implant-supported restorations fabricated with porcelain-fused-to-metal and zirconia crowns under axial an

A Prospective Study of Immediately Loaded Single Implant-Retained Mandibular Overdentures: Long-Term Results with 15-Year Follow-up.

PURPOSE: To investigate the long-term predictability of simplifying mandibular overdenture treatment using single-stage surgery and immediate prosthetic loading of a single implant to provide a more affordable treatment option. MATERIALS AND METHODS: A total of 29 patients with a mean age of 68 years with problematic mandibular dentures were treated. A single implant (Brånemark System Mklll TiUnite, Nobel Biocare) was placed into the mandibular midline for primary stability. A ball attachment was placed, and the retentive cap was incorporated into the existing denture. The patients were recal

Combination of natural teeth and osseointegrated implants as prosthesis abutments: a 2-year longitudinal study.

Twenty-three patients with Kennedy Class I mandibular dentition were supplied with prostheses in the posterior parts of the mandible. On one side they were given a prosthesis supported by two implants (prosthesis Type I) and on the other side they received a prosthesis supported by one implant and one natural tooth (prosthesis Type II). Sixty-nine fixtures were inserted and 46 prostheses constructed. Eight of the fixtures were lost during the observation period. The failure rate of the implants was about the same in the two types of prostheses; five fixtures belonged to prostheses Type I (10.9

A multidisciplinary approach for the rehabilitation of a patient with an excessively worn dentition: a clinical report.

This clinical report describes a multidisciplinary approach to the diagnosis and treatment of a patient with a severely worn dentition. The treatment included osteotomy and immediate implant placement and loading in the mandible. The definitive restorations were implant- and tooth-supported metal ceramic restorations. These restorations were fabricated with metal occlusal surfaces at an increased occlusal vertical dimension, which provided acceptable esthetics and function.

A 5-year randomized trial to compare 1 or 2 implants for implant overdentures.

The hypothesis of this 5-y randomized clinical trial was that there would be no significant difference in the satisfaction of edentulous participants with removable complete overdentures attached to 1 or 2 mandibular implants. Secondary aims were to test changes in satisfaction between and within the groups from baseline to 5 y and differences between the groups in implant survival and prosthodontic maintenance over 5 y. Each of the 86 participants (mean age, 67 y) was randomly allocated to receive either 1 implant in the midline (group 1) or 2 implants in the canine areas (group 2) attached t

Endosseous implants and bone augmentation in the partially dentate maxilla: an analysis of 17 patients with a follow-up of 29 to 101 months.

PURPOSE: The aim of this study was to analyze the survival rate of endosseous implants placed in the partially dentate maxilla treated with sinus inlay block bone grafts. MATERIALS AND METHODS: Seventeen patients were subjected to bone augmentation procedures prior to or in conjunction with implant placement. Bone volumes were regarded as insufficient for implant treatment unless a bone grafting procedure was performed. The patients were treated with sinus inlay block bone grafts and endosseous implants in a 1- or 2-stage procedure. A total of 69 implants were placed in the patients who were

Periodontal aspects of osseointegrated fixtures supporting an overdenture. A 4-year retrospective study.

196 Brånemark implants in 86 consecutive patients rehabilitated by means of overdentures (6 upper jaws, 80 lower jaws) were observed longitudinally. In each jaw, only 2 implants were used to anchor the overdenture. 2 implants in the lower jaw showed some mobility at the abutment installation and were removed immediately. During the loading period (mean loading time 19.1 months ranging from 4 to 48 months), none of the implants showed any signs of non-integration. The marginal tissue reaction and plaque accumulation were monitored using conventional indices. Clinical methods and standardized ra

Alveolar split osteotomy for the treatment of the severe narrow ridge maxillary atrophy: a modified technique.

Alveolar bone splitting and immediate implant placement have been proposed for patients with severe atrophy of the maxilla in the horizontal dimension. A new modification of the classical alveolar bone splitting for the treatment of the narrow ridge in the maxilla is provided. Thirty-three dental implants in eight consecutive patients were evaluated retrospectively following the described modified split-crest osteotomy. Inclusion criteria were: inadequate maxillary buccolingual dimension, 3-4mm of crestal width, and sufficient height from alveolar ridge tip to maxillary sinus floor. Primary st

Implant-supported oral rehabilitation of a patient with pemphigus vulgaris: a clinical report.

Pemphigus vulgaris (PV) is a rare mucocutaneous vesiculobullous disease characterized by the development of autoantibodies against the desmosomal proteins. Current treatment is largely based on systemic immunosuppression using systemic corticosteroids. Immunosuppressive drugs used in the treatment of the disease may increase the risk of infection and delayed healing, which are of concern in dental treatment procedures in this group of patients. The clinical outcomes of implants in PV have not been investigated. We present a case of PV rehabilitated with an implant-supported prosthesis with a 3

Survival rate for implants placed in the posterior maxilla with and without sinus augmentation: a comparative cohort study.

BACKGROUND: The reduced bone height and proximity of the maxillary sinus are the most common limitations for placement of dental implants in the posterior maxilla. Reconstruction of the atrophic posterior maxilla can be performed with a sinus augmentation procedure. The aim of this cohort study is to compare the survival rate of implants placed in augmented sinus to implants placed in native bone in the posterior maxilla. METHODS: This study was designed as a prospective cohort study and included consecutively treated patients. Patients who required the sinus augmentation (test group) were tr

Multiscale analyses of the bone-implant interface.

Implants placed with high insertion torque (IT) typically exhibit primary stability, which enables early loading. Whether high IT has a negative impact on peri-implant bone health, however, remains to be determined. The purpose of this study was to ascertain how peri-implant bone responds to strains and stresses created when implants are placed with low and high IT. Titanium micro-implants were inserted into murine femurs with low and high IT using torque values that were scaled to approximate those used to place clinically sized implants. Torque created in peri-implant tissues a distribution
