Pepsin assay: a marker for reflux in pediatric glue ear.

OBJECTIVE: To evaluate if analysis of pepsin/pepsinogen in middle ear effusions can be considered a diagnostic marker for laryngopharyngeal reflux (LPR) in children with otitis media with effusion (OME). MATERIAL AND METHODS: Ambulatory 24-hour dual-probe pH monitoring was carried out on 31 children with OME. Middle ear effusions were collected from 17 children during myringotomy. Total pepsin/pepsinogen concentrations in effusions were measured by ELISA using antipepsin antibody. RESULTS: Dual-probe pH monitoring showed that 22/31 (71%) of the studied children had significant LPR. The conce

Morbidity after adenotonsillectomy for paediatric obstructive sleep apnoea syndrome: waking up to a pragmatic approach.

INTRODUCTION: Adenotonsillectomy is successful at eliminating airway obstruction in the majority of otherwise normal children with obstructive sleep apnoea syndrome. Children with this condition are at significantly higher risk of post-operative respiratory complications. Identifying children at risk of post-operative respiratory complications after adenotonsillectomy for obstructive sleep apnoea syndrome remains a challenge for clinicians, especially those at district general hospitals. AIM: To review the evidence and to proffer a pragmatic approach to diagnosis and management, by classifyin

[Melioidosis-like disease: infection with Burkholderia cocovenenans as rare differential diagnosis for Lymphadenitis colli].

INTRODUCTION: Neoplasms, autimmune disorders and infectious diseases as differential diagnoses of cervical lymphadenopathies also require the consideration of rare causes. CASE REPORT: A 25-year-old patient presented for further diagnosis and treatment of a colliquating, high febrile cervical lymphadenopathy. The patient from Thailand who had been living in Germany for 8 years reported she worked as rice farmer during the 1980s. Examination showed a vast physical condition with severe weight loss, joint- and swallowing aches which did not respond to high doses of parenteral antibiotic treatme

Grisel's syndrome: the two-hit hypothesis--a case report and literature review.

Grisel's syndrome is a rare but well-documented clinical entity. It is a nontraumatic, fixed rotary subluxation of C1 on C2 (atlantoaxial). Although first described in 1830, the exact mechanism of Grisel's syndrome remains unclear. We present a postoperative case of Grisel's syndrome and an extensive literature review, and we propose a mechanism for its pathogenesis. In addition, we propose a treatment algorithm for Grisel's syndrome.

Surgical management for snoring and sleep apnea.

Obstructive sleep apnea is a serious medical problem producing both physical and behavioral derangement. It is essential to provide a thorough workup and evaluation of all patients seeking care for snoring or OSA. Polysomnography is the standard for evaluation and assessment of the severity of OSA in every patient. The evaluation and workup for surgical intervention should include a thorough history, complete head and neck evaluation, nasopharyngeal laryngoscopy with a flexible fiberoptic endoscope, and appropriate imaging (e.g., cephalometrics). This workup allows pathologic entities of the u

Chronic lymphocytic leukaemia with upper airway obstruction.

We report a case of B-cell chronic lymphocytic leukemia in a 58 year old female in whom the clinical course was dominated by upper airway obstruction due to massive enlargement of the palatine and later the lingual tonsils. The peripheral blood morphology and immunophenotype were typical of chronic lymphocytic leukaemia with expression of CDl9+, CD20+, CD5+, CD23+ and HLA-DR+ together with weak, surface immunoglobulin with monoclonal lambda light chain. Therapy included surgical removal of the palatine tonsils and later chemotherapy, both of which provided temporary relief of obstruction befor

Pathogen reservoir hypothesis investigated by analyses of the adenotonsillar and middle ear microbiota.

INTRODUCTION: Adenotonsillar and middle ear diseases result in some of the most frequently performed operations in the pediatric population worldwide. The pathogen reservoir hypothesis (PRH) suggests that the adenoids act as a reservoir of bacteria which play a potential pathogenic role in otitis media. Evidence supporting this hypothesis is limited. This study sought to comprehensively determine and compare associations between the adenotonsillar and middle ear bacterial microbiota within individual patients via next-generation sequencing and microbial network analyses. METHODS: Bacterial 16

[Sleep bruxism in children].

Bruxism is defined as repetitive activity of the masticatory muscles, characterized by clenching the teeth or teeth grinding and/or by tapping and swaying. This study investigated sleep bruxism. The etiology is multifactorial: mainly central (neuropathic disorder, anxiety) but also genetic and local (posture, mouth breathing). The diagnosis is based primarily on the anamnesis and examination of dental wear and progression over time (photos and dental castings). A diagnostic guide is proposed in this article. Frequently found in children, bruxism is not always considered pathological. The sever

Suction diathermy adenoidectomy performed in the district general hospital.

BACKGROUND: Adenoidectomy is often carried out in children for conditions such as nasal obstruction, otitis media with effusion, and obstructive sleep apnoea. Traditionally, it is performed as a blind procedure with a St Clair Thomson curette. An acceptable alternative technique is suction diathermy adenoidectomy. This study aimed to ensure that the complication rate of this latter technique was within published rates and national guidelines. METHOD: A retrospective case note review was conducted, and information regarding surgery, indications and complications was collected. RESULTS: Post-o

Variation in post-adenotonsillectomy admission practices in 24 pediatric hospitals.

OBJECTIVES/HYPOTHESIS: There is controversy about which children should be admitted after adenotonsillectomy (T&A) and limited clinical evidence to help with this decision. Current practice has evolved based on empirical or anecdotal evidence. We sought to identify practice variations in postoperative admission after T&A in tertiary care pediatric hospitals. STUDY DESIGN: Retrospective database study using administrative information stored in the Pediatric Health Information System (PHIS) database. METHODS: There were 29,920 T&As performed in 24 pediatric hospitals included in the PHIS datab

To what degree does adenotonsillectomy affect neurocognitive performance in children with obstructive sleep apnea hypopnea syndrome due to adenotonsillar enlargement?

BACKGROUND: Obstructive sleep apnea/hypopnea syndrome (OSAHS) is a common childhood problem; complications include possibly reduced neurocognitive performance. Treatment of OSAHS is usually by adenotonsillectomy, but whether surgery relieves both sleep disturbances and associated behavioral problems to the same degree or not is a reasonable question. OBJECTIVE: To correlate the degree of OSAHS due to adenotonsillar hypertrophy to the preoperative level of the intelligence quotient (IQ) of children, and to assess the degree of postoperative change, if any, in IQ. PATIENTS AND METHODS: The IQ

Relation between CT axial cross-sectional area of the oropharynx and obstructive sleep apnea syndrome in adults.

PURPOSE: To look for correlation between obstructive sleep apnea syndrome (OSAS) and axial cross-sectional area of the narrowed oropharyngeal lumen. METHODS: Thirty-six men with OSAS and 10 control subjects underwent polysomnography with registration of oxygen saturation and number of sleep apnea episodes and their duration. Nine of them underwent uvulopalatopharyngoplasty and repeated polysomnography. Each polysomnography was followed by high-resolution CT scan with reconstructions in sagittal and coronal planes. Measurements of the axial cross-sectional area of the oropharyngeal lumen were

New strategies and perspectives on managing IgA nephropathy.

IgA nephropathy is an inflammatory renal disease characterised by the deposition of IgA in the glomerular mesangium and is the most commonly reported primary glomerulonephritis worldwide. Thirty to forty percent of patients with the disease develop progressive renal function decline, requiring renal replacement therapy within two decades of diagnosis. Despite this, accurate individual risk stratification at diagnosis and predicting treatment response remains a challenge. Furthermore, there are currently no disease specific treatments currently licensed to treat the condition due to long standi

Swallowing dysfunction and quality of life in children with tonsillar hypertrophy and sleep-disordered breathing: A prospective cohort study.

BACKGROUND: Tonsillar hypertrophy is a common cause of sleep-disordered breathing (SDB) in children and may contribute to swallowing dysfunction through upper airway obstruction. However, its impact on swallowing-related quality of life has not been well characterized. OBJECTIVE: To evaluate swallowing-related quality of life in children with tonsillar hypertrophy and SDB, assess the effect of surgical treatment, and validate an Arabic version of the Pediatric SWAL-QOL questionnaire. METHODS: In this prospective cohort study, 50 children with SDB and Brodsky grade ≥2 tonsillar hypertrophy we
