Arthroscopically assisted single tunnel reconstruction for acute high-grade acromioclavicular joint dislocation with an additional acromioclavicular joint cerclage.

PURPOSE: Purpose of this study was to demonstrate that a single tunnel reconstruction of high-grade acromioclavicular (AC) joint instabilities with implants of the second generation is sufficient for stabilisation, especially in combination with an AC cerclage. METHODS: Patients with an acute AC-joint dislocation type Rockwood III-B and V were included. Besides clinical follow-up examination, radiographs were analysed. The functional outcome measures were Constant Score (CS), Taft score (TS), ACJI score and patient's satisfaction. Horizontal instability was evaluated by clinical examination a

The surgical treatment of acromioclavicular joint injuries.

Acromioclavicular joint (ACJ) injuries are common, but their incidence is probably underestimated. As the treatment of some sub-types is still debated, we reviewed the available literature to obtain an overview of current management. We analysed the literature using the PubMed search engine. There is consensus on the treatment of Rockwood type I and type II lesions and for high-grade injuries of types IV, V and VI. The treatment of type III injuries remains controversial, as none of the studies has proven a significant benefit of one procedure when compared with another.Several approaches can be

Intra-articular injection of the acromioclavicular joint.

Intra-articular punctures and injections are performed routinely on patients with injuries to and chronic diseases of joints, to release an effusion or haemarthrosis, or to inject drugs. The purpose of this study was to investigate the accuracy of placement of the needle during this procedure. A total of 76 cadaver acromioclavicular joints were injected with a solution containing methyl blue and subsequently dissected to distinguish intra- from peri-articular injection. In order to assess the importance of experience in achieving accurate placement, half of the injections were performed by an

3D C-arm navigated acromioclavicular joint stabilization.

INTRODUCTION: Surgical treatment options for acromioclavicular joint separations are varied. Frequently, suspension devices (SD) are inserted for stabilization under arthroscopic view. This study investigates the feasibility and accuracy of three-dimensional (3D) digital-volume-tomography (DVT) C-arm navigated implantation with regard to the general trend toward increasingly minimally invasive procedures. MATERIALS AND METHODS: The implantation of a TightRope® suture button system (SD) via a navigated vertical drill channel through the clavicle and coracoid was investigated in 10 synthetic sh

Management of chronic unstable acromioclavicular joint injuries.

UNLABELLED: The acromioclavicular joint represents the link between the clavicle and the scapula, which is responsible for the synchronized dynamic of the shoulder girdle. Chronic acromioclavicular joint instability involves changes in the orientation of the scapula, which provokes cinematic alterations that might result in chronic pain. Several surgical strategies for the management of patients with chronic and symptomatic acromioclavicular joint instability have been described. The range of possibilities includes anatomical and non-anatomical techniques, open and arthroscopy-assisted procedu

Tuberculosis of acromioclavicular joint: a case report.

BACKGROUND: Osteoarticular tuberculosis is a great masquerader presenting in varied forms and in atypical locations, and it is prone to misdiagnosis and missed diagnosis. Isolated acromioclavicular joint tuberculosis has been reported rarely. CASE PRESENTATION: A 19-year-old man presented with a chronic, mild pain, non-healing ulcer in right shoulder. Imaging of the shoulder revealed destruction of the acromioclavicular joint and histopathology confirmed the diagnosis of acromioclavicular tuberculosis. The patient underwent debridement, synovectomy and drainage of the abscess and recovered we

Anatomical observations of the human acromioclavicular joint.

The condition of the acromioclavicular joint (ACJ) is considered to be one factor in the etiology of shoulder impingement syndrome, but there are few supporting morphological data. Fifty-two sides of 35 cadavers were investigated macroscopically and histologically using Safranin O, Fast green, and Weigert's iron hematoxylin staining. The ACJs were classified into three major types depending on the presence or absence of the articular disk. In type 1, the articular disk divided the articular cavity completely (n = 2; 3.8%). In type 2, the articular disk was incomplete and divided the joint cavi

Radiographic evaluation of the acromioclavicular and sternoclavicular joints.

Plain radiography is useful for the initial assessment of suspected disorders of the sternoclavicular and acromioclavicular joints. Other modalities are often required to further assess more complex pathologies involving these joints, however. Ultrasound has been described as a screening tool to assess possible sternoclavicular joint dislocation; however, it is usually used only if CT and MRI are not readily available. It has also been used to confirm intraoperative relocation. Ultrasound has a limited role in the evaluation of the AC joint, where it is most useful to exclude the presence of j

The effects of arthroscopic acromioplasty on the acromioclavicular joint.

Thirty patients with 31 shoulders underwent clinical and radiologic evaluation 5 to 6 years after arthroscopic acromioplasty. The mean age was 51 years. The acromioclavicular (AC) joint was assessed for tenderness on palpation and pain on horizontal adduction of the shoulder. All patients underwent a radiologic examination consisting of an anteroposterior view of the AC joint and bilateral stress views. No patient had superior translation or widening of more than 1 mm of the AC joint on stress views on the unoperated side. On the operated side 12 (38%) shoulders showed signs of instability con

Acromioclavicular reconstruction techniques after acromioclavicular joint injuries: A systematic review of biomechanical studies.

BACKGROUND: Residual horizontal instability after surgical treatment for acromioclavicular joint injuries is seen as a potential cause of suboptimal clinical outcomes. Biomechanical studies have demonstrated that the acromioclavicular capsule/ligaments are the primary restraints for anteroposterior translation. However, limited studies have addressed the biomechanics of a reconstruction of the acromioclavicular capsule/ligaments. The aim of this systematic review was to evaluate the biomechanical role of acromioclavicular capsule/ligament reconstruction techniques after an acromioclavicular jo

Shoulder Conditions: Clavicle and Acromioclavicular Joint Conditions.

The clavicle is the most commonly fractured bone, and the most frequently fractured part of the clavicle is the middle third (ie, midshaft). X-ray usually is the first-line imaging modality for clavicle injuries. Conservative management is preferred for patients with uncomplicated and nondisplaced clavicular fractures. Typically, immobilization should last 4 weeks, then range-of-motion exercises should begin after 4 weeks, with full return to activities by 12 weeks. Distal clavicle osteolysis is a relatively uncommon pathologic bone resorption that occurs with repetitive overhead activities. C

Monoarticular acromioclavicular joint gout: a case report.

Acromioclavicular joint gout is extremely rare. The case presented represents the first case of monoarticular acromioclavicular joint gout reported. It is also the first reported case of acromioclavicular joint gout in an immunosuppressed patient. The diagnostic pitfalls inherent in the evaluation of acute monoarticular arthritis in an immunosuppressed patient are illustrated. Furthermore, the case demonstrates one of the atypical presentations of gout that may occur in renal transplant patients taking cyclosporin A.

Arthroscopic acromioclavicular joint resection. An anatomical study.

Distal clavicle resection has been an effective procedure for treatment of acromioclavicular arthritis. The conventional open surgical technique involves deltoid detachment and reattachment, which may cause postoperative weakness and requires protection during the postoperative period to allow for healing. Arthroscopic acromioclavicular joint resection has the theoretical advantages of no deltoid disruption and a shorter rehabilitation period. The purpose of this study was to compare open versus arthroscopic acromioclavicular joint resection in a laboratory setting. The goals of acromioclavicu

Combined arthroscopically assisted coraco- and acromioclavicular stabilization of acute high-grade acromioclavicular joint separations.

PURPOSE AND HYPOTHESIS: Due to high rate of persisting dynamic posterior translation (DPT) following isolated coracoclavicular double-button technique for reconstruction of the acromioclavicular (AC) joint reported in the literature, an additional acromioclavicular cerclage was added to the procedure. The aim of this study was to evaluate the clinical and radiological results of patients with high-grade AC-joint instability treated with a double TightRope technique with an additional percutaneous acromioclavicular cerclage. METHODS: Fifty-nine patients (6 f/53 m; median age 38.3 (range 21.5-6

Management of Acromioclavicular Joint Injuries: A Historic Account.

There has been a rapid evolution in best practice management of acromioclavicular (AC) joint injuries. AP, Zanca, scapular Y, and dynamic axillary radiographic views provide optimal visualization of the joint and may assess for the presence of horizontal AC instability. Severity of AC joint pathology is classified according to the 6-tier Rockwood scoring system. Over 160 surgical techniques have been described for AC joint repair and reconstruction in the last decade; as a result, determining the optimal treatment algorithm has become increasingly challenging secondary to the lack of consisten

Surgical treatments for acute unstable acromioclavicular joint dislocations.

INTRODUCTION: Surgical treatment is usually recommended for acute, high-grade acromioclavicular joint (ACJ) injuries. A wide variety of surgical techniques exist, and the literature does not strongly support one over the other. In this literature review, we describe and compare the results of different surgical treatments for the management of acute unstable ACJ dislocation and aim to guide surgeons on optimal treatment. MATERIALS AND METHODS: A literature review was performed by searching PubMed, Medline, Cochrane, and Embase databases. Seventeen studies met the inclusion criteria and were a

Arthroscopically Assisted Stabilization of Chronic Acromioclavicular Joint Instability.

UNLABELLED: This video article demonstrates biological and synthetic acromioclavicular (AC) and coracoclavicular stabilization with use of a hamstring tendon graft and a low-profile TightRope implant (Arthrex). The low-profile TightRope reduces soft-tissue irritation due to knot stacks1. The tendon graft is wrapped around the clavicle and the coracoid to avoid weakening of the osseous structures as a result of clavicular and coracoidal tunnel placement2. DESCRIPTION: For this procedure, the patient is placed in the beach chair position. After establishing standard posterior, anteroinferior, a

Acromioclavicular joint injuries: diagnosis, classification and ligamentoplasty procedures.

Acromioclavicular (AC) joint injury is a frequent diagnosis after an acute shoulder trauma - often found among athletes and people involved in contact sports. This injury occurs five times more frequently in men than in women, with the highest incidence in the 20- to 30-year-old age group. Patients usually complain of pain and tenderness over the shoulder, particularly over the AC joint.Depending on the degree of injury, the clavicle may become prominent on the injured site. The original classification was described by Rockwood and Green according to the injured ligament complex and degree and di
