Cerebrospinal fluid dynamics and rhinorrhea: the role of shunting in repair.

CSF rhinorrhea can have many causes: traumatic, neoplastic, and iatrogenic origins are common. Most traumatic rhinorrhea ceases after a trial of conservative management. While obvious erosion or traumatic destruction of vital structures may be the underlying cause, other pathophysiologic mechanisms may be working in the formation of CSF rhinorrhea, which may require the combined skills of the otolaryngologist and the neurosurgeon. Leakage of CSF is seen in "high-pressure rhinorrhea," a pathophysiologic state wherein the underlying problem is poor CSF resorption. The result is increased intracr

Cerebrospinal Fluid Prognostic Index in Ventriculoperitoneal Shunt Infection.

BACKGROUND: Considering the consequences of shunt infection, the importance of early diagnosis and effective treatment becomes clear. However, there is no clear parameter to predict the management of shunt infection. Since we thought that an index including cerebrospinal fluid (CSF) leukocyte, glucose, and protein levels may affect treatment guidance, we aimed to investigate its effect on prognosis using the index we defined as CSF prognostic index. METHODS: Ventriculoperitoneal shunt infection patients' age, gender, etiology of hydrocephalus, serum glucose, leukocytes, white blood cell, C-re

Microbiology and treatment of cerebrospinal fluid shunt infections in children.

Infections of cerebrospinal fluid (CSF) shunts remain a common surgical complication causing significant morbidity in children with hydrocephalus. As most of the literature regarding these infections includes only small cohorts from a single institution's experience, there remain large knowledge gaps and little support for the prevailing management strategies. Regarding the microbiology of shunt infections, little has changed in the past 10 years, other than the emergence of methicillin-resistant strains of coagulase-negative staphylococcus (CoNS) and Staphylococcus aureus, which remain the tw

Cerebrospinal fluid shunt infection due to Gemella haemolysans.

Gemella haemolysans has long been considered a commensal in the human upper respiratory tract. Commensals are natural inhabitants on or within another organism, deriving benefit without harming or benefiting the host. Opportunistic infection of the CNS by the species is exceedingly rare. In the present case, a 16-year-old boy was admitted with a ventriculoperitoneal shunt infection, which was confirmed to be due to G. haemolysans. Following antibiotic treatment, removal of the old shunt, and delayed insertion of a new shunt, the patient made a full neurological recovery. To the authors' knowle

Cerebrospinal fluid galactorrhea: a rare complication of ventriculoperitoneal shunting.

In this report we describe a 26-year-old woman who had an intra-abdominal pseudocyst located at the peritoneal catheter tip following ventriculo-peritoneal (VP) shunt implantation. Retrograde cerebrospinal fluid (CSF) flowed outside the catheter and communicated with the right breast lactiferous ductal system and leaked from the nipple orifice. CSF galactorrhea only occurs when the lactiferous duct is injured during VP shunt implantation, in combination with the formation of an intra-abdominal CSF pseudocyst prior to lactiferous duct healing. Leakage of CSF from the nipple orifice can be succe

Conservative management of patients with cerebrospinal fluid shunt infections.

OBJECTIVE: In patients with cerebrospinal fluid (CSF) shunt infection, removal of the shunt and antibiotic administration is the current standard of care. In 1986, we developed a protocol for the conservative management of patients with infected but functioning shunts. Treatment was based on the administration of a combination of intraventricular and systemic antibiotics. Intraventricular antibiotics were instilled via a separate access device. The purpose of this report is to describe our experience with this therapeutic intervention. METHODS: An observational study of all patients treated f

Extremely rare complications in cerebrospinal fluid shunt operations.

The cerebrospinal fluid shunt operation, from its first realization in 1908 by Kausch till our days, is still of a significant importance for the long-term treatment of the internal hydrocephalus. Well known are many complications connected with the use of the valve systems (malfunction, infectious, overdrainage, secondary craniosynostosis and etc.). For a period of 17 years (1984-2000) at the Clinic of Pediatric Neurosurgery, Department of Neurosurgery, Sofia Medical University, 414 cerebrospinal fluid shunt operations were performed on children. 216 were drained to the right atrium of the he

Multiloculated hydrocephalus related to cerebrospinal fluid shunt infection.

This study is an attempt to establish that CSF shunt infection has a role in the aetiology of multiloculated hydrocephalus. The authors carried out a review of 12 cases of multiloculated hydrocephalus who were treated at King Khalid University Hospital between 1988-1994. The multiloculation appears to have developed following the shunt infection in all cases. The hydrocephalus was related to an intraventricular haemorrhage (IVH) in 9 patients and was congenital in 2 patients and post-meningitic in 1 patient. The shunt infection was caused by a gram-negative organism in 8 patients and duration

Lumbo-omental shunt for drainage of cerebrospinal fluid in hydrocephalus.

A new cerebrospinal fluid (CSF) shunting procedure is presented. By transposing a pedicle graft of the greater omentum to the lumbar CSF compartment, the CSF absorption capacity of the omentum is utilized. The operation is applicable to the treatment of communicating hydrocephalus. The technical problems and physiological effects of the operation have been studied earlier in dogs. This paper presents the first case of an intended clinical series. The patient was studied for 18 months postoperatively by different examinations, including psychometric tests, cisternography, computer tomography, e

Is simultaneous cranioplasty with cerebrospinal fluid shunts implantation as safe as staged procedures?

OBJECTIVE: The combination of cranioplasty and ventriculoperitoneal shunt is a therapeutic strategy for patients with hydrocephalus after decompressive craniectomy (DC). However, the efficacies of simultaneous vs. staged surgery in reducing postoperative complications have not been conclusively determined. This was a meta-analysis of relevant studies to assess whether simultaneous surgery significantly reduces postoperative complication risks, compared to staged surgery. METHODS: We systematically searched PubMed, Embase, Cochrane, Web of science databases for studies (published by 11 May 202

Unique complications of cerebrospinal fluid shunts in children--a report of two cases.

The authors report on two cases with unusual CSF shunt complications. The first case had a peritoneal catheter which migrated down a patent processus vaginalis into a hydrocoele. The second case had an atrial catheter which perforated the atrial wall and came to lie in the pericardium causing an effusion. Clinicians should be aware of the frequent and occasionally bizarre complications of CSF shunting.

Emergency department management of children with tracheostomies, feeding tubes, and cerebrospinal fluid shunts.

Children with medical complexity represent a growing population in emergency medicine, and many of these children depend on medical technology. This issue provides an evidence-based framework for the emergency management of children with 3 commonly encountered technologies: tracheostomies, enteral feeding tubes, and cerebrospinal fluid shunts. Distinct risks, including obstruction, dislodgement, infection, and mechanical failure, are discussed. Strategies for early recognition, stabilization, and incorporation of caregiver input are reviewed to improve outcomes and reduce medical errors. By un

Iatrogenic Cerebrospinal Fluid Breast Augmentation: Rare Complication of Ventriculoperitoneal Shunts and Management Strategies.

BACKGROUND: Ventriculoperitoneal shunt is one of the most common neurosurgical procedures in the treatment of hydrocephalus. There are reports of migration of the distal catheter to the breast pocket where cerebrospinal fluid then collects and can develop into a pseudocyst. There exist case reports in the literature of patients with prior breast augmentation who present with distal catheter migration from the peritoneal space into the breast tissue. We present a case series of 3 patients with preexisting breast augmentation who returned with unilateral breast enlargement after ventriculoperito

Treatment of normal pressure hydrocephalus with low versus medium pressure cerebrospinal fluid shunts.

In a retrospective study, 39 patients received a low pressure (20 to 50 mm H2O) shunt and 33 received a medium pressure (55 to 85 mm H2O) shunt for the treatment of normal pressure hydrocephalus (NPH). Pre- and postoperative computed tomographic scans were obtained in 32 patients, permitting us to determine the influence of shunt pressure on ventricular size. A reduction in 3rd ventricle width was found to correlate with clinical improvement and was observed more frequently after the placement of a low pressure shunt than after the placement of a medium pressure shunt. When results were evalua

Association of cerebrospinal fluid parameters with treatment and complications among children with cerebrospinal fluid shunt infections: a multicenter study.

OBJECTIVE: Cerebrospinal fluid (CSF) white blood cell (WBC) count, neutrophil percentage, protein concentration, and glucose level are typically measured at diagnosis and serially during the treatment of CSF shunt infections. The objective of this retrospective cohort study was to describe the longitudinal profile of CSF parameters in children with CSF shunt infections and assess their association with treatment and outcome. METHODS: Participants were children treated at 11 tertiary pediatric hospitals in Canada and the United States for CSF shunt infection, from July 1, 2013, through June 30

Cerebrospinal Fluid Shunts to Treat Hydrocephalus and Idiopathic Intracranial Hypertension: Shunt Catheters and Valves.

The introduction of cerebrospinal fluid (CSF) drainage shunts has made treatment of hydrocephalus possible. However, failure rate is still high. Choice of shunt components could significantly reduce the need for revision, particularly using antisiphon devices and adjustable valves, at least in older patients. Current valves have simple design, draining CSF from normal production site to alternative body cavity. Shunt function is affected by hydrostatic factors, as well as physiologic factors that are still poorly understood. Understanding these mechanisms is crucial to develop smart shunts tha

Risk of infection after cerebrospinal fluid shunt: an analysis of 884 first-time shunts.

Postoperative infections are major complications of cerebrospinal fluid (CSF) shunting in the treatment of hydrocephalus and other conditions with obstructed CSF circulation. In a retrospective study 884 first-time shunted patients with hydrocephalus operated on in the years 1958-1989 were investigated with special reference to the infection rate and to the influence of the following variables: time period, age of the patient, education of the neurosurgeon, length and time of the operation and the exact placement of the distal drain. The overall infection rate for all implanted CSF shunts was

Impact of Air Bubbles on Cerebrospinal Fluid Drainage in Ventriculoperitoneal Shunts: An Experimental Validation.

BACKGROUND AND OBJECTIVES: Airlock has been documented as a failure mechanism in various types of medical catheters, but it remains poorly characterized in ventriculoperitoneal (VP) shunts. The objective of this study was to determine whether catheter airlocks have the capability to induce VP shunt malfunction by impeding cerebrospinal fluid flow and to identify the conditions under which it might occur. METHODS: A laboratory experiment was conducted to investigate various induced air bubble occlusion scenarios in VP shunts. Variables tested included size and number of air bubbles within the

Cerebrospinal fluid shunts in the management of behavioural problems in Sanfilippo syndrome (MPS III).

UNLABELLED: Severe behavioural disturbance is a very common feature of Sanfilippo syndrome (mucopolysaccharidosis III, MPSIII), and one of the more difficult aspects of the disease to treat. We describe a series of six patients with MPS III who had cerebrospinal shunts inserted in an attempt to ameliorate behaviour that had proved refractory to conventional treatment. Symptoms improved significantly in all six but removal of the shunt was necessitated in one patient due to shunt blockage and infection. CONCLUSION: Our experience suggests cerebrospinal fluid shunting should be formally evaluat
