Aortic angioscopy assisted thoracic endovascular repair for chronic type B aortic dissection.

BACKGROUND: Non-obstructive general angioscopy is a powerful modality for detecting areas in the aorta with vulnerable plaque, which are difficult to visualize using conventional diagnostic tools such as computed tomography (CT). The aim of the present study was to clarify the efficacy and usefulness of aortic angioscopy in patients with chronic type B aortic dissection scheduled for thoracic endovascular repair (TEVAR). METHODS: Ten patients with chronic type B aortic dissection who underwent elective TEVAR were enrolled. Before starting the stent graft procedure, the aortic intima was obser

Clarification of embolic source in ischaemic stroke by non-obstructive general angioscopy.

BACKGROUND: Aortogenic stroke is an important subtype of embolic strokes, yet lacks a diagnostic method for proactive identification. Non-obstructive general angioscopy (NOGA) is a catheter-based technique to observe spontaneously ruptured aortic plaques (SRAPs), a potential embolic source of ischaemic stroke. OBJECTIVES: This study aimed to identify the embolic source of ischaemic stroke using NOGA. METHODS: From June 2022 to January 2024, 321 consecutive patients with acute ischaemic stroke were hospitalised. 25 underwent emergent mechanical thrombectomy and NOGA. The aortic arch was scree

Angioscopy and Intravascular Ultrasound Assessment of CASPER Rx: A Pilot Feasibility Study.

BACKGROUND: Plaque protrusion (PP) following carotid artery stenting may require postdilation, which carries procedural risks. CASPER Rx dual-layer stent may reduce PP incidence. We evaluated the feasibility of dual imaging-guided selective postdilation strategy. METHODS: Ten patients underwent carotid artery stenting with CASPER Rx. Poststenting evaluation used both intravascular ultrasound and angioscopy. Postdilation was performed only when PP was detected by either modality. RESULTS: Angioscopy was safely performed in all cases. While intravascular ultrasound detected no PP, angioscopy i

Serial Changes of Aortic Vulnerable Plaques Observed via Non-Obstructive General Angioscopy.

Stabilization of aortic vulnerable plaques has not been fully elucidated. Non-obstructive general angioscopy (NOGA) is a novel method for the detailed evaluation of atheromatous plaques in the aortic intimal wall. A 57-year-old man presenting with acute myocardial infarction underwent percutaneous coronary intervention (PCI). NOGA was performed for the evaluation of aortic atherosclerosis, and vulnerable puff-chandelier plaques in the aortic arch were identified. After a strictly controlled low-density lipoprotein cholesterol lowering therapy with a strong statin for 8 months after the primary

Association between aortic thrombi detected using non-obstructive general angioscopy and atrial fibrillation.

Atrial fibrillation (AF) is an independent risk factor for stroke and systemic embolism. Cardiogenic and aortogenic emboli are causes of stroke or systemic embolism. Non-obstructive general angioscopy (NOGA) can be used to diagnose aortic intimal findings, including thrombi and atherosclerotic plaques, but little is known about NOGA-derived aortic intimal findings in patients with AF. This study focused on aortic intimal findings in patients with AF and evaluated the association between AF and aortic thrombi detected using NOGA. We enrolled 283 consecutive patients with coronary artery disease

Detection of vulnerable atherosclerotic plaque during thoracic endovascular aortic repair using nonobstructive angioscopy.

An angioscope was used to observe the intima of the aorta in an 82-year-old patient who had undergone thoracic endovascular aortic repair. The aortic angioscopic findings showed vulnerable plaques from the ascending aorta to the aortic arch that had not been visualized using preoperative computed tomography. After deploying a stent graft from zone 1 to zone 4, the proximal edge of the stent graft was adjacent to the ruptured plaque with mixed thrombi. In spite of these findings, the patient had an uneventful recovery. Angioscopy may have the potential to stratify the risk of thoracic endovascu

Visualization of left common carotid artery injury by glass using postmortem virtual angioscopy.

A deceased man in his 50 s was found with his neck over a broken glass door frame, with blood around the body. A non-contrast postmortem computed tomography (PMCT) scan revealed subcutaneous hemorrhage, temporal bone fracture, and cerebral contusion. Also, wounds extending from the anterior to posterior neck and the presence of air in the cervical vessels suggested cervical vascular injury. A virtual angioscopy image reconstructed from PMCT angiography data revealed a ruptured left common carotid artery and allowed accurate measurement of the injury. This case demonstrates the effectiveness of

Clinical significance of the presence of puff-chandelier ruptures detected by nonobstructive aortic angioscopy.

OBJECTIVE: This study aimed to investigate the prevalence and prognostic significance of atherosclerotic aortic plaques (AAPs) or specific AAP types detected by nonobstructive angioscopy (NOA) in patients who underwent percutaneous coronary intervention (PCI). BACKGROUND: Although recent studies have reported the presence of various patterns of AAPs, identified by NOA, the clinical significance of the presence of AAPs remains elusive. METHODS: In this retrospective, multicenter cohort study, a total of 167 patients who underwent PCI and intra-aortic scans with NOA were studied. The associati

Angioscopy-guided placement of balloon-expandable stents in the treatment of experimental carotid aneurysms.

This study was designed to assess the efficacy of a metallic, rigid, balloon-expandable stent (Palmaz-Schatz) for occlusion of an aneurysm. First, a glass model of two types of aneurysms was constructed, one simple lateral aneurysm and one lateral aneurysm with a side branch. After placement of the stent, the flow towards the distal, post-aneurysmal part of the glass tube was increased with immediate reduction of the flow towards the aneurysm. Beside some turbulences around the struts of the stent there was some inflow of dye into the aneurysm with partial and delayed filling of the aneurysm a

Percutaneous fiberoptic angioscopy of the left ventricle in patients with dilated cardiomyopathy and acute myocarditis.

Left ventricular luminal changes were examined by percutaneous fiberoptic angioscopy in 13 patients with dilated cardiomyopathy and in four patients with acute myocarditis. Angioscope-guided endomyocardial biopsy was also performed in six patients with dilated cardiomyopathy and in two with acute myocarditis. A balloon-tipped guiding catheter (9F) was introduced through the right femoral artery into the left ventricle, the balloon was inflated, and a 1.6 or 4.3F fiberscope was introduced through the catheter into the ventricle so as to locate the fiberscope tip at the tip of the catheter shaft

The war against error: a 15 year experience of completion angioscopy following carotid endarterectomy.

BACKGROUND: A policy of intra-operative transcranial Doppler (TCD) and completion angioscopy was previously associated with virtual abolition of intra-operative stroke (apparent upon recovery from anaesthesia) following carotid endarterectomy (CEA). The aims of this study were to determine whether the prevalence of technical error has diminished with experience and whether our monitoring/quality control policy was still associated with low rates of intra-operative stroke 20 years after its introduction. METHODS: Retrospective review of four consecutive cohorts of 400 patients undergoing CEA b

Virtual intra-arterial angioscopy (VIA) of the carotid artery based on helical CT data.

The purpose of this study was to visualize both the vessel wall and atherosclerotic plaques in virtual intra-arterial angioscopy (VIA) based on helical CT data sets. To achieve this in vitro, the optimal reconstruction threshold of the vessel wall was determined to be 56.4% of the maximum enhancement. Using this threshold, 20 patients suffering from symptomatic carotid disease were examined in a helical CT scanner. The degree of stenosis was defined using the North American Symptomatic Endarterectomy Trial (NASCET) criteria and compared with results from digital substraction angiography (DSA).

Clinical application of angioscopy during carotid endarterectomy for patients with internal carotid artery stenosis.

The accuracy of angioscopy in detecting atherosclerotic changes, such as plaque, ulcer, and mural thrombus, in the extracranial cerebral arteries was evaluated during carotid endarterectomy by comparison with angiographic and operative findings. Ten patients with internal carotid artery stenosis underwent intraoperative angioscopy during surgery. After clamping the carotid bifurcation, intra-arterial atherosclerotic lesions were observed with an angioscope (0.8 or 1.4 mm outer diameter) inserted through a small incision in the common carotid artery. Angioscopic findings correlated well with bo
