[Early diagnosis of Alzheimer disease].

On of the major challenges for public health in developed countries for the 21st century is that of aging, and in particular Alzheimer's disease. Progress has been made in the development of symptomatic treatment and prevention and early diagnosis of Alzheimer's disease is becoming mandatory. In the present state of our knowledge, no marker for early diagnosis has been described. However, the combined use of clinical observations (behavioral and neuropsychological features) and complementary information (brain morphology and functional imaging, biological data, neurophysiology explorations) co

Genetic susceptibility to Alzheimer disease.

Alzheimer disease (AD) is the leading cause of dementia in the elderly. Less than a decade ago, it was questioned as to whether or not genes were even involved in anything but rare early onset AD. Since that time, using a variety of genetic epidemiological and molecular biological techniques, four loci have been identified that play a role in the genetic susceptibility of AD. AD presents as a prototype of the power of genetic techniques in defining the etiology of a complex disease.

Apolipoprotein E and Alzheimer disease.

Inheritance of specific apolipoprotein E (apoE) alleles determines, in large part, the risk and mean age of onset of late-onset familial and sporadic Alzheimer disease. The mechanism by which the apoE isoforms differentially contribute to disease expression is, however, unknown. Isoform-specific differences have been identified in the binding of apoE to the microtubule-associated protein tau, which forms the paired helical filament and neurofibrillary tangles, and to amyloid beta peptide, a major component of the neuritic plaque. These and other isoform-specific interactions of apoE give rise …

[Erythrocyte transketolases and Alzheimer disease].

Abnormalities of thiamin metabolism have been reported in senile dementia of Alzheimer's type (SDAT). Transketolases (TK) were studied in 21 patients with SDAT, 24 age-matched controls and 12 chronic alcoholics. Erythrocytes were assessed for their TK activity coefficient (TK-AC, normal value 8.4 +/- 12.6%) and affinity for thaimin pyrophosphate (Km TPP, normal value 17.8 +/- 8.3 mumol). Comparison between study groups and controls demonstrated increased TK-AC in SDAT (16.6 +/- 15.7%, P < 0.05) and chronic alcoholism (43.4 +/- 40.6%, P < 0.05), and increased Km TPP (38.3 +/- 25.2 mumol, P < 0.

[Physiopathological bases of Alzheimer disease].

Alzheimer's disease is characterized by the presence of two types of brain lesions: senile plaques and neurofibrillary degeneration. A peptide named beta A4 is the major component of the amyloid substance that constitutes senile plaques. beta A4 is part of a much larger membrane-associated protein known as the amyloid protein precursor (APP) whose gene is on chromosome 21. Mutations on APP gene have been linked to several early-onset AD families, demonstrating the pathogenicity of APP. The amyloid substance, likely neurotoxic, provokes a cascade of biochemical dysfunctions that leads to neurof

[Severe amblyopia and Alzheimer' disease].

A 69-year-old woman developed a rapid and severe visual loss and became nearly blind in a few weeks. As she also presented with memory loss and other disturbances of cognitive functions, with progressive deterioration over one year, a probable Alzheimer's disease was diagnosed. Cerebral CT scan and magnetic resonance imaging were normal. However, clinical and electrophysiological (visual evoked potentials) data indicated an impairment of the primary visual pathways rather than a degeneration of the secondary visual cortex. This case is compared with and discussed in relation to recent reports

Gut Microbiota and Alzheimer Disease.

The hallmarks of Alzheimer's disease (AD) include brain dysfunction and the buildup of amyloid and tau proteins. The onset of dementia is one of the latter symptoms. Imaging diagnostics allowed for the detection of amyloid buildup in the brain 10-20 years before the emergence of overt signs of the disease. The application of imaging diagnostic techniques allowed for this identification. Within the next few decades, the incidence and frequency of this disease are expected to reach epidemic proportions unless measures are done to stop or slow its growth. However, unless action is taken to slow o

Microglial phagocytosis in Alzheimer disease.

Accumulating evidence indicates that Alzheimer disease (AD) is caused by dysregulated microglial phagocytosis. The main risk factor for AD is age, and ageing reduces microglial phagocytosis of amyloid-β (Aβ) plaques, while increasing microglial phagocytosis of synapses and neurons. Most of the known genetic risk for AD can be linked to microglial phagocytosis, including ABCA1, ABI3, ACE, ADAM17, APOE, APP, BIN1, BLNK, CD2AP, CD33, CLU, CR1, CTSB, CTSH, EED, GRN, INPP5D, LILRB2, PICALM, PLCG2, PSEN1, PTK2B, SIGLEC11, SORL1, SPI1, TMEM106B and TREM2. Cholinergic therapy in Alzheimer disease.

This chapter describes how the clinical efficacy of orally administered cholinesterase inhibitors has been demonstrated through placebo-controlled randomized clinical trials leading to regulatory approval worldwide for the symptomatic treatment of Alzheimer disease. Over time, other clinical indications have been found, such as Dementia with Lewy Bodies and Parkinson disease Dementia. The route of administration includes transdermal patches. Side effects predominantly arise from peripheral parasympathetic stimulation. There is hope that drugs acting on acetylcholine release or on muscarinic re…

Gait dysfunction in Alzheimer disease.

Alzheimer's disease (AD) is the most common cause of age-associated dementia and will exponentially rise in prevalence in the coming decades, supporting the parallel development of the early stage detection and disease-modifying strategies. While primarily considered as a cognitive disorder, AD also features motor symptoms, primarily gait dysfunction. Such gait abnormalities can be phenotyped across classic clinical syndromes as well as by quantitative kinematic assessments to address subtle dysfunction at preclinical and prodromal stages. As such, certain measures of gait can predict the futu

Genetic aspects of Alzheimer disease.

BACKGROUND: Alzheimer disease (AD) is a genetically complex disorder. Mutations in 3 genes, presenilin 1, amyloid precursor protein, and presenilin 2, lead to early-onset familial AD in rare families with onset of disease occurring prior to age 65. Specific polymorphisms in apolipoprotein E are associated with the more common, late-onset AD occurring after age 65. In this review, we discuss current advances in AD genetics, the implications of the known AD genes, presenilin 1, presenilin 2, amyloid precursor protein, and apolipoprotein E, and other possible genes on the clinical diagnosis, trea…

[Alzheimer disease: autosomal dominant forms].

We propose a review devoted to the autosomal dominant forms of Alzheimer disease (AD). These forms are the consequences of either PSEN1 mutations (69%), APP mutations (1%), or APP duplication (7.5%), and exceptionally of PSEN2 mutations (2%). The main characteristic of these AD forms is the early age of onset usually before the age of 60 years. The first part of the review focuses on the identification of unusual clinical and neuropathological phenotypes enlarging the AD spectrum: intracerebral hemorrhages caused by severe amyloid angiopathy, spastic paraparesis, Lewy body dementia and excepti…

Genetic aspects of Alzheimer disease.

Alzheimer disease is the most common cause of dementia and represents a major public health problem. The neuropathologic findings of amyloid-beta plaques and tau containing neurofibrillary tangles represent important molecular clues to the underlying pathogenesis. Genetic factors are well recognized, but complicated. Three rare forms of autosomal-dominant early-onset familial Alzheimer disease have been identified and are associated with mutations in amyloid precursor protein, presenilin 1, and presenilin 2 genes. The more common late-onset form of Alzheimer disease is assumed to be polygenic/…

Finger agnosia in Alzheimer disease.

BACKGROUND AND OBJECTIVE: The purpose of this study was to learn if a deficit of finger naming (finger agnosia or anomia) is a sensitive test for Alzheimer disease (AD) and the best means of testing for finger agnosia. METHODS: The subjects were 38 patients with AD and 10 matched normal controls. All subjects were asked to name the thumb, index, and pinky fingers. RESULTS: No control subject had trouble naming any of these fingers, but 37% of the AD subjects did. When AD patients had difficulty with finger naming, they always had trouble naming the index finger. CONCLUSIONS: In the absence …

[Biological markers for Alzheimer disease].

The development of diagnostic markers for earlier and more reliable diagnosis of Alzheimer disease (AD) is essential, particularly because therapeutic medication is available for AD. Cerebrospinal fluid (CSF) is a useful source of diagnostic information. Previously we found the increase of total tau protein in CSF in normal pressure hydrocephalus patients, and others reported the increase also in corticobasal degeneration and frontotemporal dementia. To differentiate the AD from other diseases, further approach was employed and oxidized protein in CSF was investigated. Heat stable fractions of…

Vascular factors and Alzheimer disease.

Vascular risk factors are normally associated with cerebrovascular disease, which may lead to vascular dementia (VaD). Several recent studies suggest that there is increased risk of developing Alzheimer disease when exposed to these same vascular risk factors. In addition to old age, hypertension, peripheral arterial disease, certain types of cardiovascular disorders, diabetes mellitus, and smoking are now considered risk factors for late-onset Alzheimer disease. In this review, we examine several vascular factors and peripheral vascular pathophysiology implicated in Alzheimer disease and sugg…

Molecular genetics of Alzheimer disease.

Epidemiological and individual case studies indicate that genetic factors play a significant role in the genesis of Alzheimer Disease (AD). To date, molecular genetic studies in families multiply affected with AD have identified three genes (Presenilin 1-PS1, Presenilin 2-PS2, and beta-amyloid precursor protein--betaAPP) associated with highly penetrant early onset AD, and one gene (Apolipoprotein E) associated with late onset AD. A fifth potential AD susceptibility locus has been mapped to a broad region of chromosome 12, but the responsible gene defect has not yet been identified. Case-contr…

Cognitive impairment of Alzheimer disease.

The clinical signs of Alzheimer Disease can be quantitated and correlated with neuropathological evidence of the disease. These clinical signs which include amnesia, aphasia and apraxia can be screened for using instruments such as the Mini-Mental State Exam. In addition, each of these cognitive functions can be quantitated using specific tests. Standardized scores are available for a mini mental state but further research is needed to develop normative data for more detailed psychological assessment tests of specific cognitive deficits. The fact that the cognitive impairment predicts the neur…
