[Crohn disease. Diagnosis and treatment].

Crohn's disease was the topic of a meeting of gastroenterologists in Bergen last year with the purpose of working out consensus guidelines for diagnosis and treatment of this chronic disease. The most important diagnostic procedures are radiology, gastrointestinal endoscopy and histopathology. Transmural, segmental involvement is characteristic, whereas granulomas are found in only 25% of biopsy specimens. Corticosteroids are effective in active Crohn's disease located to all parts of the intestine, whereas sulfasalazine and metronidazole are most effective in Crohn's colitis. Azathioprine and

[Radiological assessment of Crohn disease].

Fifty-eight consecutive cases of patients affected by Crohn's disease, with ileum (23/58), colon (10/58) and ileo-colic (25/58) involvement were studied. A good overall sensitivity was reached by the radiological procedures employed (barium meal, barium enema, enteroclysis). Enteroclysis is proposed as a second-step method for the study of ileum involvement, because it provides a quite precise assessment of disease stage and extent. Some criteria for a rational use of current radiological procedures in the follow-up of both surgically and medically treated patients are proposed. Moreover it is

Rectourethrocutaneous fistula in Crohn disease.

Fistulae from the gastrointestinal tract to the urinary tract are a well-known complication of Crohn disease. The majority of these are ileovesical. On rare occasion, patients with perianal complications of regional ileitis or granulomatous colitis may develop rectourethrocutaneous fistulae. The authors have encountered 2 such patients and add them to the 3 previously reported cases in the English literature. Retrograde urethrography and voiding cystourethrography established the diagnosis.

[Urologic complications of Crohn disease].

A report on urologic complications of Crohn's disease observed in 9 out of 51 operated patients. Primary resection of the diseased bowel segment is of decisive importance in the treatment of these vesicoenteric fistulae or ureteric stenoses. Additional ureterolysis is required only in case of extensive retroperitoneal fibrosis.

Enterovesical fistulas in Crohn disease.

Fistulous communication between the intestinal tract and the bladder is uncommon in Crohn disease. Only 2% of patients afflicted with this disease present with bladder symptoms. Five patients with a mean age of thirty years presented with a prolonged history of bowel disease but a short history of urinary symptoms. Only three patients presented with pneumaturia and fecaluria. None of the fistulas could be demonstrated radiographically. Cystoscopy disclosed the fistulous site as well as acute and chronic inflammation of the bladder. Small-bowel resection with excision of the fistula, partial bl

Metastatic Crohn Disease Manifesting as Granulomatous Cheilitis in a Patient With Well-Controlled Crohn Disease.

Crohn disease (CD) can affect any organ system outside the gastrointestinal tract, and these conditions are classified as extraintestinal manifestations and include arthritis, uveitis, and cutaneous manifestations. Cutaneous involvement includes erythema nodosum, pyoderma gangrenosum, and metastatic CD. The latter can be defined as a granulomatous skin lesion that is noncontiguous to the intestinal disease. In this article, we report a rare case of a 48-year-old woman with CD in endoscopic and clinical remission with chronic persistent orofacial granulomatosis of the lip secondary to CD, despi

Management of Crohn Disease: A Review.

IMPORTANCE: Crohn disease, a chronic gastrointestinal inflammatory disease, is increasing in incidence and prevalence in many parts of the world. Uncontrolled inflammation leads to long-term complications, including fibrotic strictures, enteric fistulae, and intestinal neoplasia. Therefore, early and effective control of inflammation is of critical importance. OBSERVATIONS: The optimal management approach for Crohn disease incorporates patient risk stratification, patient preference, and clinical factors in therapeutic decision-making. First-line therapy generally consists of steroids for rap

Crohn disease and the gynecologic patient.

Although Crohn disease (CD) is considered an inflammatory bowel disease, extraintestinal gynecologic manifestations are varied, frequent, and oftentimes difficult to manage. Its predilection for young and reproductive-age women makes it an important disease process for the gynecologist to understand, as its complications can have long-term repercussions on the developmental, sexual, reproductive, and psychological health of affected women. Patients may present with a variety of vulvovaginal, perineal, perianal, and urologic complaints. Perianal involvement from an intestinal fistula is the mos

Surgical treatment of anorectal crohn disease.

Crohn disease involves the perineum and rectum in approximately one-third of patients. Symptoms can range from mild, including skin tags and hemorrhoids, to unremitting and severe, requiring a proctectomy in a small, but significant, portion. Fistula-in-ano and perineal sepsis are the most frequent manifestation seen on presentation. Careful diagnosis, including magnetic resonance imaging or endorectal ultrasound with examination under anesthesia and aggressive medical management, usually with a tumor necrosis factor-alpha, is critical to success. Several options for definitive surgical repair

Budesonide use in pediatric Crohn disease.

BACKGROUND AND AIM: Budesonide (BUD) is being used in pediatric Crohn disease (CD) because it is believed to have the potential to reduce corticosteroid-related toxicity; however, few data are available describing its use. The aim of the present study was to describe BUD use in an inception cohort of pediatric patients with CD. METHODS: Data were derived from the prospective Pediatric IBD Collaborative Research Group Registry established in 2002 in North America. Use of BUD in children with CD was examined. RESULTS: BUD was used in 119 of 932 (13%) of children with newly diagnosed CD, with 5

Ultrasonography of Crohn disease in children.

US is increasingly performed in Crohn disease (CD) in children as a first line imaging modality. It reduces the use of other more invasive examinations such as endoscopy, CT or contrast enema. We describe bowel ultrasonography technique, normal bowel appearances on US and pathological patterns in CD. We discuss the current role and limitations of bowel US in CD in children including diagnosis, extent of disease, assessment of disease activity, follow-up and detection of complications. The diagnostic accuracy of US is discussed according to the literature and compared to other imaging modalitie

[Osteopenia in patients with Crohn disease].

BACKGROUND: Osteoporosis and osteopenia are frequently reported during chronic inflammatory bowel diseases, particularly in Crohn disease (CD). AIM: To determine the frequency of osteoporosis and osteopenia of patients with CD and to study associated risk factors. METHODS: A forward-looking study was performed about 56 cases of CD, 34 men and 22 women, aged 32 + 10.4 years (18-54 years), with no other disorders affecting osseous metabolism. A biphotonic X-rays absorptiometry densitometry (DEXA) was performed at femoral neck and lumbar spine. A multivariate statistical analysis was performed

Cutaneous Crohn disease in a child.

Cutaneous Crohn disease refers to granulomatous skin manifestations not contiguous with gastrointestinal affected areas. It is a very rare condition, especially in children, and is easily misdiagnosed when characteristic gastrointestinal symptoms are absent. We report a 10-year-old Caucasian girl with a 6-month history of erythematous, firm tumescence of the left labium majus pudendi and moist vegetations circumscribing the anal ostium. Histologic analysis of skin biopsy specimens from both types of lesions showed a characteristic granulomatous noncaseating infiltrate throughout the dermis. En

Cross-sectional imaging in Crohn disease.

The role of cross-sectional imaging in the diagnosis of Crohn disease has expanded with recent technologic advances in computed tomography (CT) and magnetic resonance (MR) imaging that allow rapid acquisition of high-resolution images of the intestines. To acquire images of diagnostic quality, administration of a fairly large amount of intraluminal contrast agent prior to examination and scanning with intravenous contrast material injection are necessary. Both CT and MR imaging are reported to have a sensitivity of over 95% for the detection of Crohn disease; however, they may not allow early

[Minimally invasive surgery and Crohn disease].

Patients with Crohn's disease are operated on with a morbidity of 15% and a mortality of 0% if the indication for surgery is decided in good time. After ileocecal resection the probability of having a second resection in 15-20 years is about 50%. When elective surgery is done at an early disease stage, ileocecal resections and reoperations for anastomotic stenosis can be performed by assisted laparoscopy assisted. Laparoscopic colonic resections are done more rarely. Acute and life-threatening conditions such as obstruction, perforation and sepsis are excluded from the laparoscopic approach. I
