Atrial fibrillation is the commonest arrhythmia observed in hypertrophic cardiomyopathy, and is associated with an acute deterioration in symptoms. Digoxin is the drug of choice in established atrial fibrillation and amiodarone the drug of choice in paroxysmal atrial fibrillation and ventricular arrhythmia. Non-sustained ventricular tachycardia occurs in 20% of patients and is the single best predictor of sudden death in adults. Sustained monomorphic ventricular tachycardia occurs only rarely. The mechanism of sudden death is likely to involve initiating factors such as arrhythmia and peripher

The treatment of hypertrophic cardiomyopathy is first and foremost symptomatic, its aim being to counteract dyspnea, angina pectoris, syncopes and lipothymias, palpitations. In therapy, use is made of beta-blockers, propranolol in particular at moderate or high doses, which are often efficacious but readily cause muscular asthenia. Calcium antagonists, especially verapamil, are a therapeutic alternative, often more efficacious but exposing the patient to the risks of iatrogenic effects which are sometimes serious and may even be fatal. Surgical myomectomy is another alternative, the functional

A 21-year-old female patient with hypertrophic obstructive cardiomyopathy and lentiginosis is described. This entity is known to belong to the LEOPARD syndrome (L-entigines, E-electrocardiographic conduction defects, O-cular hypertelorism, P-ulmonary stenosis, A-bnormalities of genitalia, R-etardation of growth, sensorial D-eafness). The patient underwent surgery for severe obstruction of the left ventricular outflow tract; the postoperative follow-up was uneventful.

This case report describes a young male patient admitted due to worsening of heart failure with recurrent embolic events. He had previously been diagnosed with hypertrophic cardiomyopathy (HCM) and left ventricular systolic dysfunction at other hospitals and was treated irregularly with medications including a beta-blocker and diuretics. On this admission, echocardiography and cardiac MR examination revealed left ventricular myocardial non-compaction combined with HCM. The coexistence of left ventricular hypertrophy and hypertrabeculation is rare, and the diagnosis has been updated according t

The inheritance of hypertrophic cardiomyopathy is autosomal dominant, "sporadic" cases are now recognised to be uncommon. In the past 5 years molecular genetic advances have isolated >100 different mutations in genes encoding sarcomeric proteins. The cardiac morphology and prognosis are linked to the underlying genetic defect, but intra and interfamilial variation suggest polymorphism or mutation of other genes, or environmental factors. Penetrance is age related and negative echocardiographic screening cannot guarantee absence of disease. Currently mutation analysis is not routine in clinical

An abnormal Q wave in the ECG of the elderly calls for differential diagnosis between an old myocardial infarction and left ventricular hypertrophy. The latter may be due to hypertrophic cardiomyopathy, the prognosis of which is good in the elderly. The echocardiogram is the only method which provides the correct diagnosis and helps to avoid incorrect treatment.

In Europe, the term 'hypertrophic cardiomyopathy' (HCM) encompasses both primary, genetic forms and secondary, acquired forms. If echocardiography confirms wall thickening of the left ventricle without a clear connection to pathological stress conditions, a targeted differential diagnosis should be performed. Cardiac magnetic resonance imaging plays a key role in definitive diagnosis. Genetic testing should also be considered for many forms of HCM, especially for the common primary HCM. Stratification with regard to arrhythmia risk is mandatory for all patients with primary HCM. Symptomatic tr

Hypertrophic cardiomyopathy (HCM) is a multifaceted genetic disorder characterized by left ventricular hypertrophy (LVH) in the absence of alternative causes, with an estimated prevalence ranging from 1 in 200 to 1 in 500 individuals. Since HCM was first characterized in 1869, a plethora of pathogenic mutations have been identified, while ongoing research continues to elucidate the various pathophysiological mechanisms present in individuals with HCM. Comprehensive physical examination findings and multimodality imaging techniques have become crucial for accurately diagnosing and risk stratify

Hypertrophic cardiomyopathy (HCM) is the most common inherited cardiomyopathy and causes changes in the cardiac muscle affecting ventricular, valvular, and cellular functions. Because HCM is an inherited disorder, all age groups are affected; however, it commonly presents in adolescents, especially athletes. Many patients are asymptomatic and undiagnosed, putting them at risk for sudden cardiac death. This article describes screening and management of patients with HCM.

Hypertrophic cardiomyopathy (HCM) is the second commonest form of heart muscle disease affecting children and adolescents and is a leading cause of sudden death in young athletes. The aetiology of HCM is heterogeneous in the paediatric population, and includes inborn errors of metabolism, neuromuscular disorders and malformation syndromes. However, most cases of apparently idiopathic HCM in childhood are caused by mutations in cardiac sarcomere protein genes. Patients with metabolic or syndromic HCM usually present in infancy or early childhood, whereas those with neuromuscular disorders are m

Hypertrophic cardiomyopathy has important differences in children compared with adults, particularly with regard to the range of causes and the outcomes in infants. Survival is highly dependent on etiology, particularly in the youngest patients, and pursuit of the specific cause is therefore necessary. The clinical utility of defining the genotype in children with familial hypertrophic cardiomyopathy exceeds that at other ages and has a highly favorable cost/benefit ratio. Although most of the available information concerning treatment and prevention of sudden death is derived in adults, manag

The case of a young primiparous woman with defibrillator-assisted familial hypertrophic cardiomyopathy (HCM) has led us to review the literature on this pathology, which is exceptional because of its scarcity and the originality of the problems encountered. To our knowledge, this is the first observation ever reported of defibrillator-assisted activation during pregnancy in a woman with HCM. Several questions raised from this particular case, e.g. what are the risks caused by pregnancy in these patients, what is the impact of therapeutics, does the activation of an internal defibrillator invol

BACKGROUND: The actual prognosis of hypertrophic cardiomyopathy, a disorder previously thought of to be notorious for an increased risk of untimely death is poorly defined.
OBJECTIVE: The present study describes the findings of patients with Hypertrophic Cardiomyopathy followed for more than nineteen years in a large clinic population.
MATERIALS AND METHODS: A clinic population of 113 patients with Hypertrophic Cardiomyopathy was prospectively studied to assess cardiac mortality in the overall groups and in selected subgroups commonly thought to be at high risk for sudden death. Diagnosis of

Hypertrophic cardiomyopathy is a relatively rare (prevalence approximately 0.2%), primary myocardial disorder with an autosomal pattern of inheritance, characterized by mostly asymmetric left ventricular hypertrophy with myocyte and myofibrillar disarray. To date, about 34 mutations of the beta-cardiac myosin heavy chain gene have been described and shown to have prognostic implications. The disease has an annual mortality rate of 3%, related to both sudden cardiac death and progressive systolic dysfunction. Not only diastolic but also progressive systolic dysfunction with cavity dilatation oc

Hypertrophic cardiomyopathy (HCM) is defined as an idiopathic heart muscle disorder which characterised by the presence of left and/or right ventricular hypertrophy in the absence of a systemic or cardiac cause. At post mortem examination the characteristic histology shows myocyte disarray surrounding areas of increased loose connective tissue. The spectrum of disease is wider than the phenotype which relies on the diagnostic presence of unexplained left ventricular hypertrophy. During the past 5 years, one of the genes which causes HCM has been discovered, several additional loci have been id
