Almost normal: experiences of adolescents with implantable cardioverter defibrillators.

PURPOSE: The purpose of this study was to describe the experiences of adolescents with implantable cardioverter defibrillators (ICDs). DESIGN AND METHODS: This qualitative study explored the lived experiences of 14 adolescents with ICDs using face-to-face, semistructured interviews. RESULTS: Six themes were identified: life interrupted; still the same, except; sometimes it's tough; it cuts both ways; unique joule; and living life regular. PRACTICE IMPLICATIONS: These findings suggest that nurses could make a significant impact on the lives of adolescents with ICDs by focusing their efforts

Implantable cardioverter defibrillators: arrhythmias. A rapid and systematic review.

UNLABELLED: PROPOSED SERVICE: The service proposed is the use of implantable cardioverter defibrillators (ICDs) in the management of risk factors leading to sudden cardiac death (SCD). ICDs are similar in size to a pacemaker and are intended to prevent death due to life-threatening ventricular tachyarrhythmias. EPIDEMIOLOGY AND BACKGROUND: SCD occurs in approximately 100,000 people annually in the UK and is usually due to ventricular tachyarrhythmia. Increasing numbers of people are surviving a first episode of ventricular tachyarrhythmia and are at high risk of further episodes. Standard tre

Managing atrial tachyarrhythmias in patients with implantable cardioverter defibrillators.

The implantable cardioverter defibrillator (ICD) is accepted as the therapy of choice in preventing sudden cardiac death. Multiple studies, such as Antiarrhythmics Versus Implantable Defibrillators (AVID), the Canadian Implantable Defibrillator Study (CIDS), the Cardiac Arrest Study Hamburg (CASH), and the Multicenter Automatic Defibrillator Implantation Trial (MADIT), have shown a substantial benefit in survival rates for patients treated with ICDs compared with antiarrhythmic drug treatment. The detection of spontaneous ventricular tachycardias (VT) is based primarily on the programmed heart

One-incision approach for insertion of implantable cardioverter defibrillators.

The placement of a transvenous implantable cardioverter defibrillator (ICD) system through a single infraclavicular skin incision has been a surgical goal for years. The development of a new investigational model of ICD with substantially reduced dimensions (volume, 83 cm3; mass, 132 g) has made the one-incision approach a clinical reality. Between March and September 1993, 4 female and 19 male patients (mean age, 60 +/- 9.6 years; range, 46 to 73 years) underwent implantation of this device for the treatment of ventricular fibrillation (n = 14) or ventricular tachycardia (n = 9). One transven

Contemporary issues in implantable cardioverter-defibrillators: Personalising device therapy.

Implantable Cardioverter Defibrillators (ICDs) have undergone substantial evolution since the 1980s, with the aim of decreasing sudden cardiac death, whilst minimising device related complications. Traditional transvenous ICDs (TV-ICDs), though comprehensive and effective, are associated with risks with having leads inside the veins and heart. The emergence of the subcutaneous ICD (S-ICD) offered a solution for patients without a pacing indication, significantly reducing the risks of complications acutely and long term. The absence of pacing and need for specific ECG requirements limit its usa

Primary prevention implantable cardioverter-defibrillators in transthyretin cardiac amyloidosis.

BACKGROUND: Due to the poor long-term prognosis of patients with transthyretin cardiac amyloidosis (ATTR-CA), the role of primary prevention implantable cardioverter-defibrillators (ICDs) in this patient population remains controversial. We aimed to study the impact of primary prevention ICDs on survival in patients with ATTR-CA. METHODS: Among 382 patients diagnosed with ATTR-CA at our institution between 2004 and 2018, 19 had primary prevention ICDs implanted. This cohort was matched in a 1:3 manner on the basis of age, gender, ejection fraction (EF) and ATTR-CA stage with 57 patients witho

Frailty, Implantable Cardioverter Defibrillators, and Mortality: a Systematic Review.

BACKGROUND: Evidence for the benefit of implantable cardioverter defibrillators (ICD) in preventing sudden cardiac death (SCD) in older adults is mixed; age alone may not predict benefit. Frailty may help identify patients in whom an ICD does not improve overall mortality risk. METHODS: Structured search of PubMed, Embase, Web of Science, and Cochrane Central Register of Controlled Trials on 1/31/2019, without language restriction, with terms for ICD, frailty, and mortality. Frailty was defined broadly using any validated single component (e.g., walking speed, weight loss) or multi-component

Adjuvant antiarrhythmic therapy in patients with implantable cardioverter defibrillators.

The risk of sudden cardiac death from ventricular fibrillation or ventricular tachycardia in patients with cardiomyopathy related to structural heart disease has been favorably impacted by the wide adaptation of implantable cardioverter defibrillators (ICDs) for both primary and secondary prevention. Unfortunately, after ICD implantation both appropriate and inappropriate ICD therapies are common. ICD shocks in particular can have significant effects on quality of life and disease-related morbidity and mortality. While not indicated for primary prevention of ICD therapies, beta-blockers and an

Implantable cardioverter-defibrillators, induced anxiety, and quality of life.

Since its approval in 1985, the implantable cardioverter-defibrillator (ICD) has supplanted antiarrhythmic drugs as the standard of care for patients with potentially lethal ventricular arrhythmias. The increased popularity of ICDs stems primarily from their safety and tolerability compared with commonly used medications notorious for adverse drug reactions. As ICD indications have broadened, the number of implantations has increased substantially, and more attention has been directed to sequelae of implantation, particularly after ICD firing. Although scant, studies of quality of life and psy

Implantation of implantable cardioverter-defibrillators from an ileofemoral approach.

Patients needing an implantable cardioverter defibrillator (ICD) system, but without suitable access from jugular or subclavian routes present a vexing problem. Such patients would normally undergo thoracotomy for epicardial lead placement. However, for patients who decline such an intervention, there have been no alternatives for them. There is scarce data from the published literature addressing this dilemma. We report two cases of successful ICD and biventricular ICD placement from an ileofemoral approach.

Implantable cardioverter-defibrillators in children: a single-institutional experience.

BACKGROUND: Implantable cardioverter-defibrillators have been infrequently used in children as therapy for resuscitated sudden death and syncope due to ventricular arrhythmias unresponsive to antiarrhythmics. METHODS: The medical records of 5 children with implantable cardioverter-defibrillators were retrospectively reviewed. All patients had experienced syncope and 3 (60%) an out-of-hospital cardiac arrest. Underlying pathology included hypertrophic cardiomyopathy in 2, long QT syndrome in 2, and ventricular arrhythmia after remote repair of congenital heart disease in 1. Open thoracotomy wi

Cardiac Implantable Electronic Device Therapy: Permanent Pacemakers, Implantable Cardioverter Defibrillators, and Cardiac Resynchronization Devices.

Cardiac implantable electronic devices (CIEDs) provide lifesaving therapy for the treatment of bradyarrhythmias, ventricular tachyarrhythmias, and advanced systolic heart failure. Advances in CIED therapy have expanded the number of patients receiving permanent pacemakers, implantable cardioverter defibrillators, and cardiac resynchronization therapy devices. These devices improve quality of life and, in many cases, reduce mortality. However, limitations remain in the management of patients who require CIED therapy. This article provides a broad overview of CIED therapy in the management of th

Analysis of implantable cardioverter defibrillator therapy in the Antiarrhythmics Versus Implantable Defibrillators (AVID) Trial.

INTRODUCTION: The implantable cardioverter defibrillator (ICD) is commonly used to treat patients with documented sustained ventricular tachycardia (VT) or ventricular fibrillation (VF). Arrhythmia recurrence rates in these patients are high, but which patients will receive a therapy and the forms of arrhythmia recurrence (VT or VF) are poorly understood. METHODS AND RESULTS: The therapy delivered by the ICD was examined in 449 patients randomized to ICD therapy in the Antiarrhythmics Versus Implantable Defibrillators (AVID) Trial. Events triggering ICD shocks or antitachycardia pacing (ATP)

The Psychological Impact of Implantable Cardioverter Defibrillators: A Narrative Review.

Implantable cardioverter defibrillators (ICDs) are critical to the prevention of sudden cardiac death caused by life-threatening arrhythmias such as ventricular tachycardia and fibrillation. While their clinical value is well-established, the psychological impact of living with an ICD remains underrecognized. Patients often experience anticipatory anxiety, depression, Post-Traumatic Stress Disorder (PTSD), and reduced quality of life. These challenges can begin before implantation, persist after both appropriate and inappropriate shocks, and be worsened by fears of device malfunction, recalls,

Evolution of Pacemakers and Implantable Cardioverter Defibrillators (ICDs) in Cardiology.

Pacemakers and implantable cardioverter defibrillators (ICDs) have revolutionized cardiology by providing life-saving interventions for patients with cardiac rhythm disturbances. Pacing the heart is an effective treatment for people suffering from bradycardia caused by sinus node dysfunction or atrioventricular (AV) block, and electronic pacing has saved countless lives since its introduction into clinical practice. AV synchronization is the typical cycle of atrial depolarization and contraction followed by ventricular depolarization and contraction. The continuation of this cycle leads to app

Electrogram morphology discriminators in implantable cardioverter defibrillators: A comparative evaluation.

BACKGROUND: Morphology algorithms are currently recommended as a standalone discriminator in single-chamber implantable cardioverter defibrillators (ICDs). However, these proprietary algorithms differ in both design and nominal programming. OBJECTIVE: To compare three different algorithms with nominal versus advanced programming in their ability to discriminate between ventricular (VT) and supraventricular tachycardia (SVT).
