Immunological reactions to chlorhexidine, including allergy (Type I hypersensitivity) and allergic contact dermatitis/stomatitis (Type IV hypersensitivity), have been recognised for many years. This potential safety issue, however, is not well known within dentistry. The purpose of this paper is to alert dentists and dental care professionals to the potential of chlorhexidine in causing hypersensitivity reactions and to consider this possibility if unexplained hypersensitivity reactions occur.

OBJECTIVE: The purpose of this study was to establish whether addition of a 2% chlorhexidine rinse to a conventional treatment protocol enhances the rate of the successful disinfection of the root canal system in vivo.
STUDY DESIGN: Twenty-four teeth with infected necrotic pulps and resorbing apical periodontitis were treated with a conventional technique in which 1% NaOCl as irrigant was used. Half of the cases received an additional rinse with 2% chlorhexidine. Prereduced thioglycollate medium was used to take cultures that were incubated for 4 weeks.
RESULTS: Cultivable bacteria were retr

Chlorhexidine has been utilized as a root canal irrigant since 1971. It is a broad-spectrum antimicrobial agent with minimal toxicity and residual long-term antibacterial capability. Although a sodium hypochlorite solution has been shown to have a better antimicrobial effect than chlorhexidine, its corrosive effect has prevented its further use especially since the in vitro antimicrobial effect of chlorhexidine has been shown to not be much less. However, the efficacy of chlorhexidine as an endodontic irrigant and intracanal medicament, requires further research in vivo. Therefore, in this stu

In a short-term longitudinal study the authors intended to compare the effectiveness in controlling plaque formation and gingival inflammation of a 1% chlorhexidine gel to the one of a 0.2% chlorhexidine solution. The gel, used once a day, resulted as good as the chlorhexidine solution, but showed a localized activity.

BACKGROUND: Chlorhexidine-gluconate (CHG) impregnated dressings may prevent catheter-related bloodstream infections (CRBSI). Chlorhexidine-impregnated sponge dressings (sponge-dress) and gel dressings (gel-dress) have never been directly compared. We used the data collected for two randomized-controlled trials to perform a comparison between sponge-dress and gel-dress.
METHODS: Adult critically ill patients who required short-term central venous or arterial catheter insertion were recruited. Our main analysis included only patients with CHG-impregnated dressings. The effect of gel-dress (vers

BACKGROUND: Chitosan mouthwash has antimicrobial and antiplaque properties with lower cytotoxicity than many conventional mouthwashes. Given the well-known side effects of chlorhexidine, particularly tooth discoloration, this study aimed to compare enamel discoloration caused by chitosan, chlorhexidine, and chitosan-chlorhexidine mouthwashes, and to assess whether chitosan-based formulations could be a suitable alternative with fewer adverse effects. MATERIALS AND METHODS: In this in vitro study, 45 extracted premolar teeth, obtained for orthodontic reasons and free from structural defects, cr

Chlorhexidine is a commonly used disinfectant throughout Australian hospitals. It is responsible for a number of iatrogenic complications. We describe a case of 27-yearold female who sustained a severe, blistering reaction at the site of chlorhexidine application, associated with significant pain. This reaction was initially managed with wet dressings and topical corticosteroids, but there was no improvement in pain or rash. Management was then changed to silver-coated polyethylene mesh dressings, with resolution of pain and rash after four days. No debridement was required, and area healed wi

PURPOSE: Chlorhexidine is an antimicrobial agent used in the prevention of postextraction alveolar osteitis, tooth decay, and periodontal diseases. There are various forms of chlorhexidine application. The most extensively studied is one that uses the rinse as the form of application. Recently, a bioadhesive gel form has become available. Its main advantage is that it prolongs the bioavailability of chlorhexidine in the application area. The purpose of this study was to compare the effectiveness of chlorhexidine gel versus a chlorhexidine rinse in reducing postoperative alveolar osteitis after

Chlorhexidine is a commonly used antiseptic and disinfectant in the health-care setting. Its usage has increased in recent years with intensive campaigns and infection control guidelines to combat hospital-acquired infections. As a result, patients and health-care workers (HCW) are exposed to increasing chlorhexidine usage. In recent years, adverse reactions to chlorhexidine ranging from allergic contact dermatitis, photosensitivity, fixed drug eruptions, urticaria and anaphylactic shock have been reported. Most have been isolated case reports on adverse reactions occurring in healthy individu

Three case reports demonstrate the clinical use of an adjunctive chlorhexidine chip capable of sustaining local antimicrobial activity for 7-10 days. The chip is easy to place, requires no additional chair time for removal, is self-adherent, and can be provided to periodontal maintenance patients on a recurring basis. This is another adjunct that can be part of the clinician's armamentarium when presented with a clinically challenging localized periodontal defect.

Six patients who developed urticaria, dyspnea, and anaphylactic shock due to topical application of chlorhexidine gluconate solution are described. Chlorhexidine gluconate was confirmed as the causative agent of type I hypersensitivity by intradermal, scratch, and epicutaneous tests. To prevent life-threatening adverse reactions, it seems important to use chlorhexidine gluconate on wound surfaces at a concentration of 0.05%, as recommended by the manufacturer; chlorhexidine gluconate may not be suitable for application to mucous membranes.

BACKGROUND: Catheter colonization and bloodstream infection during the first week after insertion of a central venous catheter have been shown to result from the patient's own skin flora.
METHODS: The backs of 32 healthy subjects were prepped with a 2% chlorhexidine gluconate (CHG)/70% isopropyl alcohol antiseptic. Three dressings, 2 of which contained CHG, were placed on the prepped skin in a randomized design. Samples of aerobic bacteria were collected using the cup scrub method. Skin under the dressings was sampled by quadrant on days 1, 4, and 7. Relative suppression of regrowth was compa

Chlorhexidine, an antiseptic belonging to family of biguanides, is used extensively in the medical and surgical environment. Late onset hypersensitivity and eczema occur regularly and are well documented events. Conversely, immediate hypersensitivity, sometimes taking the form of acute urticaria that can result in anaphylactic shock, is rarer. These manifestations can occur during contact of the skin or mucosa with chlorhexidine. Out of the fifty case reports of chlorhexidine-related anaphylaxis published worldwide over the past ten years, fifteen occurred during surgery. Signs generally appea
