Transantral endoscopic orbital floor exploration: a cadaver and clinical study.

A cadaver and clinical study was performed to determine the value of transantral endoscopy in diagnosis and treatment of orbital floor fractures. Six fresh cadaver heads were dissected using a 30 degree, 4-mm endoscope through a 1 cm2 antrotomy. In the cadaver, the orbital floor and the course of the infraorbital nerve were easily identified. The infraorbital nerve serves as a reference point for evaluation of fracture size; three zones of the floor are described that are oriented relative to the infraorbital nerve. In the clinical study, nine patients with orbital floor fracture initially und

Using a titanium mesh plate to reconstruct the orbital floor after an incompletely reduced zygomaticomaxillary complex fracture.

A 30 year-old male came to us for correcting his severe left enophthalmos five months after a facial trauma in a traffic accident. CT scan of his left orbit showed a greatly increased orbital volume, which was resulted from the incomplete reduction of left zygomaticomaxillary complex (ZMC) fracture, unrepaired medial and lateral walls, and improper restoration of the orbital floor with a silicone sheet. Also, the decreased ocular volume, due to the evisceration of the ruptured eyeball with intraocular silicone ball implantation, contributed to the patient's enophthalmos. Because the patient re

[Chronic unilateral maxillary sinusitis caused by foreign bodies in the maxillary sinus].

Chronic maxillary sinusitis is a common condition. The disease is most often caused by blockage of the sinus ostia or presence of an oroantral fistula. We report two cases of unilateral chronic maxillary sinusitis associated with foreign bodies in the maxillary sinus. In the first case the foreign body consisted of dental amalgam, in the second case of two silastic sponges left behind from a former operation because of "blowout" fracture. In the case of benign chronic maxillary sinusitis and foreign bodies, functional endoscopic sinus surgery (FESS) preceded by a CT-scan is an excellent approa

The modified lateral window technique for retrieving root stumps from the maxillary sinus: Case report.

RATIONALE: Tooth extraction is a common surgical procedure in oral and maxillofacial surgery. Extraction of the upper molar teeth may result in root fracture for anatomical reasons, leading to the migration of fractured roots into the maxillary sinus. This can cause serious complications such as oroantral fistula, sinusitis, cellulitis, and subdural abscesses. To mitigate the risk of these complications, it is important to promptly remove the root stumps from the maxillary sinus and preserve alveolar bone for subsequent prosthetic rehabilitation. PATIENT CONCERNS: A 30-year-old male patient p

Traumatic dislocation of the globe into the maxillary sinus with early recovery of visual acuity: Case report and review of the literature on management.

Traumatic orbital fracture with prolapse of the globe into the paranasal sinuses is very rare. The visual prognosis is poor, and the medical and surgical management is still a topic of debate. We herein describe an extremely rare case of globe dislocation into the left maxillary sinus with complete recovery of visual acuity. A 67-year-old man presented with an isolated left-sided orbital floor fracture with dislocation of the globe into the maxillary sinus. The visual acuity was no light perception in his left eye. He was immediately taken to the operating room for globe repositioning and orbi

Risk factors in oroantral communication while extracting the upper third molar: Systematic review.

The removal of the upper third molar is a procedure commonly performed in oral and maxillofacial surgery. Maxillary third molars are generally less difficult to extract than mandibular third molars. The surgical removal of maxillary third molars is usually associated with low complication rates and low morbidity. This procedure involves the risk of developing complications such as oroantral communication, displacement into adjacent anatomic spaces, fracture of the maxillary tuberosity, and root fracture. Orthopantomograms are the standard preoperative imaging modality, but there is no proven t

Control of Mandibular Width With 3D Printed Occlusal Splint in Children With Multiple Mandibular Fractures.

BACKGROUND: The fracture of mandibular symphysis combined with bilateral condylar fractures often leads to changes in the width of the mandible, which significantly widens the face of the child. Therefore, it is necessary to reposition the mandible through accurate adduction. METHODS: To ensure that the mandible can be accurately repositioned, a 3D printed occlusal splint was used. Bilateral maxillomandibular fixation screws were implanted. The 3D printed occlusal splint was located on the maxillary dentition and fixed to the maxillomandibular fixation screws with wire loops. The reference ba

Direct resin composite restoration of maxillary central incisors using a 3D-printed template: two clinical cases.

BACKGROUND: Three-dimensional (3D) printing technology is used widely in dentistry for applications including implant surgery, oral and maxillofacial surgery, orthognathic surgery, endodontics and prosthodontics. Using a 3D-printed template makes performing the repair procedure faster and more convenient. The aesthetic restoration of anterior teeth can recover facial beauty, enhance speaking and chewing functions and improve the quality of life of the patient. CASE PRESENTATION: This article describes two kinds of clinical cases including fractured teeth and dental caries. In both, a 3D-print

Traumatic intrusion of permanent teeth. Part 1. An epidemiological study of 216 intruded permanent teeth.

An epidemiological study of traumatic intrusion of permanent teeth was performed on 216 teeth in 151 patients treated over a 50-year period at a major trauma center in Denmark (Copenhagen). This analysis showed that intrusion of permanent teeth was of rare injury only affecting 1.9% of traumatic injuries involving permanent teeth. The main etiologic factor appeared to be falling which resulted in axial impacts on maxillary or mandibular teeth. The most common injury patterns were intrusion without additional injuries (33.5%) and intrusion with crown fractures (60.5%). A few cases were combinat

National Analysis of Risk Factors for Nasal Fractures and Associated Injuries in Trauma.

Nasal fractures account for up to 58% of facial fractures. However, the literature characterizing associated injuries and risk factors for nasal fractures is sparse and is mostly composed of single-center experiences. This study sought to provide a large descriptive analysis and identify associated injuries and risk factors for nasal fractures in trauma using a national database. A retrospective analysis of the National Trauma Data Bank (NTDB) from 2007 to 2015 was performed. Patients ≥18 years of age with nasal fractures were included. A multivariable logistic regression model was used to ide

Evaluation of dental root fracture using cone-beam computed tomography.

OBJECTIVE: To evaluate the application of cone-beam computed tomography (CBCT) in the detection of dental root fractures and to analyse the demographic profile of these fractures. METHODS: The study group comprised 398 teeth that were examined by CBCT for determining whether they had a root fracture. Patient characteristics were recorded, and the location, fractured roots, fracture types and three-dimensional images of the related skeletal structures were analysed. Two experienced oral radiologists independently analysed each case and reached a consensus, and the diagnosis was graded in one o

Does the anatomy around the pterygomaxillary suture contribute to the risk of bad fractures in Le Fort I osteotomy?

Le Fort I (LF1) osteotomy, a common orthognathic procedure for the maxilla aimed at achieving maxillary mobility by separating the pterygomaxillary suture, poses a risk of bad fracture that may lead to complications and inadequate mobility. Our study analyzed two- and three-dimensional computed tomography images to identify the anatomical factors associated with bad fractures due to an LF1 osteotomy. Point 'a' is where the lateral pterygomaxillary suture on the axial image aligns with the zygomatic alveolar line near the line used for an LF1 osteotomy, with the base line connecting the bilater
