2025 Korean Guidelines for Cardiopulmonary Resuscitation: Part 7. Pediatric basic life support.

Pediatric cardiac arrest primarily arises from asphyxia in infants and trauma in older children, contrasting with adult etiologies dominated by cardiac events. This underscores prevention as the cornerstone of pediatric basic life support, through injury mitigation like child restraint systems and water supervision, safe sleep practices including supine positioning on firm surfaces with caregiver smoking cessation to reduce sudden infant death syndrome, plus awareness of child abuse and adolescent suicide prevention. In hospitals, pediatric early warning systems (PEWS) enable early deteriorati

Child occupant safety in unconventional seating for vehicles with automated driving systems.

The objective of this study was to use computational models to study how unconventional seating positions and orientations in vehicles with Automated Driving System (ADS) may affect occupant response metrics of children with various restraint conditions. A literature review was first conducted to frame a simulation plan, including selections of surrogate ADS-equipped vehicles, potential seating arrangements, impact scenarios, anthropomorphic test device (ATD) models, and child restraint system (CRS) models that are relevant to the selected ATD models. Due to the lack of impact tests with child

Influence of car seat firmness on seat pressure profiles and perceived discomfort during prolonged simulated driving.

During a driving task, the seat-driver interface is particularly influenced by the external environment and seat features. This study compares the effect of two different seats (S1 - soft & S2 - firm) and the effect of visual simulation of different road types (city, highway, mountain, country), on pressure distribution and perceived discomfort during prolonged driving. Twenty participants drove two 3-h sessions (one per seat) on a static simulator. Contact Pressure (CP), Contact Surface (CS), and Seat Pressure Distribution Percentage (SPD%) were analyzed throughout, using two pressure mats po

Restraints for young kids on motorcycles: potential harms likely outweigh the benefits.

The transport of young children on motorcycles is a concern in many low- and middle-income countries. A 2015 World Health Organization report speculated about the potential value of safety restraints ("safety harness") that strap the child to the adult rider and aim to prevent young children from slipping off motorcycles. Although the WHO report recommended that the concept should be evaluated before adoption, India recently became the first country to mandate the use of child safety harnesses. Epidemiological data from India show that children rarely slip off motorcycles. In contrast, crashes

Injury Patterns, Imaging Usage, and Disparities Associated With Car Restraint Use in Children.

BACKGROUND: Motor vehicle collision (MVC) is a leading cause of accidental death in children. Despite effective forms of child safety restraint (eg, car seat and booster seat), studies demonstrate poor compliance with guidelines. The goal of this study was to delineate injury patterns, imaging usage, and potential demographic disparities associated with child restraint use following MVC. METHODS: A retrospective review of the North Carolina Trauma Registry was performed to determine demographic factors and outcomes associated with improper restraint of children (0-8 years) involved in MVC fro

School Bus Transportation of Children With Special Health Care Needs.

School systems are responsible for ensuring that children with special needs are safely transported on all forms of federally approved transportation provided by the school system. A plan to provide the most current and proper support to children with special transportation needs should be developed by the Individualized Education Program team, including the parent, school transportation director, and school nurse, in conjunction with physician orders and recommendations. With this statement, we provide current guidance for the protection of child passengers with specific health care needs. Gu

Protection of children restrained in child safety seats in side impact crashes.

BACKGROUND: The performance of child restraint systems (CRS) in side impact motor vehicle crashes has been under study due to the injury and fatality burden of these events. Although previous research has quantified injury risk or described injured body regions, safety advances require an understanding of injury causation. Therefore, the objective was to delineate injury causation scenarios for CRS-restrained children in side impacts and document probable contact points in the vehicle interior. METHODS: Two in-depth crash investigation databases, the Crash Injury Research and Engineering Netw

Getting Babies Safely Home: A Retrospective Chart Review of Car Safety Seat Tolerance Screening Outcomes.

BACKGROUND: The American Academy of Pediatrics recommends pre-discharge Car Seat Tolerance Screening (CSTS) for all neonates born <37 weeks estimated gestational age (EGA), or otherwise at risk for cardiorespiratory compromise. Screening is burdensome and there remains tremendous variation in testing criteria and methodology. DESIGN/METHODS: We conducted a retrospective chart review of 1,072 infants who underwent CSTS between 11/2013 and 7/2016 at a single academic health center. CSTS outcomes (failure and, separately, significant cardiorespiratory instability (CRI)), including those not meet

Oblique pole-side crash assessment using the six-year-old HBM PIPER.

Side impact crashes are one of the most dangerous impact scenarios that a child can suffer. Studies by the National Highway Traffic Safety Administration (NHTSA) have shown that the head and Thorax regions are affected severely. The objective of this work is to perform a numerical evaluation of the oblique pole-side test considering the FMVSS 214-P standard to estimate the Head, Neck, and Thorax injuries for a six-year-old child positioned in the rear seat without any Child Restraint System, two configurations were performed for the oblique pole-side impact: a nearside and a far-side positioni

Vehicle child restraint usage for Pacific children aged 6 weeks to 4 years: findings from the Pacific Islands Families study.

Child restraint systems (CRSs) for vehicles are designed to provide protection and prevent or reduce child mortality and morbidity in road traffic accidents. Overall, 90% of children under 5 years of age in New Zealand currently use CRSs. There is considerable regional variability in CRS usage, but little information exists on its ethnic variations or determinants. "Increasing the level of restraint use" is explicitly stated as one of the 13 priorities within the New Zealand Ministry of Transport's new road safety strategy. As such, understanding CRS prevalence, patterns and associates within

The effect of reclined seat-back angles on the LODC with and without a belt-positioning booster during far-side lateral-oblique impacts.

OBJECTIVE: In frontal crashes belt-positioning boosters (BPB) may prevent submarining when the seatback is reclined. It is unclear if the BPB can also mitigate injuries in far-side lateral-oblique crashes in reclined conditions, where current restraints are less effective in reducing lateral excursion. This study aimed to understand reclined child injury risk during lateral-oblique impacts, with and without a booster seat, by using the Large Omni-Directional Child (LODC) test device. METHODS: The LODC was tested in nine lateral-oblique impact (80° from frontal) sled tests (target 31.3 km/h, d

Chest wall bruising from 5-point car seat harness as a mimicker of child abuse.

Bruising rarely presents in infants younger than 9 months who are not ambulatory and is more prevalent among children beginning to walk, or "cruisers." We present the case of a healthy 3-month-old infant with asymptomatic, symmetric, bilateral, large bruises on the bony chest sparing the mid-chest/sternum with a negative non-accidental trauma work-up. The noted pattern of bruises matched the bilateral shoulder straps of a 5-point harness of the car seat belt designed for infants. Awareness of this unique pattern of bruises will help elicit a better-informed history to guide care in an appropri

In-depth evaluation of real-world car collisions: fatal and severe injuries in children are predominantly caused by restraint errors and unstrapped cargo.

OBJECTIVE: Major improvements have taken place in the development of child restraint systems and in-car safety in general, but motor vehicle accidents remain the leading cause of death and disability in children. An interdisciplinary study was therefore performed to investigate the injury mechanisms in car collisions involving children. METHODS: Motor vehicle collisions (MVCs) resulting in death or serious injuries to the drivers or their passengers in the southeastern part of Norway in the period 2007-2009 were included in the study if children less than 16 years of age were passengers. Load-Leg Supports as a Biomechanical Intervention in Rear-Facing Child Restraint Misuse.

This study evaluated the biomechanical effectiveness of anti-rotation load-leg support in rear-facing child restraint systems (CRSs) under proper and improper (misuse) installation conditions. We conducted eight FMVSS 213 sled tests using a CRABI 12-month anthropomorphic test device under four installation configurations-proper installation, loose seat, loose harness, and both loose seat and loose harness-with and without the load-leg support. The use of the load leg reduced head and neck injury metrics in all installation conditions: mean HIC36 decreased from 682 to 571 (by 16%) and AIS≥4 hea
