Economic outcomes in clinical studies assessing hyperbaric oxygen in the treatment of acute and chronic wounds.

INTRODUCTION: Hyperbaric oxygen treatment (HBOT) is used to treat acute and chronic wounds. This systematic review was conducted to summarise and evaluate existing evidence on the costs associated with HBOT in the treatment of wounds. METHODS: We searched multiple electronic databases in March 2015 for cohort studies and randomised clinical trials (RCTs) that reported on the clinical effectiveness and treatment costs of HBOT in the treatment of acute or chronic wounds. RESULTS: One RCT and three cohort studies reported on economic as well as clinical outcomes. These studies comprised differe

Does self-certification reflect the cardiac health of UK sport divers?

BACKGROUND: Since 2009, the United Kingdom diving incident data show an increasing number of fatalities in the over-50s age group. Previous studies also suggest some divers take cardiac medications. Since 2001, diving medicals have not been mandatory for UK sport divers. Instead, an annual medical self-certification form, submitted to their club/school or training establishment, is required. We documented in a survey of UK sport divers the prevalence of cardiac events and medications and the frequency of medical certifications. METHODS: An anonymous on-line questionnaire was publicised. Correction for adiabatic effects in the calculated instantaneous gas consumption of scuba dives.

INTRODUCTION: In scuba-diving practice, instantaneous gas consumption is generally calculated from the fall in cylinder pressure without considering the effects of water temperature (heat transfer) and adiabatic processes. We aimed to develop a simple but precise method for calculating the instantaneous gas consumption during a dive. METHODS: With gas thermodynamics and water/gas heat transfer, the instantaneous released gas mass was modelled. In addition, five subjects made an open-water, air, open-circuit scuba dive to 32 metres' sea water. Depth, cylinder pressure and water temperature wer

Management of severe spinal cord injury following hyperbaric exposure.

There is an increasing body of evidence that drainage of lumbar cerebrospinal fluid (CSF) improves functional neurological outcome after reperfusion injury to the spinal cord that occasionally follows aortic reconstructive surgery. This beneficial effect is considered owing to lowering of the CSF pressure thereby normalising spinal cord blood flow and reducing the 'secondary' cord injury caused by vascular congestion and cord swelling in the relatively confined spinal canal. Whilst lacking definitive proof, there are convincing randomised controlled trials (RCTs), cohort data and systematic re

Inner-ear decompression sickness: 'hubble-bubble' without brain trouble?

Inner-ear decompression sickness (DCS) is an incompletely understood and increasingly recognized condition in compressed-air divers. Previous reports show a high association of inner-ear DCS with persistent foramen ovale (PFO), suggesting that a moderate-to-severe right-to-left shunt might represent a major predisposing factor, and more properly defining it as an event from arterial gas embolism (AGE). However, other conditions characterized by bubbles entering the arterial circulation, such as open-chamber cardiac surgery, do not produce inner-ear involvement, while sometimes damaging the brai

Middle ear barotrauma in a tourist-oriented, condensed open-water diver certification course: incidence and effect of language of instruction.

INTRODUCTION: In Professional Association of Diving Instructors (PADI) Open Water Diver certification courses that cater to tourists, instruction is often condensed and potentially delivered in a language that is not the candidate's native language. OBJECTIVE: To assess the incidence of middle ear barotrauma (MEBt) in open-water diver candidates during a condensed four-day certification course, and to determine if language of instruction affects the incidence of MEBt in these divers. METHOD: The ears of participating diving candidates were assessed prior to commencing any in-water compressio

Assessing potential divers with a history of congenital heart disease.

This article describes a structured approach to assessing the medical fitness of potential divers who have a history of congenital heart disease. The importance of a complete and accurate cardiac history, including details of surgery and other interventions is emphasized. Specific assessment of intracardiac shunts, exercise capacity and ability to deal with the physical challenge of diving, risk of diving-included pulmonary oedema, of arrhythmia and of incapacity in case of arrhythmia and the consequences of surgical and catheter treatment are discussed, including the risks associated with lun

Cutis marmorata and cerebral arterial gas embolism.

Dr Kemper and colleagues reported that, when air was injected into the cerebral circulation of pigs, they developed a rash that looked very similar to cutis marmorata of cutaneous decompression illness (DCI) and to livido reticularis. They postulated that cutaneous DCI in divers may be centrally mediated as a result of cerebral gas embolism. It would be helpful if Kemper et al. described the distribution of the rash in their pigs. In divers, cutaneous DCI is generally confined to parts of the body with significant amounts of subcutaneous fat, such as the trunk and thighs, and the rash often cr

The cardiac effects of hyperbaric oxygen at 243 kPa using inchamber echocardiography.

INTRODUCTION: The adverse effects of hyperbaric oxygen (HBO) on cardiac physiology are considered a potential hazard during the treatment of some patients. The haemodynamic effects of HBO are poorly understood and the incompatibility of electrical equipment inside the chamber has made assessment difficult. At Fremantle Hyperbaric Unit, we have modified an ultrasound machine (Logiq™ e) for safe use within the hyperbaric environment. The aim of this study was to evaluate the cardiac changes that occur during HBO using in-chamber transthoracic echocardiography (TTE) in subjects without evidence o

Two fatal cases of immersion pulmonary oedema - using dive accident investigation to assist the forensic pathologist.

Immersion pulmonary oedema (IPE) is being increasingly recognized in swimmers, snorkellers and scuba divers presenting with acute symptoms of respiratory distress following immersion, but fatal case reports are uncommon. We report two fatal cases of probable IPE in middle-aged women, one whilst snorkelling and the other associated with a scuba dive. In the snorkeller's case, an episode of exercise-related chest tightness and shortness of breath that occurred 10 months previously was investigated but this proved negative, and she was on no medications. However, at autopsy, moderate left ventric

Cardiac function and oxygen saturation during maximal breath-holding in air and during whole-body surface immersion.

INTRODUCTION: The magnitude of the oxygen-sparing effect induced by the diving response in humans is still under debate. We wished to compare cardiovascular changes during maximal breath-holding (BH) in air and during whole-body immersion at the surface in a group of BH divers. METHODS: Twenty-one divers performed a maximal static apnea in air or during whole-body immersion. Dopplerechocardiography, arterial blood pressure and haemoglobin saturation (SaO₂) were obtained at the beginning of, and at 1/3, 2/3 and maximal BH time. RESULTS: BH time was on the average 3.6 ± 0.4 min, with no differ

A prospective analysis of independent patient risk factors for middle ear barotrauma in a multiplace hyperbaric chamber.

INTRODUCTION: Middle ear barotrauma (MEBT) is the most common complication of hyperbaric oxygen therapy (HBOT). We wished to determine whether independent risk factors could predict which patients will require tympanostomy tubes in order to continue HBOT. METHODS: Data regarding demographics, medical history and physical examination were collected prospectively over one year. Multivariate logistic regression was used to analyse the data. RESULTS: One hundred and six patients were included. The cumulative risk of MEBT over the first five treatments was 35.8% and that for needing tympanostomy

Acute coronary syndrome and decompression illness: a challenge for the diving physician.

Decompression illness (DCI) is a syndrome with diverse clinical manifestations but in which cardiac symptoms are rare. In the presence of cardiac symptoms, the necessity to rule out an acute coronary syndrome (ACS) which requires prompt treatment may result in delay to appropriate recompression treatment. We describe three cases with cardiologic symptoms referred to our centre by the Emergency Department (ED) of our facility. The first was a 48-year-old woman who lost consciousness during a dive and required cardiopulmonary resuscitation. The final diagnosis was acute myocardial infarction and

Spontaneous pneumomediastinum in an 11-year-old boy after a shallow breath-hold dive.

Spontaneous pneumomediastinum is caused by pulmonary barotrauma due to transiently increased intra-alveolar and intra-bronchial pressure. The most frequent triggers of spontaneous pneumomediastinum in children are asthma and manoeuvres creating forced expiration. It has been rarely associated with breath-hold diving. Chest pain and dyspnoea are the main symptoms, and the diagnosis can be confirmed by chest X-ray. The treatment of choice is oxygen, analgesics and monitoring the patient. The recurrence rate is low. The main differential diagnoses of spontaneous pneumomediastinum are oesophageal

Provisional report on diving-related fatalities in Australian waters 2008.

INTRODUCTION: An individual case review of diving-related deaths, reported as occurring in Australia in 2008, was conducted as part of the DAN Asia-Pacific dive fatality reporting project. METHOD: The case studies were compiled using reports from witnesses, the police and coroners. In each case, the particular circumstances of the accident and details from the post-mortem examination, where available, are provided. RESULTS: In total, there were 19 reported fatalities (the same as for 2007), 17 involving males. Twelve deaths occurred while snorkelling and/or breath-hold diving, and six while s

Pneumomediastinum or lung damage in breath-hold divers from different mechanisms: a report of three cases.

Normally pulmonary over-inflation is not an issue during breath-hold diving, in contrast to lung squeeze. Compared with compressed air diving, pulmonary barotrauma is rare in breath-hold diving. Several mechanisms can lead to an increase in intrathoracic pressure in breath-hold diving that may cause alveolar rupture. Here we report three cases of pulmonary barotrauma in breath-hold diving. Using high-resolution chest tomography, bullous damage in Case 1, and pneumomediastinum in Cases 2 and 3 were detected. Transient neurological symptoms in Cases 1 and 2 suggested cerebral arterial gas emboli

Pulmonary oedema in breath-hold diving: an unusual presentation and computed tomography findings.

Haemoptysis and pulmonary oedema following deep breath-hold diving have been described in recent years. We describe the case of a 33-year-old healthy military diver who presented symptoms suggestive of pulmonary oedema after two breathhold dives, the first lasting 0.5-1 min and the second 1-2 min, to 6 metres' depth in the sea. The diagnosis was promptly confirmed with chest computed tomography showing bilateral interstitial infiltrates in the upper regions of the lungs. To our knowledge, this is the first report to document pulmonary oedema in this setting of shallow breath-hold diving with a

The effect of intravenous perfluorocarbon emulsions on whole-body oxygenation after severe decompression sickness.

INTRODUCTION: Decompression sickness (DCS) results from a decrease in ambient pressure leading to supersaturation of tissues with inert gas and bubble formation. Perfluorocarbons (PFCs) are able to dissolve vast amounts of non-polar gases. Intravenous (IV) PFC emulsions reduce both morbidity and mortality associated with DCS, but the mechanism of this protective effect has not yet been demonstrated. METHODS: Juvenile Dorper-cross sheep (n = 31) were anaesthetised and instrumented for physiological monitoring, IV fluid administration and blood sampling. Animals were compressed in air in a hype

Hyperbaric oxygen in the treatment of childhood autism: a randomised controlled trial.

BACKGROUND: Promising results with hyperbaric therapy for children with autism have been reported, but most involved the use of only mild pressure with oxygen supplementation. To date, there has been no randomised, blinded trial of 100% oxygen administered at hyperbaric pressure. This study evaluated the efficacy of hyperbaric oxygen therapy (HBOT). METHODS: Sixty Thai children with autism, aged three to nine years, were randomly assigned to receive 20 one-hour sessions of either HBOT at 153 kPa (1.5 ATA) or sham air at 116 kPa (1.15 ATA). Effects on behaviour were measured using the Autism T
