Pre-operative simulation of post-operative multifocal vision.

While multifocal intraocular lenses (MIOLs) are increasingly implanted to correct for presbyopia, how one sees with a multifocal correction is hard to explain and imagine. The current study evaluates the quality of various visual simulating technologies by comparing vision with simulated MIOLs pre-operatively and the implanted MIOLs post-operatively in the same patients. Two simulation platforms were used: (1) a custom-developed adaptiveoptics (AO) system, with two visual simulator devices: a spatial light modulator (SLM) and an optotunable lens operating under temporal multiplexing (SimVis);

Refractive Surprises After Cataract Surgery: Identification and Management in Optometric Practice- A Comprehensive Review.

Refractive surprises remain one of the most challenging postoperative complications in modern cataract surgery, where precision and patient expectations for plano refractive outcomes continue to rise. Despite advances in optical biometry, intraocular lens (IOL) formula optimization, and surgical technology, postoperative refractive error still occurs in a subset of patients and may significantly affect visual satisfaction and quality of life. This review synthesizes contemporary evidence on the causes, detection, and optometric management of refractive surprises, with a practical focus on the

Evaluation of Perception of Photic Phenomena Through Diffractive Multifocal Intraocular Lens Designs by Its Virtual Implantation.

PURPOSE: To analyze the difference in objective and subjective photic phenomena following virtual implantation of three different presbyopia-correcting diffractive intraocular lens (IOL) designs. METHODS: The study was conducted at JENVIS Research Germany. A prospective cross-over and double-masked trial design was used. Seventy-four healthy individuals without cataract (age: 18 to 50 years) were enrolled. All participants assessed photic phenomena with three diffractive IOL designs by virtual implantation using the VirtIOL device (10Lens S.L.U), which allows participants to view through the

Clinical outcomes of combined presbyopia and astigmatism correction following hydrophilic trifocal toric intraocular lens implantation in cataract patients: a prospective clinical study.

BACKGROUND: Presbyopia and corneal astigmatism frequently coexist in cataract patients, reducing visual quality and increasing spectacle dependence. Toric trifocal intraocular lenses (IOLs) aim to address both conditions simultaneously. This prospective study evaluated the visual performance, quality, patient satisfaction, and safety profile of the Liberty 677MTY toric IOL, and classified its functional profile based on distance-corrected monocular defocus curve analysis. METHODS: This prospective, single-center, non-comparative clinical investigation included 28 eligible cataract patients wi

Pilot Evaluation of the Outcomes After Cataract Surgery with Implantation of a New Diffractive Trifocal Intraocular Lens.

PURPOSE: To assess the clinical performance and patient-reported outcomes of a new trifocal intraocular lens (IOL) over a 6-month follow-up period. METHODS: Prospective, non-comparative, single-center study including 20 eyes (10 patients, age, 47-79 years) undergoing bilateral cataract surgery with implantation of the trifocal diffractive IOL AT ELANA 841 P (Carl Zeiss Meditec, Jena, Germany). Visual acuity (VA) and refraction changes were evaluated over a 6-month follow-up period. Likewise, other clinical and patient reported outcomes were analyzed: binocular defocus curve (1 month), patient

PURPOSE: To evaluate visual, refractive, and quality-of-vision outcomes after cataract surgery with a low-addition rotationally asymmetric refractive multifocal intraocular lens (Lentis Comfort Mplus LS-313 MF15; Teleon) in eyes with prior hyperopic corneal laser refractive surgery. METHODS: This was a retrospective observational study of 26 eyes (26 patients) implanted with the LENTIS Comfort Mplus LS-313 MF15 after uneventful surgery. Optical Properties Influence Visual Cortical Functional Resolution After Cataract Surgery and Both Dissociate From Subjectively Perceived Quality of Vision.

PURPOSE: To investigate the relation between optical properties, population receptive fields (pRFs), visual function, and subjectively perceived quality of vision after cataract surgery. METHODS: The study includes 30 patients who had recently undergone bilateral sequential cataract surgery. We used functional magnetic resonance imaging and pRF modelling methods to assess pRF sizes across visual cortical regions (V1-V3). Subjects also performed a complete ophthalmologic and psychophysical examination and answered a quality of vision questionnaire. RESULTS: Subjects with worse optical propert

The Multifocal Pathway: A Pilot Study of a Trainee-Led Multifocal Intraocular Lens Protocol in a Tertiary Referral Hospital in Australia.

PURPOSE: To develop a selection pathway to facilitate the use of multifocal intraocular lenses (mfIOLs) in cataract surgery in a public hospital setting. METHODS: A single-surgeon prospective cohort study in an Australian tertiary referral public hospital was conducted. A mfIOL selection pathway was designed and assessed. Outcomes measured included unaided distance (UDVA), intermediate (UIVA) and near visual acuity (UNVA), dysphotopsia, spectacle dependence and satisfaction. Patient-reported outcome measures (PROMs) were assessed using Catquest-9SF (CQ) and Near Visual Acuity Questionnaire (N

Patient-reported outcome measures after multifocal and extended depth of focus intraocular lens implantation in refractive lens exchange and cataract surgery.

Presbyopia-correcting intraocular lenses (IOLs) represent a fast-growing segment of ophthalmic surgery. The evaluation of lens performance has undergone a fundamental shift: best-corrected visual acuity alone is an insufficient measure of the clinical benefit of premium IOLs, whose true value lies in spectacle independence, visual quality, and visual function in daily activities. In that regard, patient-reported outcome measures (PROMs) are a cornerstone of presbyopia-correcting IOL assessment. This review provides an overview of the current landscape of IOLs, PROM instruments and their limita

Which intraocular lens would ophthalmologists choose for themselves?

PURPOSE: Contemporary cataract surgery is increasingly customizable with the advent of presbyopia-correcting intraocular lenses (IOLs) and the femtosecond laser. The purpose of this study was to determine the types of IOLs ophthalmologists choose for themselves and whether demographic characteristics, surgical experience, and attitudes of ophthalmologists might influence their decision-making. Additional goals included evaluating the use of femtosecond laser use in cataract surgery and investigating surgeon IOL preferences for their patients. METHODS: We distributed a 29-question Survey Monke
