The temporomandibular joint (TMJ) status of 51 juvenile orthodontic patients was assessed with magnetic resonance imaging (MRI), clinical examination, and questionnaire data. The results of this study demonstrated that the prevalence of anterior displacement of the meniscus was 11.8% (6 of 51) as assessed by MRI. Clicking or pain in the TMJ area was found in 9.8% (5 of 51) of the subjects by clinical exam, and 19.8% (10 of 51) of the subjects had a history of pain or clicking of the TMJ. Three subjects had a positive MRI and a negative history and clinical examination. However, all subjects wi

Cytokines are local mediators released by cells of the immune system in response to stimulation by a variety of agents. These polypeptides may interact directly or indirectly with bone cells. The objectives of this study were (1) to localize prostaglandin E (PGE) and the cytokine interleukin-1 beta (IL-1 beta) in the periodontal ligament after the application of mechanical force to teeth in vivo and (2) to determine the effects of mechanical stress or IL-1 beta (or the two in combination) on PGE synthesis and bone resorption by fibroblasts in the human periodontal ligament (PDL). In 24 female

A three-dimensional morphometric method was used to evaluate progressive changes in shape and size of recovering dental and periodontal tissues after orthodontic loading. In 35 female rats weighing 212 +/- 4 gm, loads of 19.7 +/- 1.6 gm generated by closed-coil springs were applied for 2 weeks to the shortened lower left incisor. The rats were killed in groups of five at 0, 1, 3, 5, 7, 9, and 10 weeks (groups 0-w to 10-w) after the springs were removed. A group of rats with normal incisors (group C-1) and one with five incisors that had been continually shortened for 10 weeks (group C-2) serve

Aberration in the eruption process was found to be a prime etiologic factor in inducing impaction. Thus an ideal treatment approach should attempt to mimic the normal eruption modus. However, conventional traction methods have been found to be associated with gingiva inflammation, bone recession, reduced attached gingiva, periodontal pockets, exposed cementoenamel junction, and root resorption of the impacted and adjacent teeth. These side effects are the result of premature exposure of the impacted tooth to the oral cavity through a nonself-cleansing pathway and an uncontrolled force system.

The Class III malocclusion with mandibular prognathism can involve many factors, among which are excessive mandibular growth, underdevelopment of the maxilla, environmental factors, and trauma to the jaws. The correction of this malocclusion can involve an orthodontic or a combined orthodontic-orthognathic approach. Skeletal asymmetries can complicate this situation, making treatment either more difficult, more complicated, or both. This case presentation involves treatment with a combined orthodontic-orthognathic approach. [This case was presented to the American Board of Orthodontics in part

This is a case report of a 21-year-old man who had a gradually progressive Class III malocclusion and crepitation of the right temporomandibular joint. Routine TMJ tomograms revealed a radiopaque-radiolucent lesion of the right mandibular condyle with a compensatory anteroinferior displacement of the left mandibular condyle. Nuclear bone scans confirmed increased asymmetric technetium 99 tracer uptake in the region of the right temporomandibular joint. A two-stage surgical approach was undertaken; this consisted of excision of the condylar lesion to correct the Class III malocclusion and poste

The bony nasopharynx increases its capacity predominantly by means of vertical development. Analogous to the facial structures is an expression of some somatotopic features that give rise to a higher and more shallow pharyngeal dimension in the dolichocephalic growth pattern. The nasopharynx's ultimate patency, however, depends on the growth and relative size of the soft tissues that line the skeletal boundaries. A decreased patency of the oropharyngeal airway can induce some postural adaptations, which secure a constant sagittal dimension at that level. However, at a certain critical point, p

The purpose of this study was to assess the oral response to severe nasal airway impairment in patients with cleft palate. Inductive plethysmography was used to measure the percent of nasal breathing, and the pressure-flow technique was used to estimate nasal area in 15 persons with severe nasal airway impairment. Mean nasal area was 0.17 cm2, and the average percent of nasal breathing was 20%. Analysis revealed a strong correlation (0.87) between nasal size and percent of nasal breathing in this selected group. Modeling studies based on the mean values from the subjects' data indicated that t

Diagnostic tests are imperfect and vary in their sensitivity and specificity. The degree of imprecision may be calculated to yield probability estimates of accuracy for both the positive and negative predictions of tests under various conditions. Such information enables clinicians to decide whether to accept or reject test results or the tests themselves. Two pilot studies are reported to establish the diagnostic potential of cephalometric measurements and nasal resistance values for the identification of upper airway impairment. A linear estimate of adenoid size and an area index of adenoid

The amount of maxillary and mandibular growth and the direction of maxillary growth were studied in 38 children during the 5 years after adenoidectomy for correction of severe nasopharyngeal obstruction. The amount of mandibular growth measured between successive gnathion points on superimposed radiographs was significantly greater in the group who had an adenoidectomy than in the matched controls. In the boys the difference was 3.8 mm (p less than 0.001), and in the girls the difference was 2.5 mm (p less than 0.01). The boys also showed a tendency toward greater growth in the maxilla as meas

This study examined the records of a consecutive series of 79 patients referred for evaluation at the Oral-Facial and Communicative Disorders Program during a 3-month period in 1989. The purpose was to determine whether clinical judgments of hyponasality, based on a six-point equal-appearing interval scale or an acoustic assessment with a Kay Elemetrics nasometer could provide information concerning nasal airway patency comparable to that obtained by means of aerodynamic measurement techniques. Among the 40 adults in the series, the sensitivity of hyponasality ratings was 0.55 when nasal airwa

Ten orthodontic patients in whom a headgear-activator appliance was used in the first phase of treatment for Class II malocclusion were clinically evaluated. All patients had abnormal perioral muscle function at the initial examination. Myofunctional methods were used before and during active treatment. Pretreatment and posttreatment cephalometric tracings were evaluated to examine the effect of the appliance on dental, skeletal, and soft tissue structures. Five of the 10 cases illustrated distal movement of the lower first molar, a phenomenon not mentioned previously in the literature. Growth

Multivariate analysis of patient characteristics and clinical variables was carried out with the maximum single maxillary incisor apical root resorption for each patient as the dependent variable. Root lengths were measured in standardized intraoral radiographs from 485 consecutively treated patients, 11.5 to 25 years of age. The correlation matrix revealed a complex pattern of positive and negative associations between the six pretreatment and seven treatment variables. Variables found to contribute significantly to apical root resorption were overjet, history of trauma to maxillary incisors

The purpose of this study was to determine whether vital and endodontically treated incisors exhibit a similar severity of apical root resorption in response to orthodontic treatment. Forty-three patients who had one or more endodontically treated incisors before orthodontic treatment and who exhibited signs of apical root resorption after treatment were studied. In each patient the vital contralateral incisor served as a control. Vital incisors resorbed to a significantly greater degree than endodontically treated incisors (p less than or equal to 0.05). When patients were separated by gender

Secondary retention refers to cessation of eruption of a tooth after its emergence. It occurs mainly in the deciduous dentition, but it is also seen on rare occasions in the permanent dentition. Although ankylosis has been suggested as the main etiologic factor, the exact biologic mechanism of secondary retention is not yet known. This article presents a case of spontaneous reeruption of a secondarily retained permanent molar, which is very rare. It also describes an unusual migration of a lower third molar in the same patient.

The purpose of this study is to determine the relationship of third molars to changes in the mandibular dental arch. The sample for this study consisted of four groups and subgroups. The groups consisted of premolar extraction treated, nonextraction treated with initial generalized spacing, nonextraction treated, and serial extraction untreated subjects. The subgroups were divided into persons who had mandibular third molars that were either impacted, erupted into function, congenitally absent, or extracted at least 10 years before postretention records. The mean postretention time interval wa

The relapse tendency of orthodontically rotated teeth after electrosurgical circumferential fiberotomies and after conventional scalpel blade surgical procedures was demonstrated with a split-mouth procedure involving the maxillary second incisors of five mongrel dogs. The second incisors were rotated orthodontically an average of 63.5 degrees over a 4-month period. Electrosurgical circumferential fiberotomies were performed on the designated second incisors, and circumferential fiberotomies with a scalpel blade were performed on the contralateral incisors. The teeth were retained for 1 month

This study investigated the treatment effects produced by two types of functional appliance, one primarily tooth-borne (the Herbst appliance) and one primarily tissue-borne (the FR-2 of Fränkel). Serial cephalometric radiographs from 45 patients who had been treated with the acrylic splint Herbst appliance and from 41 patients who had been treated with the FR-2 appliance were compared with serial radiographs of 21 untreated persons with Class II malocclusions. Treatment effects were identified through the use of a conventional cephalometric analysis. The results of this study indicated that bo
