INTRODUCTION: acetabular dysplasia in adults represents a therapeutic challenge due to progressive joint instability, pain, and functional limitation. This article presents a case series using acetabuloplasty with autologous femoral head graft, combined with total hip arthroplasty, as a treatment option for adult patients with severe acetabular dysplasia and osteoarthritis.
MATERIAL AND METHODS: a prospective case series was carried out between 2023 and 2024 at Cancun General Hospital. Five adult patients diagnosed with advanced hip dysplasia (Crowe II-IV) and Tönnis grade IV coxarthrosis wer

AIM: Various pericapsular procedures are available to surgically improve the acetabular coverage of the femoral head prior to closure of the triradiate cartilage. In this study the acetabuloplasty with the modification according to Westin (Pember-Sal) was applied. Indications for surgery were congenital hip dysplasia or luxation as well as Perthes' disease. To date, the standard procedures for acetabuloplasty include the transplantation of an autologous iliac crest bone graft and the fixation with K-wires. The aim of this study was to investigate if a modification of the operative procedure wi

The goal of all therapies for Perthes' disease is to achieve an optimal shape of the acetabulum and an optimal coverage of the femoral head. Thirty patients who were included in this follow-up study (mean follow-up 12 years) underwent a shelf acetabuloplasty for Catterall group III or IV. The mean IOWA hip score at follow-up was 96 (max. 100 points, range 74-100). The mean acetabular-head quotient increased from 82.9% pre-operatively to 107.9% postoperatively, and remained 102.4% at follow-up. There was a decrease in mean lateral subluxation ratio from 1.44 pre-operatively to 1.27 postoperativ

Triple pelvic osteotomy (or triradiate osteotomy) enables a Salter innominate osteotomy to be performed in older children, or even in adults. The displacement of the acetabular fragments is made possible by a double osteotomy near the symphysis in the anterior pelvic girdle. After the age of seven to eight years, the lack of elasticity in the symphysis pubis makes it technically difficult to perform a classic Salter osteotomy. On the basis of a study of the first twenty cases in which the operation was performed, the authors discuss the indications and describe the surgical technique for this

OBJECTIVE: To study advantages of Pemberton acetabuloplasty for the treatment of developmental dislocation of hip joint.
METHODS: Among 106 children (116 hips) with development dislocation of hip joint treated by Pemberton acetabuloplasty, 19 patients (29 hips) were boy, and 87 patients (87 hips) were girl, 46 hips were left side, 50 hips were right side and 20 hips were double side, ranging in age from 18 months to 13 years, with an average of 7 years and 3 months. All the 116 hips were treated with Pemberton acetabuloplasty, or "complete operation", which included the cutting of adductor an

Since 1986, the author has been performing a modified Pemberton acetabuloplasty (MPA) with the deliberate aim of producing a greenstick fracture at the sciatic notch. This procedure was performed in 16 girls representing 17 hips. Nine hips (eight patients) were dislocated. Eight hips (eight patients) had residual or primary acetabular dysplasia. In all dislocated hips and in a teratologic sub-luxation, the MPA osteotomy was performed concomitantly with an open reduction and a femoral shortening. All femoral heads remained well covered, with one exception. One hip had a type III avascular necro

The authors analyze the complications of acetabuloplasty in the treatment of CHD during the growth age, evaluating the incidence and the possible causes, that emerged from a review of cases at the Rizzoli Orthopaedic Institute including 238 patients treated from 1970 to 1986 using different methods. The techniques adopted were: acetabuloplasty for lowering in 100 cases, Salter osteotomy in 30 cases, triple pelvic osteotomy in 48 cases and acetabuloplasty according to the Chiari method in 50 cases. Complications of various types were observed in 9.6% of the cases after a mean follow-up of 5 yea

Although 59% of children with Perthes' disease have a good result without treatment, poor results are common when the disease has onset in older children. Femoral osteotomy at this age will control the disease but may leave shortening and a persisting limp if there is insufficient capacity for remodeling and growth. Twenty children aged greater than 8 years with early disease were treated by lateral shelf acetabuloplasty. The results are compared with those in 14 children for whom no treatment had been advised. Lateral shelf acetabuloplasty has improved early outcome for these children as comp

The paper is devoted to the surgical treatment of 99 senior children (103 joints) and adolescents with dysplastic deforming coxarthroses and pathologic dislocations and subluxations of the hip, namely to the reconstruction of the hip joint by means of creation of acetabuloplastic roofs making use of the migrating osteomuscular flap graft taken from the upper flaring portion of the ilium with the anterosuperior bone and the tailor's muscle attached to it, which has allowed to reduce the terms of the treatment, to interrupt the phenomena of coxarthrosis that are present, to improve the blood sup

Shelf acetabuloplasty is an accepted treatment for Perthes' disease. Favourable outcomes have been reported. This study aimed at determining whether changes in femoral head subluxation after shelf acetabuloplasty occur and whether they predict outcome. We assumed that reduction of subluxation after shelf acetabuloplasty would improve the long-term outcome. During follow-up, growth presented as a source of error when comparing the amount of absolute subluxation. Correction for growth was achieved by calculating a subluxation index. This index showed a significant reduction during follow-up. The

The radiographic course of 101 hips with residual dysplasia treated with roof plasty combined with intertrochanteric varus derotation osteotomy using the osteotomy wedge as a roof graft after Mittelmeier were reviewed. The average followup period was 8.8 years. The acetabular angle was improved by an average of 16 degrees (postoperative mean, 19 degrees; average at followup, 18 degrees). The center edge angle also was improved by 16 degrees and was stable at 25 degrees average at followup. The neck shaft angle, abnormal in 70% of hips preoperatively, was reduced by the varus osteotomies to a m

Although head containment is extremely important for the development of the acetabulum and femoral head, there are debates about conservative and surgical treatment. Shelf acetabuloplasty is an effective means of treatment as regards the coverage of the femoral head within the acetabulum, which is the most important issue for the normal development of the hip joint. Nineteen hips of 18 patients were evaluated radiographically using the acetabulum-head index of Heyman and Herndon and the center-edge angle for containment. It was shown that postoperatively both indices improve to increase the co

The surgical treatment of Perthes' disease by femoral or innominate osteotomy is not as effective in those over the age of eight years as it is in the younger child. This has prompted the search for other types of management in those who are older. The preliminary results of the use of a lateral shelf acetabuloplasty for such cases have shown encouraging results at two years. The concern with such an operation is that it might interfere with the growth of the outer aspect of the acetabulum and so prejudice the long-term outcome. We describe a review at maturity of 26 children presenting with e

A case of premature triradiate cartilage closure secondary to a Gill acetabuloplasty performed at age 14 months is reported. The deficiency in acetabular development and failure of the pelvis to grow to its anticipated height is documented. A proximal femoral redirectional osteotomy and an innominate osteotomy performed near maturity improved femoral head coverage and hip biomechanics. Acetabuloplasties performed adjacent to the triradiate cartilage typically do not have any adverse effect on its function. Despite this low risk of injury to the triradiate cartilage after a Gill or Pemberton ac

Pedicled iliac crest epiphysis transplantation was performed after resection of the outer portion of the acetabulum in 30 puppies 3 months old. The best joint congruency was obtained when using a vertical pedicled graft, the surface of which developed into normal articular cartilage through metaplasia, and presented normal cartilage growth by means of conservation of the epiphyseal vessels. The results of this experiment enable the authors to propose a new surgical technique in the treatment of certain difficult dysplastic conditions in the hip.

INTRODUCTION/OBJECTIVES: Developmental dysplasia of the hip is associated with inferior clinical outcomes when treating concomitant intra-articular pathologies. Shelf acetabuloplasty has been proposed as a less invasive alternative to periacetabular osteotomy for developmental dysplasia of the hip; however, there is a lack of biomechanical studies regarding its efficacy. We hypothesized that a cadaveric dysplasia model would cause increased range of motion and lateral femoral head translation compared to native, and that shelf acetabuloplasty would restore native biomechanics.
METHODS: Ten ca
