In vivo rabbit models that deliver mechanical impacts to knee cartilage are important for studying injury-induced osteoarthritis, but variability in impact characteristics limit the repeatability of the models. In this study, the mechanical impact load delivered to a cadaveric rabbit posterior medial condyle was characterized, with particular emphasis on the influence of placement of the k-wire that stabilized the joint during impact. K-wire position was not prescribed and varied across specimens (7.85 ± 2.28 mm anterior and 2.61 ± 3.47 mm distal to the impact site) and was treated as a source

Forty-three patients with ununited fractures of the scaphoid were treated by open reduction, anterior inlay bone grafting and Kirschner wire fixation. All but one achieved bone union. The one failure was successfully treated with a second anterior inlay bone grafting procedure. The average period of cast immobilization was 7 weeks. The presence of an unstable nonunion did not adversely affect the results. In cases with carpal instability, we used a longer corticocancellous graft to restore a more normal alignment of the scaphoid by opening up the volar cortex. Preoperatively, carpal instabilit

PURPOSE: This study aimed to compare the biomechanical pull-out strength of the three different tension band wiring (TBW) methods employed to fix transverse olecranon fractures on bone models.
METHODS: Three different fixation models were created in groups of seven synthetic olecranon fractured bone models. The first technique was fixed the olecranon with the traditional TBW method. The second technique was fixed the olecranon with a large intramedullary screw TBW method. The third technique was fixed the olecranon with the double-screw TBW method. The pull-out force needed for the failure of

PURPOSE: The treatment of irreducible or severely displaced metacarpal and phalangeal bone fractures is still much debated. The recent development of the bioabsorbable magnesium K-wire is thought to allow effective treatment upon insertion via intramedullary fixation by minimizing articular cartilage injuries without discomfort until pin removal and drawbacks, such as pin track infection and metal plate removal. Therefore, this study investigated and reported the effects of intramedullary fixation with the bioabsorbable magnesium K-wire in unstable metacarpal and phalangeal bone fractures.

Heat generation during insertion of Kirschner wires (K-wires) may lead to thermal osteonecrosis and can affect the construct fixation. Unidirectional and oscillatory drilling modes are options for K-wire insertion, but understanding of the difference in heat generation between the two modes is lacking. The goal of this study was to compare the temperature rise during K-wire insertion under these two modes and provide technical guidelines for K-wire placement to minimize thermal injury. Ten orthopedic surgeons were instructed to drill holes on hydrated ex vivo bovine bones under two modes. The

Intraosseous epidermal cysts (IECs) are rare benign lesions caused by the proliferation of epidermal cells within the bone. The pathogenesis of IEC remains unclear; however, trauma-triggered infiltration of the bone by epidermal elements has been suggested. Here, we present a case of an IEC in the metacarpal bone of the little finger associated with K-wire fixation for treatment of a fifth metacarpal fracture.

BACKGROUND: Shoulder arthroplasty is increasingly utilized to treat shoulder arthritis, cuff tear arthropathy and proximal humerus fractures. Onlay allograft cortical strut augmentation is a common technique used in the management of humeral bone loss, periprosthetic fractures, and during revision surgeries to enhance stability and potentially reconstitute bone. This study investigated the biomechanical performance of three commonly used strut fixation methods used to secure an allograft strut to host diaphyseal bone: suture-tape cerclage, wire cerclage and lag screw fixation.
METHODS: Eight

We compared the outcomes of two groups of patients with scaphoid nonunion treated with arthroscopic cancellous bone grafting. In group 1, K-wires were introduced beforehand and in group 2 after debridement and packing bone grafts in the nonunion site. Our hypothesis was that the percentage of unions would be higher in group 2. We conducted a retrospective, single-centre comparative study of two consecutive series of patients. Seventeen patients in group 1 were operated between 2011 and 2015 and 25 patients in group 2 between 2015 and 2022. At 3 months, there was already a significant differenc

BACKGROUND: Scaphoid nonunion in adolescents is rare, and most treatments include prolonged immobilization and screw fixation. Many studies have shown that Kirschner wires (K-wires) show comparable outcomes in screw fixation in adult scaphoid nonunion. However, few studies have reported K-wire fixation results in the treatment of adolescent scaphoid nonunion. The purpose of this study was to evaluate the clinical and radiologic results after bone graft and K-wire fixation for scaphoid nonunion in adolescents.
METHODS: We retrospectively reviewed 12 adolescent patients, mean age 15.4 (12 to 17

PURPOSE: Closed reduction is commonly used for the treatment of nasal bone fractures. If the fracture is impacted, accurate reduction is difficult. Therefore open reduction is often required.
MATERIALS AND METHODS: Between January 2010 and December 2011, we used a Kirschner wire and a C-arm intraoperatively for accurate closed reduction of impacted nasal bone fractures.
RESULTS: During the follow-up period of 4 weeks to 6 months, all patients had satisfactory cosmetic results.
CONCLUSIONS: The effectiveness of open reduction was obtained without noticeable scarring with our method of closed

Calcaneal avulsion fractures are not uncommon, and they are probably more likely in patients with osteoporosis. Closed manipulation for this type of fracture often fails to achieve acceptable reduction, and open reduction and internal fixation are usually required. However, open reduction and internal fixation with either a lag screw or Steinmann pins do not provide satisfactory fixation in patients with diabetes and elderly patients because of the presence of porotic bone. Levi described a tension band fixation system used to treat a calcaneal avulsion fracture using a simple technique perfor

The aesthetic and functional success of mandibular advancement surgery often depends on the quality of the orthodontic preparation that preceded it. In order to be optimal, this preparation must try to reproduce a class II malocclusion equivalent to the sagittal skeletal discrepancy, to allow a surgical procedure of sufficient amplitude. The aim of this article is to present, thanks to a clinical case, a new mandibular posterior anchorage device, called Abalakov, used in the context of these preparations. The modalities of placement and use of this simple osteosynthesis wire, placed in the ant

We compared two methods of fixation for unstable scaphoid waist nonunions regarding rate of scaphoid union, union time, deformity correction, clinical outcomes (pain, range of motion, and grip strength), and QuickDASH scores. Eighty-nine patients who undergo cancellous iliac bone grafting and internal fixation either with a Herbert screw (n = 46) or multiple Kirschner wires (n = 43) were evaluated. The rate of scaphoid union in the K-wire group was 98% (n = 42) versus 89% (n = 41) in the screw group. The mean union time and duration of surgery for K-wire group versus screw group were 12 versus

A retrospective study was performed to determine the efficacy of a tethering procedure developed to achieve a more rigid fixation and more reliable outcome in patients with refractory dislocation of the temporomandibular joint. The cases of eight patients with dementia and systemic diseases who underwent this technique were reviewed. In these eight patients, the condyles of 13 joints were ligated using wire between screws placed in the eminence and condylar head. Additional screw-wire ligations were applied to reinforce the restraint of movement in five of the 13 joints with suspected uncontro

A surgical technique for treatment of spondylolysis or minimal isthmic spondylolisthesis is described. The authors based their treatment on the Scott technique, which involves placing an 18-gauge stainless steel wire around the transverse process bilaterally and then tightening the wires to each other inferiorly to the posterior spinal process. The modified technique consists of a tension band wire around the posterior spinous process and a 4.5 mm AO cortical screw in the pedicle. A case study of a 20-year old female athletic student is presented to illustrate the effectiveness of the modified
