Case series of 100 supraorbital mini-craniotomies in patients with good grade aneurysmal subarachnoid haemorrhage at a single neurosurgical Centre.

BACKGROUND: Endovascular techniques are becoming more common for cerebral aneurysms, but not all patients are suitable and open surgery is necessary for some. The traditional pterional approach requires a large craniotomy and this carries some morbidity in itself. With the growing expectation for minimally invasive surgery, we present our experience in supraorbital mini-craniotomy for good grade aneurysmal subarachnoid haemorrhage. METHODS: Data on good-grade subarachnoid haemorrhage patients having aneurysms clipped via this approach were collected prospectively. Aneurysms at all anterior ci

Venetoclax Combined with Azacytidine Can Be a First-line Treatment Option for Elderly Blastic Plasmacytoid Dendritic Cell Neoplasm.

An 84-year-old Japanese man was diagnosed with blastic plasmacytoid dendritic cell neoplasm (BPDCN). We administered combination therapy using venetoclax and azacytidine. We observed neutropenia (Grade 4), thrombocytopenia (Grade 2), and stomatitis (Grade 3). After six cycles of treatment, the BPDCN abnormal cells in the bone marrow specimen almost disappeared, and atypical cells were not detected in a skin biopsy. We propose venetoclax combined with azacytidine as a useful treatment approach in elderly patients, although clinicians should be mindful that therapeutic modifications may be essen

Model-dependent and model-independent approaches for evaluating hepatic fibrosis in rat liver using shearwave dispersion ultrasound vibrometry.

This study assesses gradations of hepatic fibrosis in rat livers using both model-dependent and model-independent approaches. Liver fibrosis was induced in 37 rats using carbon tetrachloride (CCl4); 6 rats served as the controls. Shear wave velocity as a function of frequency, referred to as velocity dispersion, was measured in vitro by an ultrasound elastography method called shearwave dispersion ultrasound vibrometry (SDUV). For the model-dependent approach, the velocity dispersion data were fit to the Voigt model to solve the viscoelastic modulus. For the model-independent approach, the pat

Cystocele repair with interpositional grafting.

This article discusses a systematic approach to the repair of cystoceles using interposition grafting. Surgeons' opinions vary regarding which graft is most appropriate as there are several varieties for mesh interposition. High-grade cystocele repair using the porcine dermis interposition graft is successful and associated with few complications. Cystocele repair is typically low grade and does not require additional surgery.

Comparison of Cancer Detection Between Transrectal and Transperineal Prostate Biopsy in an Active Surveillance Cohort.

BACKGROUND AND OBJECTIVE: Transperineal (TP) prostate biopsy is increasingly favored over the transrectal (TR) approach because of a lower risk of infection, but comparative data on cancer detection remain limited. We analyzed a large institutional cohort to compare detection rates and disease location between the TP and TR approaches. METHODS: We included all men on active surveillance undergoing prostate biopsy at Memorial Sloan Kettering Cancer Center during the 26-mo transition from TR to TP. Data included demographics, biopsy method, histopathology, and magnetic resonance imaging (MRI) f

Classification of Osteonecrosis of the Femoral Head Stage on Radiographic Images Using Deep Learning Techniques.

While magnetic resonance imaging (MRI) is effective for detecting early-stage osteonecrosis of the femoral head (ONFH), it is often expensive and less accessible; conversely, radiography is more widely accessible but has limited sensitivity for early-stage diagnosis. We developed a deep learning approach using radiographic images to effectively classify ONFH stages, providing a more accessible method for early diagnosis and disease stage differentiation. The dataset consisted of 909 hip radiographs, yielding 1818 femoral head images (grade 0:1495; grade 1:80; grade 2:114; grade 3:93; grade 4:3

Effect of Maxillary Sinus Pneumatization on the Ease of Access to Prelacrimal Recess.

OBJECTIVE: Prelacrimal recess approach can be used to access lesions of the anterior wall of the maxillary sinus (MS). Moreover, the longer the prelacrimal recess window distance (PLRWD), the easier it is to access the anterior wall. This study aimed to define the correlation between maxillary sinus pneumatization (MSP) and PLRWD, a previously defined anatomic factor predictive of the ease of prelacrimal recess approach (PLRA). METHODS: In total, 506 sides of 253 participants were studied. In the axial image, the PLRWD, the distance between the anterior wall of the MS and the lacrimal duct, w

Sex-related differences in postoperative complications after pelvic exenteration and their relationship with surgical invasiveness in a multicenter study.

PURPOSE: To investigate the sex-related differences in postoperative complications following pelvic exenteration. METHODS: This retrospective subanalysis used a multicenter registry of patients who underwent pelvic exenteration for locally advanced or recurrent rectal cancer. We included 218 patients (177 men and 41 women) in this study. The sex-related differences in postoperative complications were evaluated, and sex-by-factor interactions were assessed using logistic regression models, including sex, each perioperative factor, and the corresponding interaction term. RESULTS: A significant

Sagittal adjusting screws for the correction of grade IV spondylolisthesis in a patient with Ehlers-Danlos syndrome: illustrative case.

BACKGROUND: Ehlers-Danlos syndrome (EDS) and its connective tissue laxity often result in high-grade lumbosacral spondylolisthesis. Patients present with debilitating symptoms and neurological deficits. Reports of surgical techniques in non-EDS patients for the treatment of high-grade lumbosacral spondylolisthesis mainly described an open approach, multilevel fusions, and multiple stages with different circumferential approaches. Sagittal adjusting screws (SASs) can be used in a minimally invasive (MI) fashion, allowing intraoperative reduction. OBSERVATIONS: A 17-year-old female with EDS pre

Safety and pathological impact of resection after high-dose PBT for locally advanced pancreatic cancer.

BACKGROUND: A multidisciplinary approach is essential for treating lethal pancreatic cancer. Proton beam therapy (PBT), with its potential for dose escalation, shows promising effect for the local tumor, but the safety of subsequent resection and its pathological impact remain uncertain. METHODS: A retrospective single-center analysis was conducted on patients with locally advanced pancreatic cancer who underwent multidisciplinary treatment, including PBT, chemotherapy and hyperthermia (January 2014 to December 2022). PBT target dose was 67.5 Gy concurrently gemcitabine infusion and local hyp

Preliminary assessment of an MRI-based grading system for leptomeningeal disease: an exploratory prognostic framework.

BACKGROUND: Current diagnostic approaches of leptomeningeal disease (LMD) rely heavily on cerebrospinal fluid (CSF) cytology, which shows significant limitations and the requirement for invasive procedures. We aim to develop an MRI-based grading scores for LMD diagnosis and prognosis that address current diagnostic limitations and provide standardized, reproducible assessment criteria. METHODS: We conducted a retrospective analysis of 32 adult cancer patients evaluated for suspected LMD. Two experienced neuroradiologists independently assessed MRI studies using our novel grading system, which

Multimodal melphalan delivery: improved safety and outcomes with autologous myeloma transplant in India.

BACKGROUND: High-dose melphalan-based autologous stem cell transplantation (ASCT) remains a cornerstone of treatment in multiple myeloma but causes severe gastrointestinal toxicity in up to 50% of patients due to mucosal injury. OBJECTIVE: To mitigate this, we assessed a multimodal approach of combined oral cryotherapy, extended duration of melphalan infusion (1 h compared to bolus), and upright patient positioning during administration. DESIGN: A cohort study using hospital patient records to evaluate the safety and outcomes of this approach at a tertiary hospital in South India, from Janua

A pressure comparison between midline and paramedian approaches to the cervical epidural space.

BACKGROUND: In the cervical spine, the ligamentum flavum (LF) is often incompletely fused at the midline. Therefore, accessing the epidural space (ES) using the loss of resistance (LOR) technique via the midline approach could be less reliable than the paramedian approach. Since the tactile sensation of LOR is due to abrupt loss of pressure upon entering the ES, we have compared pressure changes between the 2 different cervical epidural techniques. OBJECTIVES: The aim of this study was to compare pressure changes during the pathway to the cervical ES between the 2 approaches. STUDY DESIGN: A
