Spontaneous Cerebrospinal Fluid Otorrhea Secondary to Tegmen Tympani Dehiscence: A Case Report and Literature Review.

Spontaneous cerebrospinal fluid (CSF) otorrhea is an uncommon but increasingly recognized condition, particularly in older adults. It most often results from temporal bone defects, such as tegmen tympani dehiscence, which establish abnormal communication between the subarachnoid space and the middle ear or mastoid air cells. Clinical manifestations of CSF otorrhea may range from subtle conductive hearing loss and middle ear effusion to life-threatening recurrent meningitis. High-resolution computed tomography (HRCT) is essential for diagnosis and surgical planning. We are presenting a 66-year-

Transcanal Endoscopic Repair of Cerebrospinal Fluid Otorrhea Secondary to Congenital Inner Ear Anomaly Using Topical Fluorescein.

An eight-year-old female, known to have sensorineural hearing loss due to inner ear anomalies, presented with a history of recurrent attacks of right acute otitis media complicated by bacterial meningitis. Temporal bone computed tomography showed right middle ear and mastoid effusion without bony dehiscence or erosion, with features of common cavity inner ear anomaly on the right side. Given the patient's inner ear malformation, cerebrospinal fluid (CSF) otorrhea was suspected. Further evaluation with myringocentesis and beta trace protein analysis confirmed the diagnosis. The patient underwen

Repair of Tegmen Tympani Defect Presenting with Spontaneous Cerebrospinal Fluid Otorrhea Using the Middle Cranial Fossa Approach.

Spontaneous cerebrospinal fluid (CSF) otorrhea frequently occurs without temporal bone trauma, fracture, surgery, or any identifiable causes. It is usually associated with tegmen tympani defects of the temporal bone in adults. The congenital origin theory and the arachnoid granulation theory have been accepted to explain the tegmen tympani defect. Magnetic resonance imaging (MRI) and computed tomography (CT) are used to show the defects, brain tissue, and the meninges. We recently encountered three cases of spontaneous CSF otorrhea with a defect on the tegmental plate of the temporal bone. Hig

Brainstem herniation into the internal acoustic canal secondary to hydrocephalus in context of spontaneous cerebrospinal fluid otorrhea: report of a novel entity.

INTRODUCTION AND CLINICAL PRESENTATION: The authors report a case of a 5-year-old boy presenting with vision loss, right-sided hearing loss, and facial paralysis secondary to hydrocephalus causing brainstem herniation into the internal auditory canal (IAC) following cerebrospinal fluid (CSF) otorrhea. MANAGEMENT AND OUTCOME: After placement of a ventriculo-peritoneal shunt (VP shunt), the vision and facial palsy improved whilst hearing loss persisted. Imaging demonstrated partial reduction of the herniated brainstem and resolution of hydrocephalus. To our knowledge, this is the first case rep

Case Report: Simultaneous repair of cerebrospinal fluid otorrhea and cochlear implantation in a patient with single-sided deafness and inner ear anomaly.

An-five years old male presented with recurrent attacks of headache and clear nasal discharge. Magnetic resonance cerebrospinal fluid (CSF) imaging demonstrated descent of the cerebellar tonsils, with CSF signal identified within the left middle ear. Temporal bone computed tomography showed the left cochlea was incomplete partition type I. Audiological evaluation revealed profound sensorineural hearing loss in the left ear of the pediatric patient. The surgical approach of mastoidotomy with exploration was utilized in the management of this pediatric case. CSF otorrhea was confirmed and the CS

Control of cerebrospinal fluid otorrhea via C-arm-guided reduction of the zygomatic arch as a part of the temporal bone: interdisciplinary approach to an unusual craniomaxillofacial fracture.

Maxillofacial fractures are often associated with blunt head injuries, of which skull base trauma is a common component. However, most oral and maxillofacial surgeons do not provide definitive management of temporal bone fractures involving the skull base and their sequelae. Persistent cerebrospinal fluid (CSF) leakage that is refractory to conservative measures usually requires surgical closure to decrease the risk of meningitis. In general, reduction of the displaced fragment of the skull base in temporal bone fractures is not considered a priority. We describe an unusual case of craniomaxil

Fallopian canal arachnoid cyst with acute facial nerve paralysis in children: a report of two cases and literature review.

INTRODUCTION: Symptoms induced by arachnoid cysts in the fallopian canal are uncommon, and facial nerve paralysis without cerebrospinal fluid otorrhea is comparatively rarer. METHODS: Herein, we present two cases of arachnoid cysts in the fallopian canal with acute severe facial nerve paralysis and review the relevant literature. RESULTS: The symptoms and imaging findings of these two cases resembled those of facial nerve schwannomas. Cerebrospinal fluid otorrhea occurred upon removal of the arachnoid cyst, and the facial nerve was observed to be separated into multiple filaments or compress…

[Spontaneous defects of the lateral skull base. 2. Etiology and review of the literature].

BACKGROUND: Isolated cases of spontaneous cerebrospinal fluid otorrhea (SCFO) of temporal bone origin with or without meningo(encephalo)cele have been reported. Diagnosis is predicted on a high index of suspicion. Since meningitis may occur secondary to SCFO, optimal therapy with surgical closure of the defect should immediately be performed. This study was undertaken to evaluate the incidence, characteristics and type of SCFO in literature. MATERIALS AND METHODS: We reviewed the reports on SCFO in the world literature since the first description in 1897 by Escat. Clinical presentation, gende…

[Congenital CSF otitic leakage presenting as rhinorrhea: report of a case].

A 10-year-old girl was brought to our ear, nose and throat clinic for consultation on her articulation problem in addition to an attack of bacterial meningitis and cerebrospinal fluid (CSF) rhinorrhea. These problems had bothered her and her family for about 5 years. In August 1988, she had received a bifrontal osteoplastic craniotomy for her CSF leakage, but the procedure was ineffective. Local examination showed bilaterally intact ear drums. The left ear drum looked cloudy and thick with poor mobility under pneumatoscopy. A tympanogram showed that B type. Pure tone audiometry revealed a prof

Maffucci's syndrome as cause of chronic liquorrhea.

We present the case of a 14-year-old girl suffering from Maffucci's syndrome, a rare ailment belonging to the group of osteochondrodysplasias. At the age of 6 months, a diffuse swelling first appeared in the girl's right cheekbone region and the periauricular area. Because of recurrent meningitis with massive otoliquorrhea, several surgical revisions were performed, beginning at the age of 4 years. The histological and immunohistochemical diagnosis showed hemangioma and lymphangioma with enchondroma. As a peculiarity of our patient's diagnosis, we found multiple bone defects apparently caused

Eustachian Tube Obliteration for Temporal Bone Cerebrospinal Fluid Leaks.

Temporal bone cerebrospinal fluid (CSF) leaks most commonly present as unilateral clear rhinorrhea or otorrhea and are often exacerbated by maneuvers that increase intracranial pressure. While a high index of clinical suspicion is essential, beta-2 transferrin remains the gold standard for confirming CSF in otorrhea or nasal secretions. When the Eustachian tube functions as the primary conduit for CSF egress, Eustachian tube obliteration (ETO) serves as an important adjunctive strategy for the management of high-flow or refractory leaks that fail conservative therapy. The transmastoid approach
