OBJECTIVE: Pulmonary artery catheterization provides continuous monitoring of hemodynamic parameters that may aid in the perioperative management of patients undergoing cardiac surgery. However, prior data suggest that pulmonary artery catheterization has limited benefit in intensive care and surgical settings. Thus, this study sought to determine the impact of pulmonary artery catheter insertion on short-term postoperative outcomes in a large, contemporaneous cohort of patients undergoing open cardiac surgery compared with standard central venous pressure monitoring.
METHODS: This was an obs

OBJECTIVES: Radiation from cardiac angiography procedures is harmful to patients and the staff performing them. This study sought to investigate operator radiation dose for a range of procedures and different operators in order to investigate trends and optimise dose.
METHODS: Real-time dosemeters (RTDs) were worn by operators for angiography procedures for 3 years. Dose-area product (DAP) and RTD were collected. RTD was normalised to DAP (RTD/DAP) to compare radiation dose and radiation protection measures. Comparisons were made across procedure categories and individual operators.
RESULTS:

OBJECTIVES: This study identified factors associated with risk for cardiac perforation in the setting of atrial fibrillation (AF) ablation in contemporary clinical practice.
BACKGROUND: Cardiac perforation is an uncommon but potentially fatal complication of AF ablation. An improved understanding of factors associated with cardiac perforation could facilitate improvements in procedural safety.
METHODS: Logistic regression models were used to assess predictors of cardiac perforation among Medicare beneficiaries who underwent AF ablation from July 1, 2013 and December 31, 2017. Cardiac perfora

BACKGROUND: The pulmonary artery catheter (PAC) continues to be used for monitoring of hemodynamics in patients undergoing coronary artery bypass graft (CABG) surgery despite concerns raised in other settings regarding both effectiveness and safety. Given the relative paucity of data regarding its use in CABG patients, and given entrenched practice patterns, we assessed the impact of PAC use on fatal and nonfatal CABG outcomes as practiced at a diverse set of medical centers.
METHODS: Using a formal prospective observational study design, 5065 CABG patients from 70 centers were enrolled betwe

Implanted venous access devices (VADs) are being used with increasing frequency in clinical practice. The complication rate is low and VADs are generally accepted as being safe. Catheter fracture is a rare but potentially life-threatening complication that must be recognized and treated promptly. The present report discusses a case in which the clinical presentation led to early recognition of catheter fracture and embolization, and the subsequent safe removal of the VAD.

The therapeutic possibilities in veterinary cardiology have developed rapidly in the past few years. Whereas until recently cardiac intervention in dogs could only be performed by thoracotomy, new minimally invasive techniques are adopted. Procedures like balloondilatation of pulmonic stenosis, coil embolisation of patent ductus arteriosus, pacemaker implantation in symptomatic bradyarrhyhtmia, and palliative balloon pericardiotomy are becoming more and more established. These alternative interventional methods are attractive, because no postsurgical pain and no complications potentially assoc

Infection of a cardiac pacemaker is a rare but serious complication. Percutaneous ablation of the pacemaker and pacing catheter is the only effective treatment. Techniques of extraction of pacing systems have been evaluated but the long term results require analysis. Eighteen patients with infection of cardiac pacemakers underwent extraction of one or more pacing catheters (14 atrial and 20 ventricular) in one same centre. The indication was infection of the pacemaker unit (12 cases) or septicaemia (6 cases) The causal organism was a staphylococcus (aureus: 7 cases, epidermidis: 10 cases, capi

Catheter ablative techniques have introduced an exciting new chapter of interventional therapy for patients with drug-resistant cardiac arrhythmias. A variety of techniques are presently available, and proper choice depends on proper localization of the tachycardia focus and mechanism of the arrhythmia. These techniques should be performed only in laboratories with personnel experienced in invasive electrophysiologic procedures. Over the past 5 years, we have learned that catheter ablation of the AV junction for patients with drug-resistant supraventricular tachycardia has replaced the need fo

BACKGROUND: Foetal cardiac interventions (FCIs) represent a groundbreaking advancement in maternal-foetal medicine, offering therapeutic strategies to address congenital heart defects (CHDs) in utero.
SUMMARY: The indications, techniques, complications, and outcomes of the three most reported FCIs, namely, foetal aortic valvuloplasty, foetal pulmonary valvuloplasty, and foetal atrial septal intervention, were systematically reviewed based on the published literature. The challenges and future directions of this field were also highlighted.
KEY MESSAGES: FCIs open the therapeutic opportunity

More than 50 years after its introduction in clinical practice, the increase in the intensity of care offered by the cardiac intensive care units, the shift in the population of patients treated and the wider availability of circulatory supports, still makes the pulmonary artery catheter (PAC) an essential tool for diagnosis, monitoring and prognosis in patients suffering from cardiogenic shock. In this review, we will discuss how to identify those patients who can benefit most from its use, the configuration and the correct insertion technique of a PAC. A pragmatic guide will also be provided

Ventricular arrhythmias are a common clinical manifestation in patients with cardiac sarcoidosis (CS) and other arrhythmogenic inflammatory cardiomyopathies (AIC). The management of sustained ventricular arrhythmias in these patients presents unique challenges. Current therapies include immunosuppressive, antiarrhythmic agents, and catheter ablation. Significant progress has been made in deciphering the importance of patient selection for ablation, systematic preablation evaluation, and optimal ablation timing, as well as ablation approaches and techniques. In this overview, we discuss the eva

The use of a peritoneal catheter in selected patients, in relation to the congenital heart defect and surgical procedure, may improve postoperative fluid balance and recovery. The peritoneal catheter allows to either drain ascites passively out of the peritoneal cavity or utilize cycles of peritoneal dialysis. However, potential benefits contrast with risk. This article provides a step-by-step guide on how to implant a peritoneal catheter in the operating room after cardiac surgery, or insert it at the bedside in the ICU, to minimize the risk of complications such as bowel perforation, herniat

Since their introduction >2 decades ago, percutaneous catheter-based epicardial mapping and ablation have become widely adopted by cardiac electrophysiologists around the world. Although epicardial mapping has been used for catheter ablation of a wide variety of cardiac arrhythmias, its most common use is for ablation of intramural and subepicardial substrates that give rise to ventricular tachycardia, particularly in patients with nonischemic cardiomyopathy. As such, the subxiphoid percutaneous epicardial approach has emerged as an important adjunct, and, in some cases, is the preferred strat

BACKGROUND: Sarcoidosis is a multisystem granulomatous disease of not sufficiently understood origin. Some patients develop cardiac involvement in course of the disease which is mostly responsible for adverse outcome. In addition to complications like high degree atrioventricular (AV) block or ventricular tachyarrhythmias, there is a certain percentage of patients developing atrial tachyarrhythmias. Data is limited and the role of catheter ablation uncertain. Therefore, we studied sarcoid patients who presented with supraventricular tachyarrhythmias.

BACKGROUND: Cardiac amyloidosis (CA) is associated with increased atrial arrhythmias risk. The efficacy/safety of catheter-based ablation therapy in patients with CA has not been adequately assessed.
METHODS AND RESULTS: All diagnosed CA patients who underwent atrial arrhythmia ablation therapy from 1995 to 2015 were reviewed. Arrhythmia recurrence, NYHA symptoms, and mortality were recorded. A total of 26 patients with CA and atrial arrhythmias were included; there were 7 light-chain (AL), 17 wild-type transthyretin (ATTRwt), and 2 mutated transthyretin (ATTRm) amyloidosis patients in total.

With more and more extreme premature and very low-birth weight babies being resuscitated, umbilical central venous catheterisation is now being used more frequently in neonatal intensive care. The authors present a case of cardiac tamponade following umbilical venous catheterisation in a neonate, an uncommon, yet potentially fatal complication. The patient was diagnosed at the appropriate time by echocardiography and urgent pericardiocentesis proved lifesaving.

Clinical tachycardias are a major cause of morbidity with detrimental effects on quality of life, physical activity and health care costs. Catheter ablation delivered by radiofrequency energy (RFA) has gradually expanded as a therapeutic modality for cure or palliation and is being adapted to address the most difficult to treat tachycardias. The purpose of this paper is to inform decision makers about the current evidence base of RFA through a comprehensive literature review. Of the 968 citations identified through the literature search strategy, 111 studies (11%) met the inclusion criteria. O
