Genetic and Biochemical Characterization of AXC-2 from Achromobacter ruhlandii.

Achromobacter spp. are intrinsically resistant to multiple antibiotics and can also acquire resistance to those commonly used for the treatment of respiratory infections, especially in patients with cystic fibrosis. The aim of this study was to perform the genetic and biochemical characterization of AXC-2 from A. ruhlandii and to analyze all available AXC variants. Steady-state kinetic parameters were determined on a purified AXC-2 enzyme. It exhibited higher catalytic efficiencies towards amino-penicillins and older cephalosporins, while carbapenems behaved as poor substrates. Phylogenetic an

Pneumonia due to a Rare Pathogen: Achromobacter xylosoxidans, Subspecies denitrificans.

Achromobacter xylosoxidans, subspecies denitrificans, is a gram-negative rod recently implicated as an emerging cause of infection in both immunosuppressed and immunocompetent populations. Few cases are reported in literature involving multiple body systems. Diagnosis depends on cultures of appropriate specimens, and management usually is by administration of appropriate antibiotics (usually agents with antipseudomonal activity). We report a rare case of pneumonia due to infection with this organism, in a patient with preexisting bronchiectasis secondary to chronic aspiration.

Recurrent Polymicrobial Bloodstream Infection in End-Stage Renal Disease: Co-Infection with Klebsiella pneumoniae, Enterobacter cloacae, Achromobacter xylosoxidans, and Achromobacter denitrificans in a Hemodialysis Patient.

Polymicrobial infections in hemodialysis contribute to increased morbidity and mortality. We report a patient who was readmitted due to infection with multiple pathogens and was treated with favorable outcomes. A 51-year-old male with diabetes mellitus, hypertension, and end-stage renal disease was evaluated in the emergency room following fever and chills observed during hemodialysis via a right chest tunneled catheter 2 days before admission. Two months prior, a left forearm arteriovenous graft had been placed. Initial blood cultures at his dialysis center grew Gram-negative bacilli. Empiric

Discovery, Characterization, and Bioactivity of the Achromonodins: Lasso Peptides Encoded by Achromobacter.

Through genome mining efforts, two lasso peptide biosynthetic gene clusters (BGCs) within two different species of Achromobacter, a genus that contains pathogenic organisms that can infect patients with cystic fibrosis, were discovered. Using gene-refactored BGCs in E. coli, these lasso peptides, which were named achromonodin-1 and achromonodin-2, were heterologously expressed. Achromonodin-1 is naturally encoded by certain isolates from the sputum of patients with cystic fibrosis. The NMR structure of achromonodin-1 was determined, demonstrating that it is a threaded lasso peptide with a larg

Case Report: Septic Pericarditis With Achromobacter xyloxidans in an Immunosuppressed Dog.

A 5-year-old female spayed French Bulldog presented for anorexia and increased respiratory rate. On presentation, she was dyspneic with stridor and increased bronchovesicular sounds. Point-of-care ultrasound identified pericardial effusion. Thoracic radiographs identified pleural effusion, a wide cranial mediastinum, and multifocal unstructured interstitial pulmonary opacities. Bloodwork revealed a moderate leukocytosis characterized by a mature neutrophilia with a left shift, hypoalbuminemia, mildly increased alkaline phosphatase activity, and moderate hypokalemia. Thoracic CT findings reveal

A Pan-Genomic Approach to Understand the Basis of Host Adaptation in Achromobacter.

Over the past decade, there has been a rising interest in Achromobacter sp., an emerging opportunistic pathogen responsible for nosocomial and cystic fibrosis lung infections. Species of this genus are ubiquitous in the environment, can outcompete resident microbiota, and are resistant to commonly used disinfectants as well as antibiotics. Nevertheless, the Achromobacter genus suffers from difficulties in diagnosis, unresolved taxonomy and limited understanding of how it adapts to the cystic fibrosis lung, not to mention other host environments. The goals of this first genus-wide comparative g

Establishment of a chinchilla model of Achromobacter xylosoxidans-induced otitis media.

UNLABELLED: Achromobacter xylosoxidans (Ax) is an emerging opportunistic pathogen associated with chronic respiratory infections, particularly in individuals with cystic fibrosis; however, its role in otitis media (OM) and the mechanisms that underlie middle ear infection remain poorly defined. Despite its initial isolation from middle ear effusions decades ago, no experimental models of Ax-induced OM have been established. Here, we report the development of a reproducible chinchilla model of Ax-induced OM, which, to our knowledge, represents the first experimental model of Ax-induced middle e

Management of Achromobacter xylosoxidans bacteraemia with an atypical antimicrobial resistance pattern.

Achromobacter xylosoxidans is an aerobic, Gram-negative bacillus typically associated with opportunistic infections. Bacteraemia caused by this organism is rare. We report a case of A. xylosoxidans bacteraemia in a young patient who presented with fever and a systemic inflammatory response and was successfully treated with targeted antimicrobial therapy, consisting of ciprofloxacin (a fluoroquinolone) and amikacin (an aminoglycoside), guided by antimicrobial susceptibility testing.

Biological effects of lipopolysaccharide from Achromobacter stenohalis on lymphocytes and macrophages.

The immunopotentiating activities of lipopolysaccharide from Achromobacter stenohalis (A-LPS) were examined. A-LPS was structurally atypical and gave no endotoxin shock in A-LPS-inoculated mice. Analysis in vitro showed that A-LPS was a potent activator of both macrophages and B-lymphocytes. After macrophage stimulation with A-LPS, interleukin-1 (IL-1) secretion, interferon (IFN) production and chemiluminescence (CL) response were induced. A-LPS was a potent mitogen for spleen lymphocytes. However, induction of interleukin-2 (IL-2) secretion in T lymphocytes was not observed. These activities

Enhanced degradation of sulfamethazine by Achromobacter via oxygenation through co-metabolism.

The addition of glucose as a readily biodegradable primary carbon source not only provides additional nutrients and energy but also minimizes the production of toxic intermediates of sulfamethazine (SM2), such as benzoquinone, thereby promoting the proliferation of the strain (with a 227.70 % increase in the OD600 value) and enhancing the SM2 removal rate by 64.45 %. Transcriptome sequencing revealed that co-metabolism induced strain JD417 to significantly upregulate the expression of genes encoding key oxidoreductases (such as sulfanilamide monooxygenase, catechol 1,2-dioxygenase, and acyl-Co

Pathological Thoracic Vertebral Fractures Secondary to Achromobacter denitrificans: A Case Report.

Vertebral fractures remain a diagnostic challenge nowadays. The first and most common diagnosis needed to be ruled out is osteoporosis. Other diagnoses to rule out involve pathological fractures. Pathological fractures are a group of pathologies that result in a spine fracture as part of an underlying disease process that affects the spine. This group includes Paget's disease, tumors, osteomyelitis, and vertebral compression fractures. Fractures secondary to vertebral osteomyelitis are presented as collapsed vertebral bodies secondary to bone destruction and the formation of lytic lesions. Cli

[Cellulitis due to Achromobacter xylosoxidans during bortezomib therapy for multiple myeloma].

Achromobacter xylosoxidans (A. xylosoxidans) is a non-fermentative gram-negative rod. This organism is reportedly a causative pathogen of bacteremia mainly in patients with hematological disorders. However, only one case of cellulitis due to A. xylosoxidans associated with hematological malignancy has been reported. An 80-year-old man developed cellulitis and subsequent bacteremia due to A. xylosoxidans during bortezomib therapy for multiple myeloma. Although his condition was serious enough to require intensive care, he fully recovered with appropriate antimicrobial agents and supportive care

Complicated intra-abdominal infection caused by extended drug-resistant Achromobacter xylosoxidans.

Achromobacter xylosoxidans (formerly Alcaligenes xylosoxidans) is a rare but important nosocomial pathogen. Antibiotic resistance has been increasing during the past decade. A. xylosoxidans may be confused with Pseudomonas spp. but, unlike Pseudomonas spp., this organism has peritrichous flagella. Complicated intra-abdominal infection with A. xylosoxidans has rarely been reported in the literature. This report is of an immunocompetent patient with acute cholecystitis complicated by an intra-abdominal abscess after surgery. Culture of both blood and ascites yielded extended drug-resistant A. xy

Achromobacter xylosoxidans corneal ulcer in a therapeutic soft contact lens wearer.

Achromobacter xylosoxidans is an opportunistic organism that is usually seen in immunocompromised or immunosuppressed patients. It is an aerobic gram-negative rod, often confused with other more commonly seen gram-negative bacteria such as Pseudomonas aeruginosa. The organism is usually sensitive to extended spectrum penicillins such as carbenicillin and usually resistant to aminoglycosides and first generation cephalosporins. We wish to describe a corneal ulcer from A. xylosoxidans that developed in a patient wearing a therapeutic soft contact lens. The patient did not have a preexisting micr

Antibody-guided identification of Achromobacter xylosoxidans protein antigens in cystic fibrosis.

Persistent bacterial airway infection is a hallmark feature of cystic fibrosis (CF). Achromobacter spp. are gram-negative rods that can cause persistent airway infection in people with CF (pwCF), but the knowledge of host immune responses to these bacteria is limited. The aim of this study was to investigate if patients develop antibodies against Achromobacter xylosoxidans, the most common Achromobacter species, and to identify the bacterial antigens that induce specific IgG responses. Seven serum samples from pwCF with Achromobacter infection were screened for antibodies against bacteria in a

Treatment of Achromobacter Ventilator-Associated Pneumonia in Critically Ill Trauma Patients.

BACKGROUND: Achromobacter sp are nonfermenting Gram-negative bacilli (NFGNB) that rarely cause severe infections, including ventilator-associated pneumonia (VAP). Data on the treatment of Achromobacter pneumonia are very limited, and the organism has been associated with a high mortality rate. Thus, more data are needed on treating this organism. OBJECTIVE: To evaluate the treatment of Achromobacter VAP in critically ill trauma patients. METHODS: This retrospective, observational study evaluated critically ill trauma patients who developed Achromobacter VAP. A previously published pathway fo

In vitro activity and clinical use of cefiderocol against Achromobacter xylosoxidans (Review).

Achromobacter xylosoxidans is an emerging non-fermenting gram-negative bacillus that is increasingly associated with multidrug-resistant infections, particularly among vulnerable patient populations, such as patients with cystic fibrosis and lung transplant recipients. The intrinsic and acquired resistance mechanisms of A. xylosoxidans, including efflux pumps, β-lactamase production and altered membrane permeability, limit the effectiveness of standard antimicrobial regimens. Notably, cefiderocol, a siderophore cephalosporin with activity against resistant gram-negative pathogens, has been con
