Continuous renal replacement therapy (CRRT) is one of the most commonly used treatments for patients with acute and chronic renal failure as well as end-stage chronic kidney disease, and is an important modality of renal replacement therapy. CRRT originated in the 1990s, initially focusing on a single dialysis mode. With the advancement of technology, treatment modes have gradually diversified, treatment effectiveness and safety have been optimized, and treatment efficiency has also been significantly improved. This article surveys the development history of CRRT devices and their clinical app

Carnitine deficiency is known to occur in chronic hemodialysis; however, the effect of continuous renal replacement therapy (CRRT) on carnitine homeostasis has not been studied. We hypothesized that children receiving CRRT are at risk for deficiency because of continuous removal, absent intake, decreased production, and comorbidities related to critical illness. Records of patients with acute kidney injury receiving CRRT at Children's National Health System between 2011 and 2014 were reviewed for total carnitine (TC), free carnitine (FC), feeding modality, severity of illness, and survival out

Ultrafiltration is effective in the treatment of fluid and sodium overload in congestive heart failure. There is no available device to provide this therapy to ambulatory patients. We built and tested in vivo a wearable belt that can provide continuous ultrafiltration, 168 hours a week. Nine pigs underwent ureteral ligation and subsequently were allowed fluids ad lib, producing fluid overload. Next day, ultrafiltration was performed for 8 hours. The device consists of a hollow-fiber filter, a 9 V battery-operated pulsatile blood pump, a micro pump for heparin infusion, and another micro pump t

OBJECTIVES: The optimal fluid management strategy on continuous renal replacement therapy (CRRT) is unknown for critically ill children. The pace of ultrafiltration has been highlighted as a risk predictor for adverse outcomes in adult cohorts. Whether CRRT can cause dialytrauma through excessive ultrafiltration rates (UFRs) in children is undetermined. Although fluid overload (FO) at CRRT start has been associated with adverse outcomes, net fluid balance (NFB) on CRRT has not been investigated as a predictor for renal recovery.
DESIGN: Retrospective cohort study.
SETTING: Two quaternary PIC

Continuous renal replacement therapy (CRRT) is an important support treatment technique for critically ill patients. Anticoagulation is a crucial part for CRRT. High bleeding risk, hypercoagulability, liver failure, and tissue hypoperfusion are very common in critically ill patients. Patients with these conditions, neonate/infant patients, and old patients need special considerations on clearance of anticoagulant and the efficacy and safety of anticoagulation for CRRT. Therefore, bleeding risk, coagulation status, metabolism characteristics of anticoagulation drug, and treatment needs should b

Coronavirus disease 2019 (COVID-19) appears to be associated with increased arterial and venous thromboembolic disease. These presumed abnormalities in hemostasis have been associated with filter clotting during continuous renal replacement therapy (CRRT). We aimed to characterize the burden of CRRT filter clotting in COVID-19 infection and to describe a CRRT anticoagulation protocol that used anti-factor Xa levels for systemic heparin dosing. Multi-center study of consecutive patients with COVID-19 receiving CRRT. Primary outcome was CRRT filter loss. Sixty-five patients were analyzed, includ

INTRODUCTION: Little is known about early (first 48 h) hourly and cumulative fluid balance (FB) during continuous renal replacement therapy (CRRT).
OBJECTIVES: To study the characteristics and outcome associations of early hourly and cumulative FB.
METHODS: We studied FB in CRRT patients (2016-2018).
RESULTS: Among 350 patients, mean hourly FB became negative after 20 CRRT hours, but within 6 CRRT hours in patients with baseline fluid overload. A negative early FB was never achieved in patients receiving vasopressor therapy (p < 0.001). Mortality was 31%. The percentage of hourly negative F

The incidence of the multiple organ dysfunction syndrome (MODS) is rapidly increasing in intensive care units (ICU). It usually combines with sepsis and is the most frequent cause of death in the ICU patients. The nature of the ICU patients has changed in the last years. It includes a variety of patients with severe cases due to major surgical interventions, trauma, hemodynamic instability, sepsis, and so on but also older people than previous times. All these situations can easily lead to MODS. In the prior years, the only available and efficient therapy was renal replacement therapy (RRT) fo

BACKGROUND AND AIMS: Continuous venovenous hemofiltration or hemodiafiltration is used frequently in pediatric patients, but experience of continuous renal replacement therapy (CRRT) application on extracorporeal membrane oxygenation (ECMO) circuit is still limited. Among several methods used for applying CRRT on ECMO patients, we aim to share our experience on inclusion of a CRRT device in the ECMO circuit which we believe is easier and safer to apply.
MATERIALS AND METHODS: The data were collected on demographics, outcomes, and details of the treatment of ECMO patients who had CRRT. During

BACKGROUND: Acute kidney injury (AKI) is a common condition among patients in intensive care units (ICU), and is associated with substantial morbidity and mortality. Continuous renal replacement therapy (CRRT) is a blood purification technique used to treat the most severe forms of AKI but its effectiveness remains unclear.
OBJECTIVES: To assess the effects of different intensities (intensive and less intensive) of CRRT on mortality and recovery of kidney function in critically ill AKI patients.
SEARCH METHODS: We searched Cochrane Kidney and Transplant's Specialised Register to 9 February 2

PURPOSE: Acute kidney injury in the pediatric intensive care unit (PICU) is associated with significant morbidity, with continued mortality greater than 50%. Previous studies have described an association between percentage of fluid overload (%FO) less than 20% and improved survival. We reviewed our continuous renal replacement therapy (CRRT) experience to evaluate for factors associated with mortality as well as secondary outcomes.
MATERIALS AND METHODS: This is a retrospective chart review of pediatric CRRT intensive care unit patients from January 2000 to September 2005.
RESULTS: Seventy-

The performance of continuous renal replacement therapy (CRRT) brings about alterations in the serum potassium levels in patient. Potassium is an electrolyte essential for the regulation of nerve conduction and muscle contraction, particularly important in the case of cardiac muscle. This article describes the physiological mechanisms that affect potassium distribution throughout the body and also describes the effects of hypokalaemia or hyperkalaemia on the heart. This article justifies the need for serum potassium control during CRRT, which recognizes and reconciles the differing areas of re

INTRODUCTION: Lactic acidosis is an infrequent complication of metformin therapy for diabetes mellitus. The presence of clinical conditions, such as renal failure, increases the risk of metformin-associated lactic acidosis (MALA). We present a case of lactic acidosis in a patient with diabetes treated with metformin, complicated by acute renal failure in preexisting chronic nephropathy.
CASE SUMMARY: A 70-year-old white male, weighing 77 kg, with diabetes mellitus, coronary heart disease, congestive heart failure (New York Heart Association class III), moderate essential hypertension (stage 2

Continuous renal replacement therapies (CRRT) are used to manage fluid overload and/or renal failure. The continuous nature of the fluid and solute removal has less impact on hemodynamic variables in critically ill patients, making CRRT preferred over intermittent hemodialysis for some patients in the intensive care arena. The impact of CRRT on drug removal is variable depending on the CRRT modality, the ultrafiltrate and dialysate flow rates, the filter, and the patient's residual renal function; all of these may change from patient to patient or even in the same patient depending on the clin

Re-initiation of continuous renal replacement therapy (CRRT) in neonates and young infants weighing less than 15 kg often necessitates a blood prime of the blood circuit path or a concurrent packed red blood cell (PRBC) transfusion to avoid causing hemodynamic instability due to acute hemodilution. The significant amount of time required for a routine CRRT circuit change can be associated with worsening electrolyte and acid-base abnormalities, fluid retention, greater hemodynamic instability and reducing effective hemofiltration time. In an attempt to limit the time without CRRT and to elimina

BACKGROUND: The preferred method of renal-replacement therapy for acute kidney injury in critically ill patients remains uncertain.
METHODS: In this multicenter, open-label, noninferiority trial, we randomly assigned critically ill adults with acute kidney injury who had indications for renal-replacement therapy to undergo intermittent hemodialysis or continuous renal-replacement therapy as the initial treatment. The primary outcome was a major adverse kidney event - defined as death, dependence on renal-replacement therapy, or a serum creatinine concentration more than 25% above the baseline
