Acid-Base Imbalance in Pseudohypoaldosteronism Type 1 in Comparison With Type IV Renal Tubular Acidosis.

CONTEXT: Pseudohypoaldosteronism type 1 (PHA1) has been treated as a genetic variant of type IV renal tubular acidosis (RTA), leading to the conception that PHA1 develops hyperchloremic acidosis with a normal anion gap (AG). OBJECTIVE: To delineate the acid-base imbalance in PHA1A (dominant type) and PHA1B (recessive type). METHODS: We conducted the following: (1) a retrospective chart review of our patient with PHA1B, and (2) a literature search of PHA1 cases focusing on acid-base balance. The main outcome measures were the incidence and nature of acidosis, including chloride levels and AG.

Role of brainstem sodium/proton exchanger 3 for breathing control during chronic acid base imbalance.

RATIONALE: The sodium/proton exchanger (NHE) 3 is expressed in brainstem areas with prevalence for central chemosensitivity. Selective NHE3 inhibitors can evoke CO(2) mimetic responses both in vitro and in vivo, demonstrating the functional significance of this pH-regulating protein. Moreover, levels of NHE3 expression are inversely correlated to interindividual differences of baseline ventilation in conscious rabbits. OBJECTIVES: We explored the influence of chronic acid-base disturbances on mRNA levels of brainstem NHE3 in relation to breathing control. METHODS: Alveolar ventilation (Va),

Correction of uremic acid-base imbalance in biofiltration: standardization of the amount of bicarbonate infused.

Beneficial effects of biofiltration on acid-base balance have been described, especially in patients showing poor tolerance to standard hemodialysis. This study was designed to standardize the amounts of bicarbonate to be infused for optimal control of the acid-base balance, without the adverse reactions of symptomatic metabolic alkalosis. In three adult patients (body weight greater than 55 kg) a 300 mEq. bicarbonate infusion achieved normal pre- and post-dialysis plasma levels of bicarbonate and normal pre- dialysis pH. Conversely, in three adolescent patients (body weight less than 40 kg) p

[Paradoxical reaction of intracellular pH values during therapy of acid-base imbalance with arginine hydrochloride].

A well known complication following surgical intervention is metabolic alkalosis. In general this acid-base disturbance is treated in our intensive care units by application of arginine-hydrochloride. In an experimental study on rats in vivo we determined the influence of this substance on the acid-base status of the extra- and intracellular body compartment. It was found that the extracellular bicarbonate decreases but there is nearly no influence on intracellular buffering. The "mean whole body pHi", an overall estimate of the intracellular pH as complementary to the in vivo determined extra

The Effect of the Acid-Base Imbalance on the Shape and Structure of Red Blood Cells.

Red blood cells respond to fluctuations in blood plasma pH by changing the rate of biochemical and physical processes that affect the specific functions of individual cells. This study aimed to analyze the effect of pH changes on red blood cell morphology and structure. The findings revealed that an increase or decrease in pH above or below the physiological level of pH 7.4 results in the transformation of discocytes into echinocytes and causes significant alterations in the membrane, including its roughness, cytoskeleton structure, and the cell's elastic modulus. Furthermore, the study shown

Near-infrared optical responses in feline brain and skeletal muscle tissues during respiratory acid-base imbalance.

The effects of hyper- and hypocapnia on oxidative metabolism were evaluated by near-infrared (NIR) multiwavelength spectroscopy in intact brain and skeletal muscle tissues of the anesthetized cat. A 3-wavelength NIR algorithm was used to monitor cytochrome a,a3 oxidation state, regional blood volume, and tissue oxyhemoglobin and O2 stores simultaneously in brain and muscle in ventilated animals. Incremental hypercapnia was produced in 10 cats by raising arterial pCO2 from 27.0 +/- 1.3 to 95.1 +/- 1.9 mmHg with inspired CO2. Hypercapnia produced progressive increases in cerebral HbO2, blood vol

[The mechanism and clinical significance of decreased anion gap in patients with COPD and acid-base imbalance].

The blood-gas and electrolytes of 427 blood samples from patients with COPD and acid-base imbalance were analysed. The results showed that 14 types of acid-base imbalance were present in patients with COPD and the anion gap decreased in 8 types of acid-base imbalance, especially in respiratory acidosis, metabolic alkalosis and coexistence of both. The anion gap in respiratory acidosis and metabolic alkalosis were 0.8 +/- 6.6mmol/L and 2.1 +/- 7.3mmol/L, respectively. The anion gap in respiratory acidosis with metabolic alkalosis was negative. In all patients the HCO3- increased, and the total

[Effect of acid-base imbalance on the equilibrium of elastase and its inhibitors in rat serum and aorta tissues].

In experiments on the acid-base imbalance modelling (acidosis induced with either lactate or ammonium chloride alcalosis induced with sodium hydrocarbonate) in rats, there were studied elastase activity, alpha-2-macroglobulin and alpha-1 proteinase inhibitor contents in blood serum and tissues of the aorta. The results obtained indicated that, in the models of both acidosis and alcalosis an disbalance between elastase and its inhibitors was observed. However, in NH4CL-acidosis in homogenates of aorta the inhibitors/elastase coefficient decreased at the expense of a reduction in contents of alp

Validity of venous blood gas analysis for diagnosis of acid-base imbalance in children admitted to pediatric intensive care unit.

BACKGROUND: Arterial blood gas (ABG) analysis is the major tool for proper diagnosis and treatment of acid-base imbalance, but the invasive nature of arterial puncture and its possible hazards such as arterial spasm have resulted in a worldwide trend toward less-invasive diagnostic methods including venous blood gas (VBG) analysis. This study aimed to evaluate the validity of VBG and its clinical agreement with ABG in the 10 most common diseases in pediatric intensive care unit (PICU), and to answer how far it can replace the ABG test. METHODS: In a cross-sectional analytical study from Septe

Acid-base imbalances in critically ill obstetric patients: A multicentre retrospective cohort study.

BACKGROUND: Acid-base imbalances are critical indicators of clinical outcomes in intensive care settings, yet their specific impact on critically ill pregnant obstetric patients remains underexplored. This lack of knowledge is particularly concerning given the potential for severe outcomes in this vulnerable population. AIM/OBJECTIVE: The aim of this study was to assess acid-base abnormalities and their association with severe outcomes, including mortality. METHODS: We conducted a retrospective multicentre cohort study between 2006 and 2019 across six high-complexity hospitals in Colombia (t

A genetic origin for acid-base imbalance triggers the mitochondrial damage that explains the autoimmune response and drives to gastric neuroendocrine tumours.

BACKGROUND: Type I gastric neuroendocrine tumors (gNETs) arise from hypergastrinemia in patients with autoimmune chronic atrophic gastritis. According to the classical model, the gastric H+/K+ ATPase was the causative autoantigen recognized by CD4+ T cells in chronic autoimmune scenario that secretes IL-17 and correlates with parietal cell (PC) atrophy, which drives to gastric achlorhydria and increases the risk for gastric neoplasms. However, the mechanism by which the inflammatory response correlates with PC atrophy is not clearly defined. METHODS: Recently, we found that the ATP4Ap.R703C m
