Virtual reality combined with task-oriented circuit training for upper limb rehabilitation in subacute stroke patients: A randomized controlled trial.

BACKGROUND: Task-oriented circuit training (TOCT) and virtual reality (VR) are innovative clinical treatment approaches based on motor relearning and neuroplasticity principles. However, the synergistic effects of these 2 methods on improving upper extremity function in stroke patients remain unclear. This study investigates the impact of VR combined with TOCT on upper limb functions and quality of life (QoL) in subacute stroke patients. METHODS: In this randomized controlled single-blind study, 64 subacute stroke patients were recruited and allocated to a control group (CG) or an experimenta

Task-Oriented Circuit Training for Mobility in Outpatient Stroke Rehabilitation in Germany and Austria: A Contextual Transferability Analysis.

People with stroke cite mobility deficits as one of the most burdensome limitations. National and international stroke guidelines recommend physical therapy based on task-oriented practice, with high numbers of repetitions to improve mobility. In the outpatient setting in Germany and Austria, these principles have not yet been established. The purpose of this study was to identify an evidence-based intervention that could help reduce this research-practice gap. A stepwise approach proposed by Voigt-Radloff and colleagues and Cochrane Germany was used. First, the specific health service problem

Effects of Task-Oriented Circuit Training on Dizziness, Vertigo Balance, Gait, and Quality of Life in Patients with Peripheral Vestibular Hypofunction: A Single-Blind, Randomized Controlled Trial.

Background/Objectives: Peripheral vestibular hypofunction (PVH) commonly causes dizziness, imbalance, gait disturbances, and reduced quality of life. Task-oriented circuit training (TOCT) is a rehabilitation approach in which patients perform structured, task-specific functional movements repetitively to improve real-life motor performance. TOCT integrates functional, multisensory, and repetitive exercises based on motor learning and neuroplasticity principles, potentially enhancing rehabilitation outcomes. This study aimed to investigate the effects of TOCT on dizziness, vertigo, balance, gai

Exercise programme in patients with non-tuberculous mycobacterial pulmonary disease (NTM-Exercise): protocol of a randomised controlled trial.

INTRODUCTION: Non-tuberculous mycobacterial pulmonary disease (NTM-PD) is a chronic debilitating infection requiring prolonged multidrug therapy. Common symptoms contribute to inactivity, malnutrition and sarcopenia, and low muscle mass is associated with increased mortality. Evidence supporting exercise in NTM-PD is scarce, and no randomised controlled trial has evaluated its efficacy. This study aims to determine whether a 6-week hybrid, circuit-based exercise programme improves cardiorespiratory fitness compared with delayed intervention/usual care and to evaluate its effects on physical fu

The beneficial effects of 15 units of high-intensity circuit training in women is modified by age, baseline insulin resistance and physical capacity.

AIM: To investigate the effect of a single and 15 units of high-intensity circuit training (HICT) programme on glucose metabolism, myokines' response and selected genes' expression in women. METHODS: Thirty-three, non-active women (mean age: 38 ± 12) were split into a HICT (n = 20) or a control group (CON, n = 13). The training protocol included three circuits of nine exercises with own body weight as a workload performed 3 times a week for five weeks. The CON group performed HICT twice. Blood samples were taken before, 1 h and 24 h after the first and last unit to determine IGF-1, myostatin,

Exercise Modulates Working Memory-Related Brain Activation Through Bidirectional Neural Mechanisms: An ALE Meta-Analysis of Longitudinal and Cross-Sectional fMRI Studies.

BACKGROUND: Physical exercise improves working memory (WM) across the lifespan, yet the neural mechanisms underlying this benefit remain incompletely understood, and it is unclear whether intervention-induced neuroplastic changes and long-term exercise-related differences converge on common neural circuits. METHODS: We systematically searched PubMed, Web of Science, PsycINFO, and CNKI through June 2026 and included 11 task-based fMRI studies (6 longitudinal interventions and 5 cross-sectional comparisons; 403 participants) reporting whole-brain activation coordinates. Short-term, circuit-based heat acclimation enhances intermittent work capacity, reaction time, and neuromuscular function under heat stress in active males.

Exertional heat stress impairs cardiovascular, thermoregulatory, cognitive, and neuromuscular function, compromising health and performance. Although effective, traditional active heat acclimation (HA) relies on prolonged, low- to moderate-intensity exercise heat exposures and may therefore lack context-specificity. We investigated whether short-duration, high-intensity, circuit-based HA elicits favorable performance and thermophysiological adaptations. Eighteen participants completed either five circuit training sessions in hot (HEAT: 35°C; n = 9) or thermoneutral conditions (CON: 20°C; n = 9

The Impact of Resistance Exercise on Muscle Mass in Glioblastoma in Survivors (RESIST): Protocol for a Randomized Controlled Trial.

BACKGROUND: Glioblastoma is the most common primary brain malignancy in adults, accounting for approximately 48% of all brain tumors. Standard treatment includes radiation and temozolomide chemotherapy. Glioblastomas are highly vascular and can cause vasogenic brain edema and mass effect, which can worsen the neurologic symptoms associated with the disease. The steroid dexamethasone (DEX) is the treatment of choice to reduce vasogenic edema and intracranial pressure associated with glioblastoma. However high-dose DEX or long-term use can result in muscle myopathy in 10%-60% of glioblastoma pat

Prehabilitation exercise therapy before elective abdominal aortic aneurysm repair.

BACKGROUND: An abdominal aortic aneurysm (AAA) is an abnormal dilation in the diameter of the abdominal aorta of 50% or more of the normal diameter or greater than 3 cm in total. The risk of rupture increases with the diameter of the aneurysm, particularly above a diameter of approximately 5.5 cm. Perioperative and postoperative morbidity is common following elective repair in people with AAA. Prehabilitation or preoperative exercise is the process of enhancing an individual's functional capacity before surgery to improve postoperative outcomes. Studies have evaluated exercise interventions fo

Exercise-enhanced neuroplasticity targeting motor and cognitive circuitry in Parkinson's disease.

Exercise interventions in individuals with Parkinson's disease incorporate goal-based motor skill training to engage cognitive circuitry important in motor learning. With this exercise approach, physical therapy helps with learning through instruction and feedback (reinforcement) and encouragement to perform beyond self-perceived capability. Individuals with Parkinson's disease become more cognitively engaged with the practice and learning of movements and skills that were previously automatic and unconscious. Aerobic exercise, regarded as important for improvement of blood flow and facilitati

Insufficient exercise intensity for clinical benefit? Monitoring and quantification of a community-based Phase III cardiac rehabilitation programme: A United Kingdom perspective.

BACKGROUND: In recent years, criticism of the percentage range approach for individualised exercise prescription has intensified and we were concerned that sub-optimal exercise dose (especially intensity) may be in part responsible for the variability in the effectiveness of cardiac rehabilitation (CR) programmes in the United Kingdom (UK). The aim was to investigate the fidelity of a structured Phase III CR programme, by monitoring and quantifying exercise training intensity. DESIGN: Observational study. METHODS: The programme comprised 16 sessions over 8 weeks, where patients undertook an

Effects of Kinect-based mixed-reality exercise and deep-sea water consumption on metabolic syndrome: a randomized controlled trial.

PURPOSE: The increasing prevalence of metabolic syndrome poses significant health risks, particularly for cardiovascular diseases and type 2 diabetes. Exercise interventions have shown promise in improving the components of metabolic syndrome, including blood pressure, lipid profiles, and glucose metabolism. Kinect-based mixed-reality exercise provides real-time feedback and personalized guidance, making it suitable for unsupervised training. Deep-sea water has beneficial effects on lipid metabolism and insulin sensitivity. However, the effects of Kinect-based mixed-reality exercise and deep-s

Using Visual Methods to Understand Physical Activity Maintenance following Cardiac Rehabilitation.

Few studies have explored the factors associated with long-term maintenance of exercise following cardiac rehabilitation. The present study used auto-photography and interviews to explore the factors that influence motivation and continued participation in physical activity among post cardiac rehabilitation patients. Twenty-three semi-structured interviews were conducted alongside participant-selected photographs or drawings with participants that had continued participation in physical activity for at least two years following the cardiac rehabilitation programme. Participants were recruited

Baseline Characteristics of Individuals with Metastatic Cancer Enrolled in the Alberta Cancer Exercise Study and 12-Week Findings for Symptom-Related and Physical Fitness Measures.

Exercise has been found to be safe and beneficial for people with advanced cancers, but more research is needed to understand how best to design and implement exercise programming. The Alberta Cancer Exercise (ACE) study examines the effectiveness and implementation of a 12-week community-based exercise program in Alberta, Canada, for people diagnosed with cancer. Here, we describe the characteristics of individuals with metastatic cancer enrolled in the ACE program and report 12-week changes in self-reported and objective outcomes. Of 306 participants, 274 (89.5%) completed the 12-week study.
