Hansen's disease, commonly known as leprosy, is an infectious disease caused by Mycobacterium leprae. Being rare in developed countries, it is an increasingly common imported disease due to the migratory flow from countries where it is endemic. We present the case of a 21-year-old man who went to the emergency department with complaints of additive polyarthralgia involving large joints, papules, and erythematous plaques on the limbs with bullae and central necrosis and fever with chills for one week. Skin biopsy was performed revealing neutrophilic infiltrate with perineural granulomas. The ba

BACKGROUND: Tuberculosis (TB) is a prevalent infectious disease that infects about a quarter of the world’s population and has become one of the leading causes of death in the world’s population. Early diagnosis and treatment are crucial for preventing the spread of the disease and improving the patient’s prognosis. Mycobacterium tuberculosis microscopy can provide a basis for TB disease diagnosis, the accuracy of which is influenced by the doctor’s experience. With the development of artificial intelligence technology, deep learning can provide automated bacteriological diagnostic assistance

A 40-year-old male with history of HIV infection was admitted to the hospital with a one-month history of productive cough, vespertine fever, night sweats, loss of appetite and unintentional 10-Kg weight loss. Physical exam was remarkable for cachexia. Blood tests revealed a CD4+ T lymphocyte count of 23 cells/mm3 and HIV viral load of 837,678 copies/ml. Bacilloscopies were positive. Chest computed thomography (CT) showed multiple mediastinal lymph nodes, signs of esophagomediastinal fistula, pericardial effusion and multiple micronodular pulmonary opacities. Endoscopy (EGD) revealed a 10 mm d

OBJECTIVES: Acid-fast bacillus (AFB) detection by microscopy remains a cornerstone of laboratory diagnosis for mycobacterial infections, despite the limitations of variable sensitivity, labor-intensive workflows, and interobserver variability. Recent advances in artificial intelligence, particularly deep learning, offer promising solutions to automate and augment AFB detection on microscopy images.
METHODS: A systematic review was conducted in accordance with Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines, with protocol registration in PROSPERO (registration No.

Feline tuberculosis, involving members of the Mycobacterium tuberculosis complex (MTBC), presents ongoing diagnostic and public health challenges. Mycobacterium orygis, an emerging and genetically distinct MTBC member, is increasingly recognized for its wide host range, including humans, domestic livestock, and wildlife, particularly in South Asia. We report the first documented case of fatal pulmonary tuberculosis caused by M. orygis in a domestic cat. A 5-month-old stray domestic shorthair cat in India presented in profound shock with severe, rapidly progressive respiratory distress. Thoraci

Early diagnosis of leprosy remains primarily clinical, based on recognition of skin lesions with altered sensation and peripheral nerve involvement. However, complementary tests play crucial roles in confirming uncertain cases, guiding operational classification, detecting subclinical neural involvement, and distinguishing late reactions from relapse. This review synthesizes the practical application of diagnostic methods available to dermatologists. Slit-skin smear microscopy demonstrates high specificity but limited sensitivity in paucibacillary forms, while histopathology reveals the charac

Leprosy is a rare chronic infectious disease that can be challenging to diagnose in non-endemic settings due to limited clinical experience among physicians and its marked clinical heterogeneity. We report the case of leprosy in a Nordic patient with an extensive travel history who presented with cutaneous lesions, nasal congestion, and peripheral neurological symptoms. Although the histopathological findings were not fully specific, modified Ziehl-Neelsen staining revealed numerous acid-fast bacilli within the tissue samples. Mycobacterium leprae was subsequently confirmed by molecular analys

The present report describes a case of tuberculous otitis media in a young Brazilian male recently diagnosed with pulmonary tuberculosis who presented with painless unilateral purulent otorrhea and hearing loss for several months. He had received several antibiotic regimens without a clinical response. An acid-fast bacilli test and polymerase chain reaction test from an ear secretion confirmed the diagnosis. Despite its rare occurrence, this case is very similar to others reported elsewhere and reinforces the relevance of investigating this infection in patients with chronic otitis media refra

Mycobacterium bovis and Mycobacterium tuberculosis are the most relevant among pathogenic mycobacteria, both belonging to the M. tuberculosis complex (MTC). Samples of blood, liver, spleen, kidneys, lungs and caseous tubercles were collected from a free-ranging juvenile black capuchin monkey (Sapajus nigritus) showing non-specific signs of illness. Macroscopic findings included emaciation, a caseous lesion in a tooth and gingiva, disseminated nodules in both lungs and left kidney parenchyma and caseous nodules on the pleura and mesentery. The lesions suggested MTC infection, a diagnosis subseq

INTRODUCTION: Anti-Neutrophil Cytoplasmic Antibodies (ANCA) are auto-antibodies directed to intracellular components of neutrophils and used to be considered as present almost exclusively in granulomatous vasculitis. Recently, these auto-antibodies have been found in other autoimmune disorders as well as infectious diseases.
MATERIALS AND METHODS: We studied patients with leprosy confirmed by bacilloscopy and/or skin biopsy, in reaction phase from the Ambulatório de Hanseníase do Hospital Universitário Professor Edgar Santos. ANCA and Antinuclear antibodies (ANA) were determined by indirect i

This study had the aim of determining the coefficient of leprosy detection among the student population of the municipality of Buriticupu, State of Maranhão. An active case search method was used, covering a population of 14,653 students, in 53 schools within the municipal network. Clinical examination gave rise to a leprosy diagnosis for 20 students, which represents a detection coefficient of 13.6/10,000 students. Cutaneous lymph bacilloscopy was performed on all cases, and it was positive in two samples. The predominant clinical form was the indeterminate form, with 12 cases (60%), followed

An exhaustive search for the clinical records of patients diagnosed with tuberculous disease was done in the hospitals of the area under study, which involves 392,000 population. During the years 1992, 1993 and 1994. There were included: 1) patients who had positive bacilloscopy and/or positive Lowenstein's culture in any specimen: 2) patients younger than 35-years-old who had pleural effusion, significant Mantoux and adenosine deaminase (ADA) over 47 U/I in the pleural effusion. In total 814 patients remained in the study with an average age of 38.39(19.39 DE) in 1992, 39.02 (20.04 DE) in 199

Overlapping clinical manifestations of Mycobacterium tuberculosis (MTB) detection and diseases caused by non-tuberculous mycobacteria (NTM) pose significant diagnostic challenges in tuberculosis-endemic settings. Conventional methods often fail to provide a discriminated diagnosis and/or detect co-detections; however, molecular assays enable precise species-level identification and simultaneous detection of multiple pathogens, thereby enabling detection of multiple mycobacterial species, including co-detection patterns. The purpose of this study is to evaluate the utility of molecular methods

BACKGROUND: Drug-resistant tuberculosis remains a major public health challenge in Ethiopia. Delayed sputum culture conversion prolongs infectiousness and worsens treatment outcomes, yet evidence on its timing and predictors is inconsistent.
OBJECTIVE: To estimate the pooled time to sputum culture conversion and identify its predictors among drug-resistant tuberculosis patients in Ethiopia.
METHODS: A systematic search of PubMed, Google Scholar, Embase, Scopus, Web of Science, and Ethiopian university repositories was conducted for studies published between January 2000 and March 2026. Data

Mycobacterial spindle cell pseudotumor is a rare manifestation of atypical mycobacterial infection. The process consists of a tumor-like proliferation of bland, spindle-shaped histiocytes that contain acid-fast-positive mycobacteria. Most patients are immunocompromised, and the lymph nodes are the most commonly involved site. The presentation of this process as an isolated aggressive osseous lesion is rare and conjures up a wide differential diagnosis, including neoplastic conditions. Accurate diagnosis of this entity is essential for proper management. We report a 79-year-old man with hypogam

The aim of our study was to determine whether patients with pulmonary tuberculosis may have subclinical forms of urogenital tuberculosis. Between 2011 and 2012, a prospective study was conducted. Basic demographic parameters were recorded and the following investigations were performed: direct bacilloscopy of sputum, evaluation of affected lung fields and presence of cavities on chest X-ray, Mantoux tuberculin skin test II, and interferon gamma release assay. Culture and molecular methods for Mycobacterium tuberculosis in urine were performed. In cases with a positive urine test, an ultrasound

RATIONALE: Mycobacterium tilburgii is the causative agent of disseminated non-tuberculosis mycobacterial infections in individuals who are immunocompromised, including those with human immunodeficiency virus infection. Owing to its non-culturable nature, identification of M. tilburgii relies solely on genetic analysis, making reports of M. tilburgii infections rare. We report a case of disseminated M. tilburgii infection complicated by a mixed pulmonary non-tuberculosis mycobacterial infection in a patient with acquired immunodeficiency syndrome.

Leprosy continues to be reported in India despite ongoing control efforts. We conducted a two-year retrospective study (May 2023-2025) at a tertiary-care microbiology laboratory in Hyderabad, India, to describe laboratory-confirmed cases of leprosy. Samples from clinically suspected cases were examined using Modified Ziehl-Neelsen staining, and only smear-positive cases were included. Demographic details, treatment follow-up status, and district of residence were reviewed, and spatial distribution was visualized using spot mapping of 93 samples processed, 24 were smear-positive.
