Management of teeth with open apices remains technically challenging because conventional apexification techniques may be associated with prolonged treatment, multiple clinical visits, difficulty in controlling apical barrier placement, and the risk of extrusion of biomaterial beyond the apical foramen. This technical report involves capturing the three-dimensional internal morphology of the prepared root canal using a light-body silicone impression, reproducing the canal configuration in heavy-body putty, fabricating a mineral trioxide aggregate (MTA) or Biodentine plug within the negative re

Immature permanent teeth with necrotic pulps present significant therapeutic challenges due to thin dentinal walls, wide-open apices, and poor fracture resistance. Apexification has traditionally been used to induce apical closure, but it does not promote further root development. Regenerative endodontic procedures (REPs) aim to restore biologic vitality and continue root maturation. This systematic review synthesizes randomized controlled trial (RCT) evidence comparing clinical and radiographic outcomes of REPs and apexification in young patients. A comprehensive search of PubMed, Cochrane Li

BACKGROUND/AIM: The evidence related to the superiority of the recommended treatment protocols for necrotic immature permanent teeth is unclear. We aimed to compare the clinical, radiographic, and overall success rates of apexification and regenerative endodontic treatment (RET), assess the methodological quality, and assess the strength of evidence for outcomes.
METHODS: A search was performed in PubMed, Scopus, LILACS, EMBASE, Web of Science, and the Cochrane Library. The titles, abstracts and full texts of systematic reviews (SRs) comparing RET and apexification were screened. The quality

BACKGROUND: Traumatic tooth avulsion requires an urgent intervention to replant the tooth. Prolonged post-injury dry extra-oral conditions worsen the prognosis and increase the risk of root resorption. Fluoride has the potential to delay replacement resorption. Calcium silicate cements (CSC) are used to seal the root canal system and to stimulate periapical regeneration in immature open apex teeth (apexification). This report suggests the application of a novel fast-setting CSC with fluoride for apexification in an attempt to hinder root resorption.

Teeth having open apices in the absence of a natural apical constriction pose several challenges during conventional endodontic treatment, especially when establishing an apical seal. Treatment is further challenging when these teeth are associated with large periapical or cystic lesions having resulted in expansion and/or thinning of adjacent cortical plates. Although surgical intervention is commonly adopted in the management of such cases, a conservative treatment option offering equally good prognosis is always desirable. This case series describes 3 cases: a 39-year-old woman, a 45-year-o

AIM: To evaluate the time-dependent alterations in fractal dimension (FD) of the periapical bone in IPTs treated with calcium hydroxide (CH) apexification, mineral trioxide aggregate (MTA) apexification, or regenerative endodontic treatment (RET). METHODS: A total of 70 teeth were divided into three groups (CH group: n = 25, MTA group: n = 22, RET group: n = 23) and retrospectively evaluated. In each case, the FD value of the periapical bone was calculated using fractal analysis (FA) on pre-treatment and 1-year post-treatment periapical radiographic images. RESULTS: In all the groups, the FD v

Pulp necrosis is the most common complication following dental trauma and is often associated with apical periodontitis. The management of these teeth is challenging in terms of large root canals, open apices, thin dentinal walls, and short roots. Over decades the conventional treatment for these teeth was calcium hydroxide apexification, a time-consuming procedure despite high success rates. Subsequently after the introduction of mineral trioxide aggregate and hydraulic calcium silicate materials single visit apical plug procedures became increasingly practiced with comparable success rates t

Apexification with calcium hydroxide is associated with certain difficulties, such as longer treatment time, risk of tooth fracture and incomplete calcification of apical bridge. Mineral trioxide aggregate (MTA) is an alternative material that can be used for apexification of open apices due to its biocompatibility, non-mutagenicity, non-neurotoxicity, regenerative abilities, and good sealing properties. This case report demonstrates application of MTA apical plug and anatomic post and core for the reconstruction of maxillary central incisor. The patient was recalled after six months and no co

BACKGROUND: Segmental root developmental arrest (SRDA) is an uncommon developmental pattern in immature permanent teeth with apical periodontitis, in which a separated apical segment continues to develop independently of the main root. This retrospective study evaluated the frequency, clinical features, and factors associated with SRDA after regenerative endodontic procedures (REPs) or apexification.
METHODS: A total of 103 immature permanent teeth with apical periodontitis treated with REPs or apexification were retrospectively reviewed. Clinical records and radiographs were assessed for SRD

Early loss of immature permanent teeth due to pulpal necrosis secondary to trauma can have dire consequences for a child's growth and development. The treatment alternatives include surgical endodontics, traditional calcium hydroxide apexification, and mineral trioxide aggregate (MTA) apexification. These options pose potential complications, including: arrest of root development; weakened dentinal walls; and increased potential for fracture. Revascularization of the dentin-pulp complex is a new approach that involves disinfecting the root canal system followed by tissue repair and regeneratio

Occasionally, an orthodontic patient will accidentally traumatize a maxillary anterior tooth before or during orthodontic treatment. In some situations, the trauma will be substantial and avulse the tooth. In other accidents, the tooth may not avulse, but the pulp becomes nonvital. If the pulp is devitalized, and the root has not fully formed, the apex of the root canal may be wide. In this situation, the endodontist may recommend apexification procedures to help close the apex before conventional obturation of the root canal. If the patient is currently undergoing orthodontic movement of the

An electronic apex locator was used to determine length and control apical bridge formation in the treatment of teeth with incomplete root formation. Measurements were taken in 10 teeth in which apexification therapy had just started and in 11 teeth where the formation of the apical barrier was considered to be complete. In the beginning of the apexification therapy, when the apical foramen was still open, endometric measurements gave incorrect results in all instances. After apical closure had occurred and definite obturation of the root canal seemed possible, the apex locator in all teeth ga

The present study reported on the case of a 10-year-old male patient with four second premolar teeth undergoing regenerative endodontic procedures (REPs) for periapical periodontitis due to abnormal central cusp fracture, and the follow-up over 7 years. Annual follow-up clinical and radiographic examinations were performed to assess the effectiveness of treatment. After the initial RPEs, the apical inflammation of teeth #15 and #45 disappeared and their roots continued to develop. However, teeth #25 and #35 exhibited different signs of inflammation and were treated with calcium hydroxide apexi

BACKGROUND: Severe damage to the tooth, periodontal ligament, and pulpal tissue often occurs with intrusive luxation, and outcomes are quite unpredictable because of the variable ways in which the injury may occur, which influences both the choice of treatment and prognosis.
CASE REPORT: This case presents a novel method for the management of an intruded permanent maxillary central incisor with an immature apex. At first, watchful waiting for spontaneous re-eruption was the chosen treatment option. A palatal gingivectomy was performed and endodontic access was established, following which, an
